An extension of the previously proposed model of Mg metabolism (Robson et al. 1997) has been developed to consider the transactions of Mg that are associated with cerebrospinal fluid (CSF) and bone. The representation of the CSF as a single MG compartment with uptake from the plasma described by Michaelis-Menten kinetics gives very good agreement with published experiments. Analysis of the available information on resorption of Mg from adult bone indicated that this process makes a negligible contribution to Mg homeostasis and can be omitted from the model.

Balloon valvuloplasty has been shown to be a relatively safe and effective intervention in palliating the symptoms of severe calcific aortic stenosis in the elderly [1-7]. Although the mechanism of balloon valvuloplasty is plastic deformation and fracture of the calcified valve tissue [2], complications such as embolic events or acute myocardial infarction due to aortic balloon valvuloplasty are very rare. Previously published reports mentioned only one case of cerebral embolism [5] and two patients with late fatal myocardial infarction [4]. In this report we present an elderly patient with calcific aortic stenosis who developed an acute lateral myocardial infarction during percutaneous balloon aortic valvuloplasty.

We have provided an introduction and brief overview covering a new field of enteral nutrition and an important topic for research and patient care. Several advances have been developed and we must seize the opportunity to exploit new opportunities. Also, the endocrinology, biochemistry, and physiology of the gastrointestinal tract can be better understood in terms of response to enteral feeding. This "nutritional medicine" has potential to reduce the severity of illness, hasten recovery and agility, and restore quality to life.
