OBJECTIVE: To carry out a clinical and radiological assessment of the central vein sign (CVS) as a diagnostic marker for multiple sclerosis (MS) and other demyelinating and non-demyelinating diseases with focal brain damage, using clinical and laboratory examination data, as well as MRI. MATERIAL AND METHODS: The results of clinical and neuroradiological examination of 107 patients diagnosed with MS or with other diseases accompanied by focal brain damage according to MRI data were analyzed. RESULTS: CVS is a sensitive but low-specific diagnostic marker of MS. According to our data, the sensitivity and specificity of 40 and 50% of the threshold of perivenular lesions in the diagnosis of MS are the same and amount to 100% and 39.4%, respectively. Neither the type of MS course, nor the severity of the course, nor the intake of DMT (disease modifying treatment), affect the proportion of foci with CVS. The spread of the proportion of foci with CVS in patients with MS was 60-100%. The proportion of foci with CVS is below 40 and 50% of the threshold in patients with demyelinating and non-demyelinating diseases (NMOSD, migraine, systemic lupus erythematosus, Susak disease, CLIPPERS), which allows for differential diagnosis with MS. The proportion of foci with CVS comparable to MS in patients with acute disseminated encephalomyelitis, small vessel disease, as well as in patients with radiologically isolated syndrome does not allow using this symptom in the differential diagnosis of these conditions. CONCLUSION: The use of CVS as a diagnostic marker of MS is possible only in combination with the already existing diagnostic criteria of MS.

RESUMO

This article reviews the literature concerning the possibility of using magnetic resonance imaging (MRI) with different field strengths to detect the central vein sign (CVS). CVS is a relatively new and promising symptom of multiple sclerosis (MS). This symptom is a parenchymal vein visualized with the help of special MRI modes, which is localized in the focus of demyelination. Studies show that CVS has a high sensitivity and specificity in the diagnosis of MS, allowing the differentiation from other demyelinating, vascular, systemic diseases. However, most studies in this direction have been done using different MR-sequences and on tomographs with different field strengths, so the purpose of this review is to summarize and compare the results of various studies, which may be of practical benefit to clinicians.

Evolution of magnetoacoustic (MA) waves in the heat-releasing plasma is analyzed. Due to the temperature and density dependence of the heating and cooling processes, the dispersion properties of MA waves in the considered medium is rather specific. The dispersion of phase velocity can be either positive or negative, and waves can be further damped or amplified. The amplification of MA waves takes place in the case of isentropic instability. In order to analyze waves in such a medium, we use an approach based on an analogy between nonequilibrium relaxing gas and heat-releasing plasma. The uncompensated isentropic instability restricts the applicability of linear equations describing evolution of magnetoacoustic waves. It appears that for a stabilization of the isentropic instability to be reached, the inclusion of quadratic nonlinear terms is sufficient. In the current research, we derive the nonlinear magnetoacoustic equation (NMAE), which can describe evolution of fast and slow MA waves. The obtained nonlinear equation is different from the known analogues used for the analysis of waves in the considered type of medium, which are modifications of Korteweg-de Vries or Burgers equations. In contrast to the known analogues, it is obtained without the restrictions on wave spectrum and takes into account the main features of nonadiabatic processes that affect the formation of stationary wave structures. We describe analytical solutions of this equation in the form of shock waves including the self-sustained (autowave) pulse and investigate the dependence of these waves on the direction and magnitude of the external magnetic field. The evolutionary stability of the obtained structures is confirmed with the help of numerical solutions of the NMAE. The applicability of NMAE and the correctness of its solutions have been confirmed by the numerical solution of the initial system of magnetohydrodinamic equations. It is shown that the self-sustained (autowave) pulses, which may be realized only in the case of isentropic instability, completely recover their shape after the collision.

Reaction of mast cells of adult male Wistar rats (n=15) in the zone of polypropylene mesh fixation was studied by histochemical, immunohistochemical, and traditional morphological methods on days 1, 5, 10, and 30 after implantation. Immediately after the intervention, mast cells stimulated the processes aimed at wound healing. Secretion of mast cells was clearly regulatory. These cells migrated to the zone of injury for subsequent activation of their function. The number of cNOS+ mast cells near the polypropylene mesh was maximum on day 1 and the number of iNOS+ mast cells peaked on day 5 of the experiment, which probably represented a compensatory reaction. Presumably, stimulation of fibrillogenesis was largely due to the activatory effect of mast cells on the fibroblast function, but not to collagen production by these mast cells.

