RESUMO

We report a case of anterior mediastinal abscess treated by percutaneous drainage under computed tomography (CT) guidance. The relationships of the abscess to the adjacent organs were clearly delineated by an intravenous injection of contrast material, and the drain was inserted between the sternum and the vasa thoracica interna. Selection of this route and the choice of a large-bore catheter (16 F) are discussed.

RESUMO

This interest will consider very closely the present possibilities offered by new therapies an recent methods in order to give our over-aged group the achievement and the benefit of the afore mentioned therapeutic methods. The estimated surgical risk varies from one practitioner to another. The mortality can be reduced by paying close attention to the physical characteristics of that age, to preop preparation, to the type of surgery indicated as well as to the quality of post operative intensive care. The author reviews the cardiovascular, pulmonar, neurologic, mental, kidney, endocrine, and gastro-intestinal insufficiencies.

RESUMO

The results of retrospective series counting 178 cases of surgically resected stomach, duodenum and small bowel benign tumours are analysed. It is a Belgian retrospective multicentric study that extend over a mean duration of 7 years 9 months. The main symptoms are haemorrhage, dyspepsia, obstruction and invagination. The most frequent tumours are leiomyoma, adenomatous polyps, lipoma and schwannoma. Preoperative investigations are endoscopy and radiology. To finish off the gastro-duodenal tumours examinations, endoscopy is necessary and arteriography can be helpful in small bowel tumours. Operative mortality of this series is 2.2%; local and general postoperative complications are 34.2%. Observed local recurrence in 118 patients with mean follow-up of 27 months is 1.7%. The authors emphasize the surgical resection necessity of these tumours as their malignant potentiality is not inconsiderable. Considering local recurrence, malign change and afar dissemination, a long-term follow-up is recommended.

RESUMO

The authors, having clinically observed favourable post operative effects on patients operated under combined general and epidural anaesthesia in digestive surgery, have proceeded to a retrospective and comparative study of the last 50 colic resections. They have observed: a +/- 50% reduction of the quantity of morphine used; a +/- 50% reduction of the time required for peristaltic resumption; a +/- 50% reduction of the time required for gastric intubation thereby contributing to carefree post-operation effects in colic surgery where a series of known complications are usually to be expected. They are of opinion that the benefit of epidural anaesthesia is due essentially to the preponderance of the para sympathetic nervous system allowing the release of some hormones with intestinal tropism as well as the disappearance of the negative action of the splanchnic vaso-constriction.

RESUMO

A pelvic pain is sometimes extremely difficult to define and to treat. The origin of this pain was said to be due to either neurological, psychosomatic, urological or orthopaedic factors. It was in fact an unilateral fibrosis of the perineal transverse muscle. A surgical section has definitively cured the patient.

RESUMO

The specific properties of Stomahesive tested in 116 stomal and 20 fistulous patients render it really useful in stomatherapy where it increases the comfort of the patients: 1. it offers an excellent peristomal skin protection whatever the nature of the stoma or origin of the fistula; 2. it promotes rapid healing of skin lesions, even in ileostomy, transverse colostomy, ureterostomy and fistula patients; 3. it ensures an effective degree of impermeability; 4. it provides a large base for adhesion of the collecting bag; 5. it can be kept on for about 6 days in most cases; 6. The local tolerance is excellent (Acta chir. belg., 1977, 76, 533-537).
