RESUMO

Adonis amurensis is a herb with the effect of cardiotonic and diuretic.Overdose can lead to various bradyarrhythmia and gastrointestinal reaction. This article analyzed the treatment process of 5 patients with bradyarrhythmia caused by adonis amurensis to enhance clinical doctors' understanding of the treatment principles for adonis amurensis poisoning.

RESUMO

Technical Specifications for Occupational Health Surveillance (GBZ 188-2014) has played an important role in screening occupational contraindications and preventing occupational diseases since its implementation. However, during the use of occupational health examination, we found that the use of occupational contraindication on cardiovascular disease was not "homogenized" due to the differences in the understanding of various physical examination institutions. Therefore, this paper mainly discussed the connotation and quantitative standards of organic heart disease, arrhythmia, hypertension in the occupational contraindication cardiovascular disease in the specification for "homogenization".

BACKGROUND: In East Asians, a common method to ameliorate puffy eyelids is to resect the excess orbital fat and the orbicularis. However, in some patients, a satisfactory improvement cannot be achieved because of the thick submuscular fibroadipose tissue (SMFAT) and minimal orbital fat. The SMFAT lies between the orbicularis and the orbital septum and can also be found in the pretarsal area. It is responsible for puffy eyelids and the slackening of the tissue after removal. In this study, we combined an extended SMFAT resection with the ladder suture technique to correct puffy SMFAT eyelids. METHOD: A total of 76 patients with a thick SMFAT and minimal orbital fat were recruited. The thick SMFAT, which included the soft tissue near the artificial fold and the pretarsal area soft tissue, was resected. The ladder suture technique was performed by positioning the stitch 1 mm below the orbicularis margin after a skin flap margin puncture. The esthetic effect was evaluated pre-operatively and at 12 months post-operatively. RESULTS: The pre-operative thickness of the SMFAT and full layers of the eyelid were 0.27 ± 0.03 mm and 2.01 ± 0.11 mm, respectively. One year after surgery, the thickness was 0.08 ± 0.02 mm and 1.68 ± 0.14 mm, respectively (p < 0.05). The post-operative upper eyelash lift-up angle and the eyelash exposure length were significantly increased on both sides compared with the pre-surgery (p < 0.05). No patients experienced complications. CONCLUSION: An extended SMFAT resection combined with the ladder suture technique can effectively correct puffy SMFAT eyelids without complications.

BACKGROUND: Studies suggested that vascular dysfunction might increase the risk of developing Alzheimer's disease (AD), but the underlying mechanisms still remain obscure. OBJECTIVE: To evaluate the associations of vascular risk burden with AD core pathologies and investigate the effects of AD core pathologies on relationships between vascular risk burden and cognitive impairments. DESIGN: The Chinese Alzheimer's Biomarker and LifestyLE (CABLE) study was principally focusing on aging, as well as the risk factors and biomarkers of AD initiated in 2017. SETTING: The CABLE study was a large cohort study established in Qingdao, China. PARTICIPANTS: A total of 618 non-demented elders were obtained from CABLE study. MEASUREMENTS: The general vascular risk burden was assessed by the Framingham General Cardiovascular Risk Score (FGCRS). Multivariate linear regression analyses were performed to evaluate the associations of FGCRS with cerebrospinal fluid (CSF) AD biomarkers and cognition. Casual mediation analyses were performed to investigate the mediating effects of AD biomarkers on cognition. RESULTS: Increased FGCRS was related to higher levels of CSF total tau (t-tau, p < 0.001), phosphorylated tau (p-tau, p < 0.001) as well as the ratio of t-tau and amyloid-ß 42 (t-tau/Aß42, p = 0.010), and lower Chinese-Modified Mini-Mental State Examination (CM-MMSE, p = 0.010) score. Stratified analysis indicated that age modified the associations, with FGCRS being significantly associated with tau pathology (p < 0.001 for t-tau and p-tau) in middle-aged group (<65 years old), instead of older group. The influences of FGCRS on cognitive impairments were partially mediated by tau pathologies (a maximum proportion of 20.9%). CONCLUSIONS: Tau pathology might be a pivotal mediator for effects of vascular risk on cognitive decline. Early and comprehensive intervention for vascular risk factors might be a potential approach to delaying or preventing cognitive impairment and AD.