AIM: To evaluate the effect of various methods of thoracoplasty on local production of cytokines in patients with pulmonary tuberculosis. MATERIAL AND METHODS: The main group consisted of patients after traditional thoracoplasty. Thoracoplasty was compared with endoprosthesis of polypropylene mesh 'Surgipro - SPMM-149'. TNF-α and IL-10 cytokines in exudate from surgical area were measured by using of solid-phase enzyme immunoassay (OOO 'Vector-Best', Russia). RESULTS: It was revealed that local production of pro- and anti-inflammatory cytokines is more pronounced after thoracoplasty with polypropylene mesh 'Surgipro - SPMM-149'. Local cytokine balance changes towards inflammation were more pronounced after thoracoplasty with mesh implant and did not depend on the degree of drug sensitivity.

This paper presents an explanation based on torsionally mediated proton-spin-overall-rotation interaction for the observation of doublet hyperfine splittings in some Lamb-dip sub-millimeter-wave transitions between ground-state torsion-rotation states of E symmetry in methanol. These unexpected doublet splittings, some as large as 70 kHz, were observed for rotational quantum numbers in the range of J = 13 to 34, and K = - 2 to +3. Because they increase nearly linearly with J for a given branch, we confined our search for an explanation to hyperfine operators containing one nuclear-spin angular momentum factor I and one overall-rotation angular momentum factor J (i.e., to spin-rotation operators) and ignored both spin-spin and spin-torsion operators, since they contain no rotational angular momentum operator. Furthermore, since traditional spin-rotation operators did not seem capable of explaining the observed splittings, we constructed totally symmetric "torsionally mediated spin-rotation operators" by multiplying the E-species spin-rotation operator by an E-species torsional-coordinate factor of the form e(±niα). The resulting operator is capable of connecting the two components of a degenerate torsion-rotation E state. This has the effect of turning the hyperfine splitting pattern upside down for some nuclear-spin states, which leads to bottom-to-top and top-to-bottom hyperfine selection rules for some transitions, and thus to an explanation for the unexpectedly large observed hyperfine splittings. The constructed operator cannot contribute to hyperfine splittings in the A-species manifold because its matrix elements within the set of torsion-rotation A1 and A2 states are all zero. The theory developed here fits the observed large doublet splittings to a root-mean-square residual of less than 1 kHz and predicts unresolvable splittings for a number of transitions in which no doublet splitting was detected.

BACKGROUND: It is proved that levosimendan administration improves overall outcome and reduces mortality in high risk cardiac patients. However up to now there is no optimal scheme of its use in intraoperative settings. THE AIM: To compare two approaches of levosimendan administration in patients with left ventricle ejectionfraction less than 35% who underwent cardiac surgery. MATERIALS AND METHODS: After approval by the local ethics Committee, 40 patients older than 18 years with severe preoperative left ventricular dysfunction (left ventricle ejection fraction less than 35%), who were planned for cardiac surgery operation with cardiopulmonary bypass and cardioplegia were randomly assigned to two groups of 20 patients each. In group I (n=20) infusion of levosimendan started intraoperatively after induction of anesthesia, a loading dose of 6 mg/kg with subsequent injection of a dose of 0.1 µg/kg/minfor 24 hours. In group 2 (n=20) bolus of levosimendan 24 µg/kg was injected 15 min before aortic clamping. Anesthesia and methods of cardiopulmonary bypass in the groups did not differ Results. We got thefavorable clinical results in a group of 'full" levosimendan dose (12.5 mg as a daily infusion with an, initial bolus of 6 mg/kg just after the induction of anesthesia) in terms ofreduction of the total dose ofsympathomimetics andpostoperative troponin T level in comparison with the group where levosimendan was administered as a single bolus. (24 mg/kg) 15 minutes prior to aortic clamping. CONCLUSION: Based on these data it can be assumed that the use of prolonged infusion of levosimendan in a dosage of 12.5 mg is preferable to a single bolus 24 µg/kg.

RESUMO

The results of the ultrastructural study of the epithelium of the patient cervix infected by the human papillomavirus (HPV) types 16 and 18 before and after treatment by contrasting thermo-laser therapy (CTLT) are presented. It was shown in this work that 1.5 and 6 months after treatment HPV DNA was not detected in the biopsy and the smear of the cervix using the polymerase chain reaction (PCR). In the ultrathin sections, the structure of the epithelial cells from the biopsy after treatment corresponded to norm. There was effective elimination of HPV types 16 and 18 as Induces by CTLT method.

