RESUMO

Paraneoplastic syndromes have a variety of clinical manifestations most frequently resembling systemic diseases and phlebothrombosis. Also, hypercoagulative paraneoplasic syndrome may have some clinical features of myocardial infarction which makes difficult nosological diagnostics.

RESUMO

AIM: To analyse results of treatment in new cardiological critical care units in Moscow (62 beds). MATERIAL AND METHODS: Five years reports presented in 1993-1999 to Moscow Public Health Committee. RESULTS: The activity of the new cardiological units proved efficient: cardiovascular lethality reduced by 20%; quality of diagnosis and treatment improved; bed turnover increased. CONCLUSION: A new system of medical care for critical cardiological patients is recommended for introduction as basic form of organization of urgent cardiological inpatient care in Moscow.

RESUMO

Therapeutic and prophylactic antiarrhythmic efficacy of sotalol hydrochloride (Sotahexal, "Hexal", Germany) and its effects in intracardiac hemodynamics and ECG parameters were evaluated in 95 patients with ischemic heart disease (IHD). The highest response to the drug was observed in ventricular extrasystoles, arterial flutter and fibrillation. Acute episodes of arrhythmia are managed by bolus administration of Sotahexal [correction of Hexal]. In this case greater risk of side effects exists. It is desirable to decide on the drug dose, mode of administration on the individual basis with due consideration of the risks and dangers which could be avoided in case of adequate instrumental control.

RESUMO

Changes in blood rheological properties and central hemodynamic parameters were studied in patients with Functional Classes III-IV angina pectoris who had undergone therapeutical plasmapheresis. Drastic baseline changes in blood rheological properties, a reduction in myocardial contractility, and high total peripheral vascular resistance (hypokinetic circulation) were found to be indications for plasmapheresis supplemented to antianginal therapy. A positive clinical effect of this therapeutical method may be, to a definite extent, predicted in these patients. The first plasmapheresis procedure improved blood rheology due to the plasma component, making the high baseline levels of fibrinogen and total protein normal, lowered increased platelet aggregation, repeated procedures enhanced the electric superficial charge of red blood cells and the stroke index of the left ventricle.

RESUMO

Plasma sorption was performed in patients with Functional Classes III-IV angina pectoris by using the sorbent FAS. A positive clinical effect was achieved in 81%. There was a significant reduction in the levels of total cholesterol and low density lipoprotein cholesterol. The concentration of high density lipoprotein cholesterol remained unchanged.

RESUMO

Patients with angina pectoris, functional classes III-IV, were examined for changes in the rheological blood properties (red blood cell aggregation ratio, fluidity limit, apparent viscosity, electrophoretic mobility of red blood cells, fibrinogen, total plasma protein and ADP, induced platelet aggregation) under the effect of continuous plasmapheresis (PA). PA was established to produce a selective effect on the rheological blood properties depending on the initial level of its disorders. The most remarkable clinical effect was produced by PA in patients with initially high characteristics of hemorheology. The first PA session ameliorated hemorheology at the expense of the plasma component, reduced high platelet aggregation whereas the repeated sessions largely affected red blood cell function.

RESUMO

Red cell and platelet surface membranes, platelet adhesion to glass were examined in coronary patients treated among other treatment modalities with gravitation surgical correction of the blood. Scanning electron microscopy was used in the study. The studies revealed the minimal injurious effect of the extracorporeal contour on blood cells; a manifest improvement of cellular morphology and function was observed after a course of procedures. An attempt was made to assess the mechanism of gravitation surgery effect on blood cells.
