Introduction: Antiresorptive drugs (AR) have been used for many years in the treatment of various bone conditions such as osteoporosis, osteopenia, Paget's disease, bone metastases, multiple myeloma and the associated malignant hypercalcemia. As a side effect of AR therapy, medication-related osteonecrosis of the jaw, which affects the mandible more commonly than the maxilla, (MRONJ) has become an increased risk regarding patients' health and quality of life. The incidence of osteonecrosis has increased significantly in the last few years. One of the main methods of the disease prevention is the education of patients and doctors of dental medicine (DDMs). This is evidenced by the national program of information and prevention of antiresorptive therapy side effects, which was also the impetus for this study. Purpose: This study aims to test the knowledge of DDMSs on AR, especially on bisphosphonate (BF) therapy, MRONJ as well as on the risk factors of the disease itself. Material and methods: 458 DDMs from the Republic of Croatia participated in the survey and responded by an anonymous questionnaire to questions about the knowledge of AR/BF and the risk of MRONJ. Results: The results showed that 36.68% of DDMs do not know that MRONJ is the main complication of AR/BF therapy. The results are significantly different in terms of academic degree, specialization, workplace, and work experience. 60.26% of respondents do not know the main indications for AR/BF use, 53.26% do not know the factors affecting the onset of the disease, and 42.58% do not know which therapy is not recommended for patients on AR/BF therapy. 93.89% of respondents expressed the desire to educate on this issue. This current study was carried out to further investigate the pilot study findings which was conducted in the year 2015, but it had a significantly lower number of participants. Conclusion: This research suggests that further education of DDMS on this topic is necessary to prevent or to start early treatment of MRONJ.

The data about bond strength between digitally produced denture base resins and artificial teeth are scarce. Several studies investigated shear bond strength values of milled denture base resins and different types of artificial teeth. The purpose of the present study was to compare and evaluate the available evidence through a systematic review. A bibliographic search was conducted in PubMed, Scopus, and Web of Science to assess adequate studies published up to 1 June 2022. This review followed the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. The appropriate studies that determined the shear bond strength values between milled denture base resins and artificial teeth were selected. The initial search identified 103 studies, which were included in the PRISMA 2020 flow diagram for new systematic reviews. Three studies met the inclusion criteria, and all of them present a moderate risk of bias (score 6). Two studies found no statistical differences between heat-polymerized and CAD/CAM (milled) denture base materials when attached with different types of artificial teeth, while one study showed higher values of CAD/CAM (milled) denture base materials. Bonding agents ensure bonding strength at least similar to the conventional methods. In order to improve the quality of future studies, it would be advantageous to use a larger number of specimens with standardized dimensions and a blinded testing machine operator to decrease the risk of bias.

This study aimed to investigate the effects of radiotherapy on the mechanical, chemical, and surface properties of two recently introduced restorative dental materials (a glass hybrid and an alkasite), while two conventional restorative materials served as references. Material specimens of the experimental groups (irradiated) were compared to the specimens of the control groups that underwent the same preparation procedure but without irradiation. The experimental groups of restorative material specimens were irradiated with a total of 70 Gy over 35 days (2 Gy/day × 35 days), while the control groups received no treatment. The following properties were evaluated: surface microhardness (Vickers), surface roughness, color change, flexural strength, flexural modulus, material reliability, and infrared spectra. For the experimental groups, measurements were performed 24 h after specimen preparation, i.e., before radiotherapy and after the completion of the irradiation protocol. For the control groups, measurements were performed after the corresponding periods of no treatment. A statistically significant increase in microhardness (p = 0.001-0.004) and surface roughness (p = 0.013) was observed as a result of material aging/maturation in both the control and experimental groups. However, the only statistically significant difference between the control and experimental groups was observed in the discoloration of the conventional reference material (p < 0.001). In conclusion, no statistically significant negative effects of a therapeutic dose of radiotherapy on any of the tested properties of the alkasite and glass hybrid materials were observed, whereas only a minor negative effect of radiotherapy in terms of discoloration was found for a conventional resin composite that was used as a reference material.

OBJECTIVE: To evaluate the surface of glazed lithium disilicate dental ceramics after irradiation under different irradiation settings of Nd:YAG and Er:YAG lasers using a scanning electron microscope (SEM). MATERIALS AND METHODS: Three glazed-press lithium disilicate ceramic discs were treated with HF, Er:YAG, and Nd:YAG, respectively. The laser-setting variables tested were laser mode, repetition rate (Hz), power (W), time of exposure (seconds), and laser energy (mJ). Sixteen different variable settings were tested for each laser type, and all the samples were analyzed by SEM at 500× and 1000× magnification. RESULTS: Surface analysis of the HF-treated sample showed a typical surface texture with a homogenously rough pattern and exposed ceramic crystals. Er:YAG showed no effect on the surface under any irradiation setting. The surface of Nd:YAG-irradiated samples showed cracking, melting, and resolidifying of the ceramic glaze. These changes became more pronounced as the power increased. At the highest power setting (2.25 W), craters on the surface with large areas of melted or resolidified glaze surrounded by globules were visible. However, there was little to no exposure of ceramic crystals or visible regular surface roughening. CONCLUSIONS: Neither Er:YAG nor Nd:YAG dental lasers exhibited adequate surface modification for bonding of orthodontic brackets on glazed lithium disilicate ceramics compared with the control treated with 9.5% HF.

BACKGROUND/AIM: Published data about orofacial injuries and mouthguard use by professional handball players are scarce. The aim of this study was to investigate the prevalence of orofacial trauma and mouthguard use in professional handball players. MATERIALS AND METHODS: Data were collected from 100 professional handball players through a questionnaire, which contained 17 questions about age, experience in playing handball, playing position, orofacial trauma experience during the past 12 months, type of injury and mouthguard use. RESULTS: Almost half (49%) of the interviewed players experienced head and/or facial trauma during the past year. The most common injuries were soft tissue lacerations (39.6%). Dental injuries occurred in 22% of the participants, with socket bleeding being the most frequent injury (14%). Of the affected teeth, 76.9% were upper incisors. Mouthguards had a statistically significant protective role regarding tooth fractures and tooth avulsion (P=.043). Players who wore a mouthguard had a 5.55 times less chance of suffering dental injuries. Almost 76% of dental injuries resulted in complications afterward. Sixty-seven percentage of the players knew that mouthguards could prevent injuries, but only 28% used them regularly. Of the players who wore a mouthguard regularly, 76.9% were advised to do so by their dentists. CONCLUSIONS: The incidence of head and orofacial injuries among professional handball players is high. Mouthguards prevented severe dental injuries such as tooth fracture and avulsion, but their use was still limited.

RESUMO

This case report describes the management of a crown-root fractured maxillary right central incisor and a crown fractured maxillary left central incisor using two different techniques. A complex procedure was designed to manage this case including orthodontic extrusion to move the fracture line above the alveolar bone and surgical recontouring of the altered gingival margin. Finally, the right incisor was restored prosthodontically. Prosthetic treatment was based on performing a post and core, and all-ceramic crown on the extruded tooth. The left, less-damaged incisor was restored directly using composite resin. The treatment resulted in good esthetics and secured periodontal health. This case report demonstrates that a multidisciplinary treatment approach is a reliable and predictable option to save a tooth.
