RESUMO

It is a common belief amongst strength and power athletes that nutritional supplementation strategies aid recovery by shifting the anabolic/catabolic profile toward anabolism. Factors such as nutrient quantity, nutrient quality, and nutrient timing significantly impact upon the effectiveness of nutritional strategies in optimizing the acute responses to resistance exercise and the adaptive response to resistance training (i.e., muscle growth and strength expression). Specifically, the aim of this review is to address carbohydrates (CHOs), protein (PRO), and/or amino acids (AAs) supplementation strategies, as there is growing evidence suggesting a link between nutrient signaling and the initiation of protein synthesis, muscle glycogen resynthesis, and the attenuation of myofibrillar protein degradation following resistance exercise. Collectively, the current scientific literature indicates that nutritional supplementation strategies utilizing CHO, PRO, and/or AA represents an important approach aimed at enhancing muscular responses for strength and power athletes, primarily increased muscular hypertrophy and enhanced strength expression. There appears to be a critical interaction between resistance exercise and nutrient-cell signaling associated with the principle of nutrient timing (i.e., pre-exercise, during, and post-exercise). Recommendations for nutritional supplementation strategies to promote muscular responses for strength and athletes are provided.

A rapidly emerging area of interest in high-pressure environments is that of pupillometry, where handheld quantitative infrared pupillometers (HQIPs) are able to track psycho-physiological fatigue in a fast, objective, valid, reliable, and non-invasive manner. However, the application of HQIPs in the context of athlete monitoring is yet to be determined. Therefore, the main aim of this pilot study was to examine the potential usefulness of a HQIP to monitor game-induced fatigue inside a professional female basketball setting by determining its (1) test-retest repeatability, (2) relationship with other biomarkers of game-induced fatigue, and (3) time-course from rested to fatigued states. A non-ophthalmologic practitioner performed a standardized Pupil Light Reflex (PLR) test using a medically graded HQIP among 9 professional female basketball players (2020-2021 Euro Cup) at baseline, 24-h pre-game (GD-1), 24-h post-game (GD+1) and 48-h post-game (GD+2). This was repeated over four subsequent games, equalling a total of 351 observations per eye. Two out of seven pupillometrics displayed good ICCs (0.95-0.99) (MinD and MaxD). Strong significant relationships were found between MaxD, MinD, and all registered biomarkers of game-induced fatigue (r = 0.69-0.82, p < 0.05), as well as between CV, MCV, and cognitive, lower-extremity muscle, and physiological fatigue markers (r = 0.74-0.76, p < 0.05). Three pupillometrics were able to detect a significant difference between rested and fatigued states. In particular, PC (right) (F = 5.173, η2 = 0.115 p = 0.028) and MCV (right) (F = 3.976, η2 = 0.090 p = 0.049) significantly decreased from baseline to GD+2, and LAT (left) (F = 4.023, η2 = 0.109 p = 0.009) significantly increased from GD-1 to GD+2. HQIPs have opened a new window of opportunity for monitoring game-induced fatigue in professional female basketball players. However, future research initiatives across larger and heterogenous samples, and longer investigation periods, are required to expand upon these preliminary findings.

Long COVID, also known as Post COVID-19 condition, is defined by the WHO as the continuation or development of new symptoms three months after the initial SARS-CoV-2 infection, with these symptoms lasting for at least two months with no other explanation. Despite many studies examining the causes and mechanisms of this disease, fewer studies have sought to understand the experience of those suffering from long COVID, or "long-haulers," This study contributes to the understanding of long-haulers (N = 14) by examining the role of agency and social support in shaping their journeys with long COVID. Drawing on a combination of interviews, questionnaires, and video diaries over a three-month period, journey mapping was used to document the participants' experiences, including symptoms, coping strategies, and lifestyle changes. Analysis of these journey maps resulted in a framework with four clusters demonstrating the importance of social support and patient agency shaping participants' Long COVID trajectory; the study contributes valuable insights into the daily lives and challenges individuals face with long COVID, informing the development of targeted support programs.

