RESUMO

Clinically healthy diurnally active young adult women were studied during the same season (March) at the Universities of Kyushu (Fukuoka City, Japan) and of Minnesota (Minneapolis, U.S.A.), under comparable conditions, except that the habitual diets were not changed. The subjects (20 Japanese and 16 Americans of mixed Caucasian background) were studied over a single 24-hr span. Urine was collected at 4-hr intervals. A circadian rhythm in total urinary norepinephrine excretion showed similar characteristics in Japanese and Americans. In epinephrine excretion, the Japanese women showed a statistically significantly higher amplitude with higher peak values, but no statistically significant difference in the rhythm-adjusted mean. This intergroup difference is strictly time dependent; it does not come to the fore in urine samples covering the nocturnal rest span of the subjects.

RESUMO

A new fluorescent polarization immunoassay (Abbott Laboratories, Diagnostics Division, North Chicago, Ill.) was compared with a standard radioimmunoassay (American Diagnostics Corp., Newport Beach, Calif.) in 34 patients being treated with vancomycin. A total of 123 serum samples were divided and quantitatively analyzed for vancomycin by both assay methods. The results obtained indicated that the two assay methods are comparable (y = 1.01x - 0.81; r = 0.99).

Radioimmunoassay (RIA) and fluorescent polarization immunoassay (FPI) methods for quantitative gentamicin serum concentration assay have been shown to be comparable. The purpose of this study was to determine if serum concentration-time data from the same patients assayed by RIA and FPI would provide the same estimates for half-life, elimination rate constant, distribution volume, drug clearance, and gentamicin dose. A total of 99 pre-and postinfusion serum samples were obtained from 30 patients. Samples were divided and assayed by RIA or FPI, and the resultant serum concentration-time data were fitted to a standard one-compartment model. The correlation between the two assay methods was 0.99 (p less than 0.005). A mean difference of 10% was seen in distribution volume, gentamicin clearance, and gentamicin dose from quantitative data from the two methods. These differences were significant (p less than 0.01). Although the two methods appear to be interchangeable, based on in vitro comparison, differences in calculated pharmacokinetic parameters resulted in significant differences in dose recommendations.

Thirteen circadian rhythms in plasma hormone levels (ACTH, aldosterone, cortisol, C-peptide, insulin, DHEA-S, estradiol, LH, prolactin, 17-OH progesterone, testosterone, T4 and TSH) were studied in April 1981 in 25 males and 25 females 57 to 91 years of age, institutionalized at the Berceni Hospital for the aged. The radioimmunoassays and the statistical rhythmometric evaluation by the cosinor procedure were done at S. Paul-Ramsey Medical Center, St. Paul, MN, USA. Circadian rhythms were found and quantified for each of these variables. Elderly subjects of both sexes thus maintain a circadian time structure of their endocrine system as a group phenomenon. In comparison with previous data from the Endocrine Rhythms Laboratory ("C. I. Parhon" Institute) and series of younger subjects studied in Minnesota (St. Paul-Ramsey Medical Center) as well as in comparison with data published from other centers, the aged seem to experience an earlier arousal of their endocrine system which may be related to certain disturbances of old age e.g. of sleep. The latter observations will have to be confirmed by additional studies which take both circadian and circannual variations in account.
