RESUMO

Lower gastrointestinal bleeding (LGIB) can be caused by a variety of causes. Pseudoaneurysms have been described as a rare etiology of LGIB and are associated with pancreatic pseudocysts that involve adjacent vasculature. Our study describes a 38-year-old man with recent severe coronavirus disease 2019 and necrotizing pancreatitis presenting with hematochezia and blood clots by gastrostomy-jejunostomy. Initial flexible sigmoidoscopy did not elicit an etiology for the LGIB. Recurrent hematochezia prompted colonoscopy and angiography, which demonstrated a pseudoaneurysm in the marginal artery of Drummond as the source. Our case highlights the importance of repeat evaluation of gastrointestinal bleeding of unknown etiology.

RESUMO

Jaundice can be caused by a wide variety of disorders in adults including pancreatic malignancy. Most pancreatic malignancies are primary in origin. However, other malignancies, including lymphomas, may present as secondary pancreatic masses, with a variety of clinicopathologic features and outcomes for patients. This study describes a patient with previous tobacco use who was diagnosed with a pancreatic mass and found to have diffuse large B-cell lymphoma.

RESUMO

Nocardiosis is caused by various strains of the genus Nocardia, a gram-positive, acid-fast bacillus found in organic matter. Disseminated nocardiosis with central nervous system involvement is a rare infection that is most commonly seen in immunocompromised patients. We present a case of a 67-year-old veteran with stage IV adenocarcinoma of the lung undergoing chemotherapy who presented with new cavitary pulmonary lesions. Three weeks into his hospitalization, a sputum culture with acid-fast staining returned positive for Nocardia cyriacigeorgica. A subsequent computed tomography scan of the head demonstrated numerous rim-enhancing lesions presumed to be due to disseminated nocardiosis with central nervous system involvement. Disseminated nocardiosis has a high mortality rate, making prompt detection and treatment paramount for these patients. Our case highlights the importance of considering central nervous system involvement in the evaluation of a patient with nocardiosis.

RESUMO

Hemoglobin A1c (HbA1c) is an important tool for diagnosis and management of patients with diabetes mellitus (DM). However, hemoglobin variants can interfere with laboratory assays and lead to inaccurate results. This study describes a patient who was found to have falsely elevated HbA1c values secondary to hemoglobin Wayne, a rare hemoglobin variant. Although hemoglobin Wayne is often clinically silent, falsely elevated HbA1c results could lead to unnecessary medical interventions that could cause patient harm. This variation in results highlights the importance of utilizing additional measurements such as glucose readings and evaluating for hemoglobin variants when results are discordant.
