RESUMO

OBJECTIVE: To perfect and estimate the possibilities of using a mini-invasive diagnostic procedure under magnetic resonance (MR) guidance in the complex anatomic situations in cases of pathological small pelvic changes of unknown origin. MATERIAL AND METHODS: Ten small pelvic interventions were made under MR guidance. The indications for bone biopsy under MR guidance were poor visualization of bone changes or their absence during radiography, including computed tomography. Small pelvic organ and soft tissue biopsies were carried out in the situations of complex location of pathological changes adjacent to critical organs, large vascular and neural structures. All interventions were done using a high-field MR scanner. Freehand biopsy was performed in a stepwise fashion under axial and sagittal T2-weighted image control. RESULTS: Informative histological material was obtained in 100% of cases. No complications were observed. CONCLUSIONS: It became possible to verify the diagnosis based on morphological findings and to timely determine management tactics only by target biopsy under MR guidance in these 10 patients with small pelvic changes of unknown origin.

RESUMO

Comparison of the diagnostic importance of traditional US and low-field MRI (0,23 T) in the characterization of solid tumors of internal reproductive system at 55 patients is performed by calculation of operational characteristics of tests and use of characteristic curves (ROC-analysis). Advantage MRI in distinguishing between of benign and malignant tumors (accuracy of 82% against 71% at ultrasonic) is established. Advantage is based on use, besides morphological criteria of a tumor, the characteristic of its MR-signal in T2-weight imaging. Advantage MRI in determination of the organ of origin of pelvic mass appeared insignificant.

RESUMO

Diagnostic significance of MR imaging used for cervical carcinoma staging was established during the examination of 71 patients with verified tumors stage I-III. These findings were compared with the clinical and surgical data. MRI procedure proved highly effective in detecting uterine body involvement, identification of hydroureter caused by the ureters being squeezed by parametrial infiltrates as well as reliable exclusion of parametrial infiltration of bladder wall invasion. It is instrumental in assessing involvement of tissues around the uterus and regional lymph node condition.

RESUMO

The article analyses the results of medical rehabilitation of 176 patients with defects in the leg bones complicated by chronic osteomyelitis by methods of transosseous osteosynthesis. In average hospitalization terms of 380.8 +/- 15.6 days, medical rehabilitation was achieved in 88.6% of patients. The results of complex clinical and laboratory examination of 46 patients by means of angiographic, radionuclide, and physiological methods confirm gradual correction of the initial regional arterial insufficiency which was combined with anatomical and physiological rehabilitation of the affected leg by the end of treatment. It was established that the high clinical result of the developed methods of transosseous osteosynthesis is based on the formation of a new zone of the vascular channel in the affected limb segment.
