RESUMO

Water fluoridation is a public health measure to reduce levels of dental caries in populations. A report of the recently completed CATFISH study has been published. This was the first UK evaluation of fluoridation introduction for many years; it was carefully designed and executed and is welcomed. The purpose of this article is to highlight the 180-page report of the study and comment on some aspects to aid interpretation of the findings. Significant features were that two cohorts, from birth and from five years, were followed for six years in a fluoridated and a non-fluoridated area, and clinical and cost analyses were reported. Areas of the report which deserve comment are: a) interruption of fluoridation for a year for half the children in the intervention area and its effect; b) clinical results were reported as absolute reduction in caries prevalence, with the preventive fraction for caries severity downplayed; c) the power of the study was diminished by an unexpected lower caries increment; and d) control for differences in diet was not possible. Nevertheless, this major UK study showed clinical and cost benefits of water fluoridation. The risk of cessation of water fluoridation is highlighted in examples of three recently published studies.

CONTEXT: On December 1, 2020, Drs. Wolfgang Wodarg and Micheal Yeadon petitioned to withhold emergency use authorization of the BNT162b2 messenger ribonucleic acid vaccine for coronavirus disease 2019 (COVID-19) manufactured by BioNTech and Pfizer, raising concern for female infertility risks but acknowledging the lack of evidence. The European Medicines Agency and the US Food and Drug Administration ultimately issued emergency use authorizations, but misinformation claiming that COVID-19 vaccines cause female infertility began circulating on social media, potentially influencing public perception and medical decision making among pregnant patients or those seeking to become pregnant. OBJECTIVES: To determine the potential influence misinformation may have had on public interest in infertility related topics, as analyzed through internet search statistics in the US. METHODS: The Google Trends tool was used to analyze results for the search terms "infertility," "infertility AND vaccine," and "infertility AND COVID vaccine" in the US from February 4, 2020 to February 3, 2021. We applied autoregressive integrated moving average models to forecast expected values, comparing them with actual observed values. RESULTS: At peak interest (100), the forecasted relative search volumes interest for the search terms "infertility," "infertility AND vaccine," and "infertility AND COVID vaccine" were 45.47 (95% CI, 33.27-57.66; p<0.001), 0.88 (95% CI, 2.87-4.63; p<0.001), and 0.29 (95% CI, -2.25-2.82; p<0.001). The actual relative search volumes at peak searching represented 119.9, 11,251, and 34,900% increases, respectively, when compared with forecasted values. CONCLUSIONS: COVID-19 vaccine misinformation corresponded with increased internet searches for topics related to infertility in the US. Dispelling misinformation and informing patients about the risks and benefits of COVID-19 vaccination may prevent unnecessary vaccine hesitancy or refusal, contributing to successful vaccination efforts.

RESUMO

Richard III is known to have had a particular devotion to St Apollonia who is the Patron Saint of sufferers from toothache. The dentition in his skeleton exhumed from a car park in Leicester shows missing teeth which are thought to have been extracted due to caries. It is suggested that Richard, when he was Duke of Gloucester, had prayed to St Apollonia for the relief of toothache before submitting to undergo dental extractions.

Relative to European Americans, type 2 diabetes (T2D) is more prevalent in African Americans (AAs). Genetic variation may modulate transcript abundance in insulin-responsive tissues and contribute to risk; yet, published studies identifying expression quantitative trait loci (eQTLs) in African ancestry populations are restricted to blood cells. This study aims to develop a map of genetically regulated transcripts expressed in tissues important for glucose homeostasis in AAs, critical for identifying the genetic etiology of T2D and related traits. Quantitative measures of adipose and muscle gene expression, and genotypic data were integrated in 260 non-diabetic AAs to identify expression regulatory variants. Their roles in genetic susceptibility to T2D, and related metabolic phenotypes, were evaluated by mining GWAS datasets. eQTL analysis identified 1971 and 2078 cis-eGenes in adipose and muscle, respectively. Cis-eQTLs for 885 transcripts including top cis-eGenes CHURC1, USMG5, and ERAP2 were identified in both tissues. 62.1 % of top cis-eSNPs were within ±50 kb of transcription start sites and cis-eGenes were enriched for mitochondrial transcripts. Mining GWAS databases revealed association of cis-eSNPs for more than 50 genes with T2D (e.g. PIK3C2A, RBMS1, UFSP1), gluco-metabolic phenotypes (e.g. INPP5E, SNX17, ERAP2, FN3KRP), and obesity (e.g. POMC, CPEB4). Integration of GWAS meta-analysis data from AA cohorts revealed the most significant association for cis-eSNPs of ATP5SL and MCCC1 genes, with T2D and BMI, respectively. This study developed the first comprehensive map of adipose and muscle tissue eQTLs in AAs (publically accessible at https://mdsetaa.phs.wakehealth.edu ) and identified genetically regulated transcripts for delineating genetic causes of T2D, and related metabolic phenotypes.

A rapid, inexpensive method using alkoxysilanes has been developed to selectively coat the interior of polydimethylsiloxane (PDMS) microfluidic channels with an integral silicaceous layer. This method combines the rapid prototyping capabilities of PDMS with the desirable wetting and electroosmotic properties of glass. The procedure can be carried out on the open faces of PDMS blocks prior to enclosure of the channels, or by flowing the reagents through the preformed channels. Therefore, this methodology allows for high-throughput processing of entire microfluidic devices or selective modification of specific areas of a device. Modification of PDMS with tetraethoxysilane generated a stable surface layer, with enhanced wettability and a more stable electroosmotic flow rate than native PDMS. Modification of PDMS with 3-aminopropyltriethoxysilane generated a surface layer bearing amine functionalities allowing for further chemical derivatization of the PDMS surface.

OBJECTIVE: To determine whether or not and to what extent health promotion and smoking cessation activities varied between dental practices relative to their National Health Service (NHS)/private treatment mix. METHODS: A piloted questionnaire was posted to all dental practitioners within West Yorkshire, with two follow-up mailings. RESULTS: The response rate was 50% (386/769) of dental practitioners within West Yorkshire. Respondents were dichotomised at the median according to the proportion of NHS/private patients seen (>89% NHS, described as 'NHS-orientated practices' [NHSOPs] and <90% NHS, referred to as 'more privately-orientated practices' [POPs]). Practice profiles: Compared with POPs, NHSOPs' respondents had a significantly higher proportion of adult patients exempt from NHS charges (34% vs 57%), together with a higher proportion of children. Health promotion activities: there was no significant difference between the two groups of practitioners regarding the proportion claiming to give smoking cessation advice (42% vs 37%), although a higher proportion of those from POPs offered guidance on diet and nutrition (67% vs 54%; P<0.05). However, a significantly higher proportion of POP respondents reported (a) recording smoking status in the clinical notes, (b) giving out smoking-related leaflets, and (c) referring to an NHS 'Stop Smoking Service'. NHSOP respondents were more likely to cite 'lack of time', 'no incentive' and 'lack of expertise' as potential barriers to providing health promotion advice. General: 7% of POP respondents thought that the new NHS contract arrangements would influence their smoking cessation activities versus 19% of NHSOP residents. CONCLUSIONS: Considerable variation between NHSOPs and POPs was found when comparing aspects of their health promotion/smoking cessation activities. The findings identified here suggest that the current situation, with regard to the NHS, will tend to increase health inequalities.
