RESUMO

On the basis of an analysis of published data, the authors draw the attention of medical circles to generally- and little-known facts of using a variety of general and local anesthesia types by Russian doctors. It is pointed out that not the very first application based on a calendar date of using a method but the first contribution made into the medical science and supported by efforts promoting such method should be regarded as genuine priority. Prominent medical figures are listed, i.e. N.I. Pirogov, S.K. Klikovich, N.P. Kravkov, S.P. Fyodorov, V.K. Anrep, I.N. Katsaurov, A.V. Vishnevsky, whose priority in certain anesthesia variations, as the authors believe it, is indisputable.

RESUMO

Monitoring of cerebral evoked potentials during total anesthesia provides objective data on the depth of anesthesia. Somatosensory and auditory evoked potentials (SSEP and AEP, respectively) were recorded in 48 patients (ASA physical status 1-2) before and during surgery. AEPs were assessed using a two-stimulus oddball paradigm under passive ignore conditions. Decreased amplitudes and increased latencies of mid-latency SSEP were regarded as decreased conduction of pain pulse via specific and nonspecific pathways. The amplitude of late-latency AEP indicated the level of consciousness during total anesthesia. The removal of evoked potentials was followed by development of painful reaction. Registration of cerebral evoked potentials helps evaluate the depth of anesthesia and intraoperative memory in patients subjected to total anesthesia.

RESUMO

Immunity system changes and intoxication characteristics were studied in two groups of patients with pemphigus treated with traditional methods (group 1, 14 patients) and routine therapy combined with hemoperfusion, plasmapheresis, and enterosorption (group 2, 12 patients). During exacerbation pemphigus patients develop manifest changes in the immunity status in the presence of endotoxicosis. Introduction of hemoperfusion, plasmapheresis, and enterosorption in the complexes of treatment of pemphigus patients activates erosion epithelialization, helps sooner reduce the doses of corticosteroids whose total dose is reduced by 39 percent, improves cell-mediated and humoral immunity parameters, and is conducive to a more favorable time course of the body intoxication parameters.

RESUMO

The experience of using sombrevin anesthesia in 3644 cases and its 3 severe complications which resulted in the development of terminal states up to the clinical death (reanimation was successful) is analyzed. This anesthesia is shown to be necessary under conditions providing a possibility of immediate fulfillment of the complex of highly skilled reanimation measures, with early solution of problems of succession between the polyclinic link and the reanimation hospital. Special attention is called to strict observation of the principle of the preliminary (the day before operation) examination by an anesthesiologist of the patients who are to be subjected to planned short operative interventions, for the individual choice of the method of anesthesia and prevention of severe complications after narcosis.