AIM: Study the influence of low temperature (cold) electrolyte plasma (CEP) on survivability of some mycoplasma strains growing in agar as well as mycoplasma that most frequently contaminate transplantable human cell lines of normal and malignant origin with the aim of decontamination. MATERIALS AND METHODS: Mycoplasma hominis, Mycoplasma arginini and Aholeplasma laidlawii grown in agar and mycoplasma that contaminated transplantable human cell lines of normal (MT4) and malignant (HeLa) origin. Plasma source--Plasmatom device that generates CEP at normal atmosphere pressure and environment temperature. Exposure to plasma was carried out with adherence to the same modes for all the variants of biological substrate. The duration of exposure was selected randomly from 15 to 300 seconds. RESULTS: A pronounced bactericidal effect of high doses of CEP on all the tested mycoplasma variants exposed immediately after seeding into agar was shown. However after a passage a residual number of survived colonies was registered. Passage of colonies exposed in grown state even to high doses of CEP also showed survival of a residual number of bacteria in all the tested mycoplasma species. Exposure of M. hominis immediately after seeding to low doses of CEP resulted in formation of unusual mini-colonies identical to those isolated from humans infected by the same mycoplasma. During microbiological seeding into agar of cultural fluid from 2 spontaneously contaminated strains of transplantable human cells and exposed to CEP growth ofmycoplasma was not detected. CONCLUSION: CEP has pronounced bactericidal properties on various mycoplasma strains growing in both agar and contaminating eukaryotic cells. However even at high doses of exposure to CEP an insignificant part of bacterial cells growing in agar still survives. This may indicate a high degree of heterogeneity and adaptation of mycoplasma subjected to even such hard exposure as cold plasma with plasma-chemical mechanism of destruction of biological substrate.

A model of spin relaxation of Kondo lattices is proposed to explain the angular dependence of the electron spin resonance (ESR) parameters in the heavy fermion compounds Y bIr(2)Si(2) and Y bRh(2)Si(2). A perturbational scaling approach reveals a collective spin motion of Yb ions with conduction electrons in the bottleneck regime. A common energy scale due to the Kondo effect regulates the temperature dependence of different kinetic coefficients to result in a mutual cancelation of all divergent parts in a collective spin mode. The angular dependence of the ESR intensity, linewidth and resonant frequency is shown to be in good agreement with experimental data on Y bIr(2)Si(2) and Y bRh(2)Si(2). In particular, the unexpectedly weak dependence of the ESR intensity on the orientation of the microwave magnetic field agrees with the properties of the discussed model.

RESUMO

Elimination of types 16 and 18 human papilloma virus from the surface of cervix uteri for secondary prevention of cervical cancer was evaluated. The method is protected by patent of invention of the Russian Federation. Infrared laser therapy of cervix uteri was carried out in patients with precancer diseases of cervix uteri at Department of Gynecology of Municipal Clinical Hospital No. 52 (Moscow). Papillomavirus infection was eliminated using a Russian diode laser (lambda=1.06 µ, radiation power 10 W) with a collimating headpiece using carbon die at a distance of 10-12 cm from the exposed surface. The treatment resulted in a high percentage of elimination of types 16 and 18 oncogenic virus 4-6 weeks and during delayed periods after exposure.

RESUMO

The action of a current in the radio frequency range with a periodic impulse mode of modulation on the activation of recovery processes in the skin and skeletal muscles has been studied. The action of a radio frequency current with a power of 1 W, as opposed to that of the weaker action (0.1 W) and stronger (4 W) action, leads to the activation of recovery processes in the skin and skeletal muscles. Recovery processes are manifested in the increase in proliferation and activation of angiogenesis in the skin, and also in formation of new muscle fibers. Recovery processes in muscles are accompanied by activation and migration of satellite cells of muscle tissue in the zone of action of the radio frequency current.

A general method for the preparation of biodegradable microcapsules capable of antigen inclusion is suggested. Multilayer microcapsules were obtained by the method of level-by-level sorption of various polyelectrolytes (alginate, poly-L-lysine, kappa-carrageenan, and chitosan and dextran derivatives). High inclusion efficiency was found for protein and plasmid DNA (no less than 90%). A series of microcapsules with included pTKShi plasmid that incorporated a genome site encoding the E(2) polypeptide of the classic pig plague virus were obtained for carrying out in vivo experiments. It was shown that introduction to mice of microcapsules with the included pTKShi plasmid induced an immune response. The highest antibody titers of the mouse blood sera were obtained in immunization by microcapsules based on the modified dextran/carrageenan and modified chitosan/carrageenan. The method of antigen inclusion into biodegradable microcapsules could be used for the development of encapsulated vaccines of a new generation (DNA vaccines).
