RESUMO

The demand for new healthcare services is growing rapidly. Improving accessibility of the African population to diabetes care seems to be a big challenge in most countries where the number of care centers and medical staff is reduced. Information and communication technologies (ICT) have great potential to address some of these challenges faced by several countries in providing accessible, cost-effective, and high-quality health care services. This paper presents the Mobil Diab system which is a telemedical approach proposed for the management of long-term diseases. The system applies modern mobile and web technologies which overcome geographical barriers, and increase access to health care services. The idea of the system is to involve patients in the therapy process and motivate them for an active participation. For validation of the system in African context, a trial was conducted in the Democratic Republic of Congo. 40 Subjects with diabetes divided randomly into control and intervention groups were included in the test. Results show that Mobil Diab is suitable for African countries and presents a number of benefits for the population and public health care system. It improves clinical management and delivery of diabetes care services by enhancing access, quality, motivation, reassurance, efficiency, and cost-effectiveness.

Being able to manage and adjust insulin doses is a key part of managing type-1 diabetes. Children and adolescents with type-1 diabetes mellitus often have serious difficulties with this dosage adjustment. Therefore, this paper aims to investigate the impact of using novel mobile, web and communication technologies in assisting their therapy and treatment. A trial was conducted in the north-eastern part of Germany to evaluate the impact of the "Mobil Diab", a mobile diabetes management system, on the clinical outcome. 68 subjects aged between 8 and 18 years, divided randomly into control and intervention groups, were included into the study. Metrics such as changes in the quality of metabolic control, changes in psychological parameters, usability and acceptance of the technology were used for evaluation purpose. Metabolic control was mainly assessed by the mean HbAlc. Analysis showed a good acceptance of the proposed system. An overall improvement in mean levels of HbA1c was observed, however further studies will be conducted to prove evidence of the weight and BMI improvements. Moreover, initial indications of positive impact on the improvement in psychological parameters were presumed based on the result of the conducted study. The system appeared to be an efficient and time saving tool in diabetes management.

OBJECTIVE: The increasing number of elderly and chronically ill patients is currently stressing healthcare systems. One of the solutions for reaching an economically and socially viable solution is exploiting the benefits of modern technologies. This article presents an innovative mobile teledermatology program for assisting both the therapy and the aftercare of patients suffering from skin diseases. SUBJECTS AND METHODS: The system is based on store-and-forward teledermatology and consists of two main components: (1) an application implemented on the mobile phone in order to enable patients to transmit both skin image data and biofeedback information and (2) an online portal for the care providers. The key idea is to ensure a sufficient involvement of the patients in the therapy process and to motivate them through their active cooperation. RESULTS: Smartphone applications with features as well as the online Web-based portal for medical care providers are presented. The entire system is embedded on a secure telematics platform. The system was tested at the Clinic for Skin Diseases of the University of Greifswald, Greifswald, Germany, by patients with skin diseases such as ulcus cruris, postoperative wound documentation, eczema, psoriasis, and pemphigus for a period of 1 year. Medical care providers could also analyze patients' data from the developed online Web-based application during that test period. CONCLUSIONS: Positive feedback from both medical care providers and patients confirms a high potential for mobile teledermatology as a means of dermatological healthcare delivery.

Lack of regular physical activity and high stress levels are the leading causes of several illnesses. There is thus a real need for a personal low-cost and mobile monitoring solution over extended periods to prevent health risks. Based on the above fact, this article presents a system capable of estimating and monitoring both stress and fitness levels without a physical consultation of a medical specialist. The system consists of three main subcomponents: a mobile real-time acquisition of physiological as well as subjective data, an expert model for stress and fitness estimations based on physiological signals collected from wireless vital sensors, and a secure and scalable telematics platform on which the entire system is embedded. Features and tasks performed by the telematics platform will be presented. The experimental part of the work involved a representative number of subjects. Results for 110 subjects whose fitness levels were assessed at different periods of the year and 50 individuals whose stress scores were assessed at different times of the day showed a high correlation of the estimated values with the true ones. The application of such a low-cost monitoring system will improve the quality of service in preventive medicine.

A total of 140 obese patients (mean age 14 years) participated in a structured treatment and teaching programme (STTP) in hospital, with the aim of weight reduction. At both the start and finish of the STTP, patients underwent clinical and psychological examination. During an average hospital stay of 35 days, their mean bodyweight decreased from 82.4 kg to 76.0 kg (P < 0.001). Patients were then followed up with a telemedicine support programme. During the next 12 months, the acceptance of the telemedicine support programme declined from 93% to 46%. The body mass index was 30.5 kg/m(2) at admission and 27.7 kg/m(2) at 12-month follow-up (P < 0.05). In parallel, wellbeing and treatment satisfaction increased, and there was a positive effect on eating behaviour and exercise. Intervention was needed in up to 64% of the children and adolescents who participated in the programme, most frequently due to poor results in exercise. Telemedical follow-up care and counselling seemed to be highly effective, and allowed not only an initial weight reduction, but long-term stabilization as well.
