RESUMO

The epidemiological situation in Armenia is very grave: the tuberculosis cases registered in 1994 are 19.6 per 100,000 inhabitants, this is 16.6% higher than the figure in 1993. Mortality rates in 1994 are 3.1 per 100,000 persons. Morbidity in males is 2.5 times higher than that in females. The greatest proportion of the first cases detected is in the age group of 25-34 years. Infiltrative tuberculosis with multiple decay and its acute course is prevalent in the pattern of tuberculosis morbidity. Activities in prophylactic measures drastically has declined due to power crisis and grave socioeconomic conditions of the republic. Groups of migrants as potential carriers of undiagnosed tuberculosis forms are gaining a great significance among patients with tuberculosis.

RESUMO

The function of the pancreatic incretory apparatus was studied in 51 patients with primary active pulmonary tuberculosis by analysing the blood levels of immunoreactive insulin, C peptide and glucose before and after glucagon stimulation. A pronounced enhanced insulin secretion was found. At the same time signs of relative insulin deficiency appeared as persistent hyperglycemia and apparently delayed concentration peaks of immunoreactive insulin and C peptide. Prednisolone therapy caused a noticeable, but rapidly reversible aggravation of relative insulin deficiency. Relative insulin deficiency coupled with the higher secretory function of the pancreatic insular apparatus which decreased its functional reserves with increases in the disease duration is likely to underlie the more frequent development of severe diabetes mellitus in patients with pulmonary tuberculosis.

RESUMO

Clinical symptoms and running of tuberculosis of the lungs were compared for matched patients with the presence of borderline disturbances of carbohydrate metabolism (group 1) or their absence (group 2). Group 1 patients were distinguished for more acute onset, severe intoxication, rapid development of advanced bilateral lesions with multiple destructions. Side effects in group 1 were more frequent and chemotherapy results worse than in control group 2. It is emphasized that derangements of carbohydrate metabolism should be verified to make adequate corrections in treatment programs of tuberculosis.
