Uterine undifferentiated (UC)/dedifferentiated (DEAC) carcinomas are rare malignant neoplasms. They tend to pursue an aggressive clinical course with an advanced stage at presentation. It has been found that androgen receptor (AR) might play a role as a prognostic and therapeutic marker in endometrial carcinoma. However, its expression in UC/DEAC has not been investigated. Herein, the aim of this study was to evaluate the expression of AR along with estrogen receptor (ER), progestin receptor (PR), and HER2 in UC/DEAC and also in other subtypes of high-grade endometrial carcinomas. Review of our pathology database over the period of 2011 to 2019 identified 16 UC/DEAC cases (N=16). We also randomly selected other high-grade endometrial carcinomas including FIGO 3 endometrioid carcinoma (N=9), serous carcinoma (N=8), clear cell carcinoma (N=12) and carcinosarcoma (N=10) for comparison. Immunohistochemical stains for AR, ER, PR, and HER2 were performed on all 55 cases. The protein expression was evaluated both quantitatively and qualitatively. In DEAC cases both the undifferentiated component and the well-differentiated component were recorded separately. Overall, variable degrees of AR reactivity (by Allred scoring method) was present in 63% of UC/DEACs(10/16), 67% of FIGO 3 endometrioid carcinomas (6/9), 88% of serous carcinomas (7/8), 80% of carcinosarcomas (8/10), and 9% of clear cell carcinoma (1/12). AR expression was most often seen with PR (70%) or ER (60%) staining in UC/DEACs. Thirteen cases of UC/DEACs were positive for at least 1 hormone receptor. HER2 was negative in all UC/DEACs. Almost all other high-grade carcinoma cases were negative for HER2 except 20% of carcinosarcoma (2/10) and 13% of serous carcinoma (1/8) which showed 3+ HER2. Loss of AR appears to be associated with worse clinicopathologic parameters in UC/DEAC. AR is highly expressed in UC/DEAC, and in the majority of FIGO 3 endometrioid carcinomas, serous carcinomas, and carcinosarcoma. These findings suggest a potential role for androgen inhibitors in the management of patients with these tumors.

BACKGROUND: Prolonged Mechanical Ventilation (PMV) is associated with a higher cost of care and increased morbidity and mortality. Patients requiring PMV are referred mostly to Long-Term Acute Care (LTAC) facilities. OBJECTIVE: To determine if protocol-driven weaning from mechanical ventilator by Respiratory Therapist (RT) would result in quicker weaning from mechanical ventilation, cost-effectiveness, and decreased mortality. METHODS: A retrospective case-control study was conducted that utilized protocol-driven ventilator weaning by respiratory therapist (RT) as a part of the Respiratory Disease Certification Program (RDCP). RESULTS: 51 patients on mechanical ventilation before initiation of protocol-based ventilator weaning formed the control group. 111 patients on mechanical ventilation after implementation of the protocol formed the study group. Time to wean from the mechanical ventilation before the implementation of protocol-driven weaning by RT was 16.76 +/- 18.91 days, while that after the implementation of protocol was 7.67 +/- 6.58 days (p < 0.0001). Mortality proportion in patients after implementation of protocol-based ventilator weaning was 0.21 as compared to 0.37 in the control group (p=0.0153). The daily cost of patient care for the LTAC while on mechanical ventilation was $2200/day per patient while it was $ 1400/day per patient while not on mechanical ventilation leading to significant cost savings. CONCLUSION: Protocol-driven liberation from mechanical ventilation in LTAC by RT can significantly decrease the duration of a mechanical ventilator, leading to decreased mortality and cost savings.

La brucelosis es una zoonosis que se trasmite directa o indirectamente al hombre, a partir de reservorios animales, y es considerada una enfermedad ocupacional. Con el objetivo de estratificar el riesgo de brucela, para el diseño de un sistema de vigilancia, se realizó una investigación de innovación tecnológica bietápica durante los años 2009-2010 en el municipio San Cristóbal. Durante la primera se estratificó el riesgo de brucela y en la segunda, se diseñó un sistema de vigilancia. Las unidades de análisis para la investigación están constituidas por la totalidad de animales, pertenecientes al sector estatal del municipio, de la cual se abastece (carnes, leche y derivados), la población y la totalidad de trabajadores expuestos ocupacionalmente al riesgo de brucela. La brucelosis es un importante problema de salud entre la masa animal, incide en ello, los bovinos, porcinos y los búfalos, todas las fuentes de alimento de la población, las principales zonas de riesgo para la salud de los humanos se encuentran en los Consejos Populares José Martí y López Peña, considerados estos de alto riesgo epidemiológico para la salud de la población. Se propone un sistema de vigilancia conjunto con medicina veterinaria que garantizará el control de la enfermedad, tanto en los animales como en las personas(AU)

Brucellosis is a very important health problem among the great mass of animals, having an effect on bovines (mainly buffalos) and porcine, all of them foodstuffs of the population. Jose Marti Township Council and Lopez Peña town are the principal zones where the population is at epidemiological risk. A system of disease surveillance, together with veterinary medicine will guarantee the control of the disease within animals and people(AU)