Ruminating behavior accompanies the development of the rumen and the intake of solid feed in calves. However, few studies have reported on the emergence and development of rumination. In this study, we observed ruminating behavior changes of 56 Holstein calves (body weight at birth = 40.1 ± 3.96 kg; mean ± standard deviation) from birth to 30 d of age under the feeding management of suckling calves that were only fed pelleted concentrate feed and milk. All calves were housed in individual pens equipped with infrared cameras. We explored feed intake within 30 d of age, body weight on 61 d of age, and other apparent indicators, including the age of first eating the bedding, duration of non-nutritive oral behavior at 25 and 30 d of age, total starter feed intake within 30 d of age, average daily starter feed intake within 30 d of age, and duration of ruminating behavior at 25 and 30 d of age for all calves, to further explore the effects of the age of first ruminating behavior (AFR). The AFR fitted the normal distribution and ranged from 15 to 20 d of age for 50% of the experimental population. The AFR was positively correlated with the age of first eating the bedding and duration of non-nutritive oral behavior at 30 d of age. Total starter feed intake within 30 d of age, average daily starter feed intake within 30 d of age, duration of ruminating behavior at 25 and 30 d of age, and duration of eating the bedding at 25 and 30 d of age were negatively correlated with AFR. Overall, to the best of our knowledge, this is the first study that has analyzed the correlation between AFR and other indicators. We found that earlier AFR was associated with shorter duration of non-nutritive oral behavior, longer durations of rumination and eating the bedding, and higher feed intake by 30 d of age.

Objective: To investigate the spectrum of pathogens causing lung fungal disease diagnosed by histopathology through histochemical special staining, compared to the fungal culture results, and to further evaluate the diagnostic value of histochemical special staining in pulmonary fungal disease. Methods: We performed a retrospective analysis of 187 cases of pulmonary fungal disease diagnosed by histopathology in Peking Union Medical College Hospital from 2001 to 2015 (including 92 cases with pulmonary resection or open lung biopsy, 33 with percutaneous lung biopsy and 62 ones with fiberoptic bronchoscopic lung biopsy). All cases were treated with hexamine silver, PAS, mucus carmine and acid-fast staining in addition to conventional HE staining. The clinical records and the fungal culture results were reviewed. Results: There were 103 male and 84 female patients, aged from 12 to 70 years [average (48±14) years]. There were 85 cases(45.5%) of pulmonary aspergillosis(including 60 cases of invasive infection and 25 cases of aspergilloma), 51 cases(27.3%) of pulmonary cryptococosis, 6 cases (3.2%)of pulmonary mucormycosis, 3 cases(1.6%) of pulmonary histoplasmosis, 3 cases (1.6%)of pulmonary candidiasis, and 2 cases (1.1%) of pneumocystosis, while in the remaining 37 cases (19.8%) the pathogens could not be clearly classified by microscopy due to limited tissue or degeneration. Among the 88 patients with pulmonary fungal disease diagnosed by histopathology from 2011 to 2015, 35 ones (39.9%) were detected by fungal culture (including lung biopsy, intraoperative swab, blood, bronchoalveolar lavage fluid and sputum, etc.). The diagnostic results of 18 cases were completely consistent between histopathological examination and fungal culture (18/35, 51.4%), while 13 cases (13/35, 37.1%) were diagnosed by histopathology but no fungi were cultured, and in 3 cases (3/35,8.6%) the culture was positive for fungi which could not be classified clearly by histopathology. In another case the pathogen was found to be Cryptococcus histopathologically but the lavage culture grew"candida", but the patient's blood cryptococcal antigen was positive. Conclusions: Among patients with histopathological diagnosis of pulmonary fungal disease, pulmonary aspergillosis was the most common, followed by pulmonary cryptococcosis, pulmonary mucormycosis, pulmonary histoplasmosis, pulmonary candidiasis and pneumocystosis. A small number of cases could not be classified by histopathology through histochemical special staining. There was a high consistency in discovering fungal pathogens between pathological histochemical special staining and culture method, but 37% pulmonary fungal disease diagnosed by histopathology were culture negative. In practice, the role of histochemical special staining in diagnosing pulmonary fungal disease should be paid more attention.

