RESUMO

Intracardiac thrombus is an exceptional complication revealing Behçet's disease. We here report the case of a 48-year-old patient admitted with stroke. Clinical examination showed recurrent oral aphthous ulcers. Cardiovascular examination was normal, magnetic resonance imaging (MRI) objectified left posterior parietal stroke. Transthoracic echocardiogram showed totally thrombosed apical aneurysm measuring 42mm/36mm. Total-body computed tomography (CT) scan was performed to detect other abnormalities. It was normal apart from cardiac involvement. The diagnosis of Behçet's disease was made based on oral aphthous ulcers, polyarteritis nodosa and thrombosed left ventricular aneurysm. Outcome was favorable under medical treatment combining anticoagulants, corticosteroids and immunosuppression.

Aortic dissection in the most common fatal disease affecting the aorta. Ascending aortic dissection can lead to coronary malperfusion causing myocardial infarction with ST elevation. The distinction between aortic dissection and a primary myocardial infarction can be difficult because both conditions can have similar presentations. Making the right diagnosis is essential because the therapies used to treat myocardial infarction can be fatal for patients with aortic dissection. Emergency transthoracic echography presents a rapid imaging procedure that provides strong hints of the coexistence of these two diseases, leading to further imaging examination and prevent inappropriate administration of treatments that could cause catastrophic outcome. We report a case of a 62-year-old man admitted to our hospital with chest pain, who was diagnosed as inferior wall myocardial infarction based on electrocardiographic findings. The diagnosis was reassessed due to a significant aortic regurgitation and an intimal tear in the ascending aorta on transthoracic echocardiography. Computed tomography angiogram of the chest and transesophageal echography fully confirmed the presence of ascending aortic dissection. Emergency surgery was successfully performed and the patient recovered well.

RESUMO

Infectious endocarditis is regarded as a potentially serious pathology despite the advances in diagnostic and treatment options. Valves on the left side of the heart are most affected and embolic events, mycotic aneurysms, abscesses as well as valve perforations are life-threatening consequences of this disease. We here report a case of endocarditis with aortic and mitral valve involvement complicated by splenic and renal infarction, cerebral mycotic aneurysm and mitral valve perforation. The interest in the subject of this case study is the positive medico-surgical treatment outcome obtained despite a multitude of complications.