La oncocercosis es la segunda causa de ceguera a escala mundial, después del tracoma, según la Organización Mundial de la Salud. Fue descubierta en América por Rodolfo Robles en Guatemala, en 1915 (enfermedad de Robles); en 1923 en Chiapas y en 1926 en Oaxaca, México. En 1930 se estableció el programa para su control; es el más antiguo del país y ha realizado trabajos ininterrumpidamente hasta la fecha. Se pueden describir tres grandes etapas del programa para el control de la oncocercosis: a) de 1930-1946 se llevó a cabo la lucha antilarvaria con creolina, la eliminación de larvas de las corrientes de agua y la extirpación de nódulos; b) la administración de la dietilcarbamazina en 1947, la extirpación de nódulos y la aplicación de DDT en 1952; y c) en 1993 la eliminación de la enfermedad con el tratamiento con ivermectina y la extirpación de nódulos. Hasta 1980 se observaba una notificación promedio de 20 mil casos anuales pero, a partir de 1993, al iniciar la administración de ivermectina en dos rondas anuales, se redujo a menos de 100 casos nuevos por año para finales del año 2000 y se eliminó la transmisión en dos focos (en el norte de Oaxaca y en Chamula, en Chiapas), aunque todavía permanece en uno (en Soconusco, Chiapas). En el presente artículo nos referimos a la lucha, durante los últimos 17 años, en contra de la oncocercosis y qué nos permite suponer que, en breve, podrá ser erradicada del país.


According to the World Health Organization, onchocerciasis is the second cause of global blindness after trachoma. It was first discovered in America by Rodolfo Robles in Guatemala in 1915 (Robles's disease); in Chiapas, Mexico in 1923; and in Oaxaca in 1926. In 1930, the first control program was established in Mexico that, to date, has worked uninterruptedly. Three stages of the program can be described: a) from 1930-1946 the antilarvae campaign with creolin was carried out along with the elimination of larvae from water and the removal of nodules; (b) administration of diethylcarbamazine in 1947, removal of nodules and application of DDT in 1952; and c) in 1993 the elimination of the disease with ivermectin treatment and the removal of nodules. Until 1980, an average of 20,000 cases have been reported each year. Since 1993, with the initiation of the administration of ivermectin in two annual doses, the incidence was reduced to <100 new cases per year by the end of 2000 and the transmission in two foci (northern Oaxaca and in Chamula in Chiapas) has been deleted, with one remaining in Soconusco, Chiapas. In this article, we report on the campaign against river blindness during the past 17 years and why we assume that, in brief, this disease can be eliminated in Mexico.

La estratificación epidemiológica, permitió identificar los consejos populares del municipio con similitudes en el comportamiento de diversos factores de riesgo, con el propósito de contribuir a la selección de los sitios de mayor riesgo en el incremento de la incidencia de seropositivos alVIH/SIDA y así establecer la vigilancia y control epidemiológico de este problema de salud, además, de aplicar intervenciones de acuerdo con las condiciones de cada estrato, facilitar la planificación y ubicación de recursos. Se realizó un estudio epidemiológico, descriptivo y transversal, para la estratificación de riesgo de los consejos populares en el municipio, sobre la distribución de los factores seleccionados de VIH/Sida, durante el período comprendido entre enero de 2007 al 31 de diciembre del 2009. El universo estuvo integrado por todos los consejos populares del municipio San Cristóbal, en los cuales se compararon las diferencias obtenidas entre los Consejos "casos" y los Consejos "controles", para los factores seleccionados: presencia de contactos con enfermos VIH/ SIDA, presencia de casos con conductas o comportamientos inadecuados, presencia de reclusorios, poca información sobre la enfermedad e inobservancia de normas de bioseguridad. La opinión de expertos fue decisiva a la hora de determinar los estratos epidemiológicos de riesgo, para los cuales, fue implementado un plan de intervención que abarcará medidas que favorecen la participación intersectorial y comunitaria.


The epidemiological stratification allowed identifying the Popular Councils of the municipality with similarities in the behavior of diverse risk factors with the purpose of selecting the sites that presented the highest risks in the increase of seropositive incidence (HIV/AIDS) to establish the epidemiological surveillance and control of this health problem applying health interventions regarding the conditions of each stratus to draw up a plan and to arrange the resources. An epidemiological, descriptive and cross-sectional research was conducted with the purpose of stratifying the risks (HIV/AIDS) in the municipal Popular Councils and distribution of the factors chosen during January 2007 to December 2009. The target group was comprised of the Popular Councils in San Cristobal, where the differences obtained were compared among the "cases" Councils and "control" Councils to the factors chosen: presence of the contacts with HIV/AIDS carrier, cases with misconduct or inappropriate conduct and reclusive behavior, lack of information about the disease and non-observance of the security norms. The opinion of the experts on the subject was conclusive to determine the epidemiological stratus of the risk to establish a plan of intervention considering the measures that favour the inter-sectorial and community participation.

RESUMO

The success of regional networks to provide incentives and mechanisms for sharing information is illustrated through the North American Regional Action Plan on DDT (NARAP). This NARAP was designed to reduce human and environmental exposure to DDT and its metabolites in North America through cooperative efforts. The increasing role of environmental concerns in the planning and design of Mexico's DDT phase-out program motivated the Mexican Ministry of Health to undertake an alternative program for malaria control maximizing community participation without negatively affecting human and ecosystem health. This program relies on: 1) elimination of parasites in people, 2) improvement of personal and household hygiene, and 3) use of environmental management practices to eliminate mosquito breeding sites. This new strategy resulted in the elimination of DDT for malaria control in Mexico in the year 2000. While malaria and its corresponding vectors show considerable variability within a single country and certainly around the globe, the case example of Mexico provides a methodology for developing alternatives to DDT.