Objective: To evaluate the effect on bleeding volume and postoperative recovery of regional cerebral oxygen saturation (rSO(2)) guides controlled hypotension in elderly patients with hypertension undergoing spinal surgery. Methods: One hundred and twenty elderly patients who underwent spinal surgery in the department of anesthesiology of Qingdao Municipal Hospital and the Affiliated Hospital of Qingdao University from January 2017 to December 2019 were selected and divided into 2 groups according to the random number table method (n=60): rSO(2) guides the controlled hypotension group (group A) and control group (group C). Both groups were performed with endotracheal intubation for general anesthesia, maintain anesthesia with sevoflurane and remifentanil, rSO(2) were monitored throughout the procedure. If necessary, sodium nitroprusside or esmolol were used to control blood pressure. In group A, the goal of controlled hypotension was that rSO(2) decreased ≤ 10% of the basic value or maintained at 64±3 and the moderate operative field bleeding. Group C underwent routine anesthesia management. Intraoperative blood loss and urine output, the incidence of hypothermia after operation, postoperative delirium, chills, nausea and vomiting, the PACU residence time, postoperative drainage volume, eating time, postoperative hospital stay were compared between the two groups. Results: Compared with group C, the blood loss [(589±157) vs (764±213) ml] and urine output [(778±121) vs (1 079±239) ml] of group A were decreased (t=-5.120, -8.712, all P<0.05). The rates of hypothermia after operation (26.7% vs 45.0%), postoperative delirium (18.3% vs 36.7%), chills (10.0% vs 25.0%), nausea and vomiting (21.7% vs 40.0%) of group A were decreased (χ(2)=4.385, 5.057, 4.675, 4.728, all P<0.05) . The PACU residence time [(56±9) vs (63±11) min], postoperative drainage volume [(217±66) vs (289±81) ml], eating time [(17.8±2.8) vs (22.3±4.1) h] and numbers of days in hospital [(7.2±2.7) vs (8.2±2.9) d] were decreased of group A (t=-3.399, -5.334, -7.000, -2.031, all P<0.05). Conclusion: The guidance of controlled hypotension with rSO(2) monitoring can reduce the blood loss and infusion volume during spinal surgery in elderly patients with hypertension, reduce postoperative related complications and enhance recovery after surgery.

BACKGROUND: Puffy upper eyelids are normal in Asians, and full-incision blepharoplasty is commonly performed to remove some orbital fat to ameliorate the problem, but not all patients would exhibit obvious improvement. Local retro-orbicularis oculus fat (ROOF) is a layer of fibrofatty soft tissue that lies much deep into the orbicularis oculi in the upper eyelid, which is an important factor in the formation of puffy upper eyelids. This study was carried out to evaluate the clinical application (including the evaluation of photographs for the thickness of upper eyelid) of upper blepharoplasty combined with ROOF resection for correcting puffy upper eyelids. METHOD: A total of 65 patients (5 males, 60 females) with puffy upper eyelids recruited from October 2015 to October 2016 were included in the study. Full-incision blepharoplasty combined with partial ROOF resection was performed on all patients. The thickness of soft tissue in the upper eyelid was measured by ultrasound biomicroscopy preoperatively and at 12 months postoperatively. RESULT: All patients underwent surgery successfully, and 62 patients (124 eyes) were followed up for 12-15 months (mean 13.8 ±â€¯2.7 months). Before the surgery, the thickness of the ROOF was 0.35 ± 0.12 mm on the left and 0.42 ± 0.08 mm on the right. Twelve months postsurgery, the thickness of the ROOF was 0.18 ± 0.03 mm on the left and 0.20 ± 0.02 mm on the right. During the follow-up period, all patients were satisfied with the esthetic effect, and no severe postoperative complications were observed. CONCLUSION: Puffy upper eyelids can be corrected effectively by local ROOF resection in esthetic blepharoplasty.

