In 2004 the Scottish Executive examined the planning, organisation and delivery of coherent services for those with dementia and their carers. Accompanying this was a planning and audit tool for the design of dementia services.

The term 'intermediate care' has not been widely used in NHS Scotland. That is not tosay that the types of services associated with intermediate care in England and Wales do not exist in Scotland, it is just that the old Scottish Executive Health Department (SEHD) was wary of using the term because it had previously been used to re-label and reinvent existing services ( SEHD 2006 ). However, in the past 18 months, the term has started to become more common. For example, the Joint Improvement Team (established in late 2004 to work directly with local health and social care partnerships across Scotland) has an Intermediate Care Learning Network.
RESUMO
This article's aim is to describe the processes used and results found when conducting an audit of nutritional care within a health board area in Scotland. Nutritional standards and an audit tool were developed by a multidisciplinary team and all in-patient clinical areas took part. The results demonstrate that there is still a considerable amount of work that needs to be undertaken to ensure that patients are getting the most effective and efficient nutritional care and this article denotes some of the initiatives instilled to address areas of identified challenges.

The patient's arm affected by hemiplegia is a common problem following a stroke. The term hemiplegia is a limited physiological description of the effects of a stroke on a patient's limb. Any nurse involved in the care of stroke patients must possess essential knowledge on the pathophysiology of hemiplegia and its subsequent treatment and care management programmes. This article reviews the stroke literature for patients with arms affected by hemiplegia. It describes common complications that may arise and subsequent preventive care management strategies that need to be considered. Finally it discusses why success in rehabilitation and recovery programmes need to be considered from the patient's perspective and measured in the context of the patient's experiences and feelings of well-being.