Unilateral double-eyelid blepharoplasty is often required by people with unilateral single eyelid. The full incisional method may cause permanent skin scar formation, and the buried suture method may lead to early eyelid fold loosening, which could not reduce the soft tissue thickness of eyelid. A total of 85 patients (8 males and 77 females) with unilateral single eyelids were studied between January 2015 and January 2016, with a mean age of 25.57 ± 6.5 years. Small-incision orbicularis-levator fixation blepharoplasty was performed on all patients on the single eyelid side. During surgery, the orbicularis oculi (OO) muscle and the orbital fat (OF) were selectively removed through the small skin incisions. The thicknesses of upper eyelid were measured preoperatively and at 6 months postoperatively. The position and tension of the eyelid creases, eyelids symmetry, and satisfaction status were also evaluated. Eighty patients (10 left eyes and 70 right eyes) were followed up for 19 ± 3.4 months. Before surgery, the average thickness of OO muscle is 0.895 ± 0.199 mm (left) and 0.970 ± 0.185 mm (right) (P = 0.0127), the thickness of OF is 0.717 ± 0.154 mm (left) and 0.867 ± 0.143 mm (right) (P = 0.0187). Six months postsurgery, the average thickness of OO muscle is 0.894 ± 0.199 mm (left) and 0.907 ± 0.188 mm (right) (P = 0.293), the thickness of OF is 0.703 ± 0.143 mm (left) and 0.747 ± 0.0957 mm (right) (P = 0.534). In conclusion, small-incision orbicularis-levator fixation blepharoplasty can produce a desired aesthetic effect for unilateral single-eyelid patients as its personalized designing, symmetrical eyelid thickness, and slight skin scar formation.

Objective: To investigate the clinical effect and safety of rock salt aerosol therapy in the treatment of occupational allergic contact dermatitis. Methods: A total of 65 patients with acute exacerbation of occupational allergic contact dermatitis who were treated in the Outpatient Service and Inpatient Department of our hospital from March 2013 to December 2015 were enrolled and randomly divided into observation group and control group using a random number table. Both groups were givensymptomatic treatment including desensitization, and the patients in the observation group were given rock salt aerosol therapy for 2 courses in addition to the symptomatic treatment. The changes in symptoms, signs, blood eosinophil count, and IgE were observed. Results: There were significant changes in symptom score at the first and second courses of the treatment (P<0.05) , and there was an interaction between time of therapy and grouping (P<0.05) . There was no significant difference in symptom score before treatment between the two groups (P>0.05) , while there were differences at the first and second courses of the treatment (P<0.05) . After the second course of treatment, the observation group had a significantly higher overall response rate than the control group (P<0.05) ; both groups had significant reductions in blood eosinophil count and the observation group had a significantly greater reduction than the control group (P<0.05) . After two courses of treatment, both groups had significant increases in the number of patients with normal IgE (both P<0.05) , and after the second course of treatment, the observation group had a significantly higher number than the control group (P<0.05) . Both groups had mild adverse events, which did not affect the treatment. Conclusion: In the treatment of occupational allergic contact dermatitis, rock salt aerosol therapy has a certain effect on the recovery of symptoms, signs, blood eosinophil count, and IgE.

NLRC5 plays an important role in the innate immunity and cellular immunity in many species, but the regulatory mechanism of NLRC5 expression in chickens remains unclear. In this study, a series of deletion fragments of the NLRC5 promoter region were constructed and dual-luciferase assay was performed. Then, we detected the SNP in the core region and its function. Important transcriptional regulatory elements were predicted and identified. Methylation of CpG islands was measured. The results revealed that the two core regions of -4372 to -3756 and -2925 to -2265 in the NLRC5 promoter were essential for NLRC5 mRNA expression in which a SNP (A/G), located at -2470, was found to have an effect on the transcriptional activity. Also, the STAT1 element in the second core region of the NLRC5 promoter was identified to bind with the STAT1 transcription factor, which was necessary for the transcriptional activity. In addition, many other elements in the NLRC5 promoter, including YY1 and CEBP, may contribute significantly to the expression activity of NLRC5. Moreover, two CpG islands were searched. Part of one was located in the first core region, which suggests that epigenetic modification may regulate the activity of the first promoter region, and the other was mostly in an unmethylated state. Collectively, these results suggest the complex regulation of NLRC5 expression includes SNPs, transcription factors and methylation.

The objective of this study was to determine the optimum conditions for calcium oxide (CaO) treatment of anaerobically stored corn stover by in situ and in vitro methods. Four ruminally cannulated, non-lactating, non-pregnant Holstein cows were used to determine the in situ effective degradabilities of dry matter (ISDMD), organic matter (ISOMD), neutral detergent fibre (ISNDFD), in vitro organic matter disappearance (IVOMD) and gas production in 72 h (GP72h ) of corn stover. A completely randomized design involving a 3 × 3 factorial arrangement was adopted. Ground corn stover was treated with different levels of CaO (3%, 5% and 7% of dry stover) at varying moisture contents (40%, 50% and 60%) and stored under anaerobic conditions for 15 days before analysis. Compared with untreated corn stover, the CaO-treated stover had increased ash and calcium (Ca) contents but decreased aNDF and OM contents. The moisture content, CaO level and their interaction affected (p < 0.01) the content of aNDF, ash and OM, and the ratio of aNDF/OM. The greatest ISDMD, ISOMD and ISNDFD were observed when stover was treated with 7% CaO and 60% moisture, while no differences (p > 0.01) in these in situ degradability parameters were observed between the stover treated with 5% CaO at 60% moisture content and those treated with 7% CaO at 60% moisture content. Corn stover treated with 5% CaO at 50% moisture had the maximum IVOMD and GP72 h among the treatments, and there was no difference (p > 0.01) between 50% and 60% moisture. Results from this study suggested that 5% CaO applied at 60% moisture could be an effective and economical treatment combination.

OBJECTIVE: Reactive oxygen species (ROS) are significantly upregulated after spinal cord injury (SCI). MicroRNAs (miRNAs) are reported to be widely involved in regulating gene expression. This paper aims to explore the correlation between ROS-induced cell apoptosis and abnormal miRNA expression after SCI. METHODS: To profile the expression of miRNAs after SCI, miRNA microarray was applied and the result was verified by reverse transcription quantitative PCR (RT-qPCR). ROS production following H2O2 stimulation was examined using dihydroethidium staining and flow cytometry. The levels of miR-200c after H2O2 treatment were determined using RT-qPCR. Cell viability and apoptosis were examined in murine BV-2 cells transfected with miR-200c mimics, inhibitor or negative control. Immunofluorescence and western blot were used to further explore the effects of miR-200c on Fas-associated phosphatase-1 (FAP-1) expression. RESULTS: MiR-200c was showed to be significantly increased after SCI by miRNA microassay and RT-qPCR. ROS production enhanced miR-200c expression in a dose-dependent manner and induced significant apoptosis in BV-2 cells. The upregulation of miR-200c reduced cell viability and induced BV-2 cell apoptosis. MiR-200c negatively regulated the expression of FAP-1, thereby inducing FAS signaling-induced apoptosis. RT-qPCR analysis showed that the FAP-1-targeting small interfering RNA (siRNA) did not affect the level of miR-200c in murine BV-2 cells. In addition, suppression of FAP-1 by siRNA promoted apoptosis, even in cells that were co-transfected with the miR-200c inhibitor. CONCLUSIONS: The current data suggested that miR-200c contributes to apoptosis in murine BV-2 cells by regulating the expression of FAP-1. This proposes a therapeutic target for enhancing neural cell functional recovery after SCI.

Asymmetry of the eyelid crease is a major complication following double eyelid blepharoplasty; the reasons are multivariate. This study presents, for the first time, a novel method, based on high-definition magnetic resonance imaging and high-precision weighing of tissue, for quantitating preoperative asymmetry of eyelid thickness in young Chinese women presenting for blepharoplasty. From 1 January 2008 to 1 October 2011, we studied 1217 women requesting double eyelid blepharoplasty. The patients ranged in age from 17 to 24 years (average 21.13 years). All patients were of Chinese Han nationality. Soft-tissue thickness at the tarsal plate superior border was 5.05 ± 1.01 units on the right side and 4.12 ± 0.96 units on the left. The submuscular fibro-adipose tissue area was 95.12 ± 23.27 unit(2) on the right side and 76.05 ± 21.11 unit(2) on the left. The pre-aponeurotic fat pad area was 112.33 ± 29.16 unit(2) on the right side and 91.25 ± 27.32 unit(2) on the left. The orbicularis muscle resected weighed 0.185 ± 0.055 g on the right side and 0.153 ± 0.042 g on the left; the orbital fat resected weighed 0.171 ± 0.062 g on the right side and 0.106 ± 0.057 g on the left. In conclusion, upper eyelid thickness asymmetry is a common phenomenon in young Chinese women who wish to undertake double eyelid blepharoplasty. We have demonstrated that the orbicularis muscle and orbital fat pad are consistently thicker on the right side than on the left.