INTRODUCTION: Due to the COVID-19 pandemic, the use of digitally delivered exercise classes to promote physical activity has become widespread amongst various populations as an alternative to in-person activities. OBJECTIVES: To examine the feasibility, acceptability, and participant engagement variables to delivering Qi Gong and Tai Chi programs through telehealth interventions. METHODS: Ten databases (Seven English databases; three Chinese databases) were searched between October and November 2021. Studies published in English or Chinese, or having translations in English or Chinese, were included. Titles and abstracts of identified articles were screened, relevant studies were then retrieved for full-text screening. Study selection, assessment of methodological quality, data extraction, data transformation, and data synthesis were completed following a convergent integrated approach to mixed method systematic reviews. RESULTS: Seven articles were included in review. Digital literacy of both participants and providers was found to be a significant hurdle towards digital program implementation. There were no notable issues pertaining to access to an internet connection, participant safety, program costs, or connectivity. A major theme for sustaining program engagement was found to be individual perceived relevance for intervention involvement. Online social involvement was noted to be both a facilitator for participant acceptability and engagement. Overall, participants expressed satisfaction with the use of telehealth, while providers expressed acceptability concerns regarding quality of care. CONCLUSION: It is recommended that planned measures be taken prior to program commencement to decrease digital literacy requirements while also including a participatory approach to encourage uptake. During the program, provision of technical support alongside appropriate social-environmental engagement facilitators would promote sustained adherence.

PURPOSE: This study aimed to comprehensively investigate the global implementation of flywheel training (FT) by basketball strength and conditioning (S&C) coaches in various contexts, encompassing daily practice, games, and sessions. METHODS: Survey data were collected from 117 basketball S&C coaches who participated in a 24-question online survey. The survey was structured into 6 key areas, including country and competition, S&C coach context, training methodology, flywheel and competition, postactivation performance enhancement, and recovery. RESULTS: Notably, all respondents emphasized the necessity of a familiarization period with flywheel technology, with a substantial 96% indicating that FT yielded improved player performance on the court. The predominant mention was the conical pulley system. During the season, the prevalent approach involved integrating FT into training twice a week, allocating <15 minutes per session, often in conjunction with traditional strength training. A diverse array of lower-body closed kinetic chain exercises were reported, encompassing squats, decelerations, and backward lunges. Intriguingly, FT implementation on match days was unlikely (77%), with the primary aims cited as injury prevention (34%) and enhancing players' strength levels during various phases of the regular season (27%). CONCLUSIONS: Recognizing its inherent limitations, this descriptive study provides valuable contextual insights and practical applications for professional basketball practitioners grappling with the utilization of FT.

Objective: To identify gaps among Australian Long COVID support services and guidelines alongside recommendations for future health programs. Methods: Electronic databases and seven government health websites were searched for Long COVID-specific programs or clinics available in Australia as well as international and Australian management guidelines. Results: Five Long COVID specific guidelines and sixteen Australian services were reviewed. The majority of Australian services provided multidisciplinary rehabilitation programs with service models generally consistent with international and national guidelines. Most services included physiotherapists and psychologists. While early investigation at week 4 after contraction of COVID-19 is recommended by the Australian, UK and US guidelines, this was not consistently implemented. Conclusion: Besides Long COVID clinics, future solutions should focus on early identification that can be delivered by General Practitioners and all credentialed allied health professions. Study findings highlight an urgent need for innovative care models that address individual patient needs at an affordable cost. We propose a model that focuses on patient-led self-care with further enhancement via multi-disciplinary care tools.

The purpose of this study was two-fold: (1) to determine the sensitivity of the sEMG shorts-derived training load (sEMG-TL) during different running speeds; and (2) to investigate the relationship between the oxygen consumption, heart rate (HR), rating of perceived exertion (RPE), accelerometry-based PlayerLoadTM (PL), and sEMG-TL during a running maximum oxygen uptake (V˙O2max) test. The study investigated ten healthy participants. On day one, participants performed a three-speed treadmill test at 8, 10, and 12 km·h-1 for 2 min at each speed. On day two, participants performed a V˙O2max test. Analysis of variance found significant differences in sEMG-TL at all three speeds (p < 0.05). A significantly weak positive relationship between sEMG-TL and %V˙O2max (r = 0.31, p < 0.05) was established, while significantly strong relationships for 8 out of 10 participants at the individual level (r = 0.72-0.97, p < 0.05) were found. Meanwhile, the accelerometry PL was not significantly related to %V˙O2max (p > 0.05) and only demonstrated significant correlations in 3 out of 10 participants at the individual level. Therefore, the sEMG shorts-derived training load was sensitive in detecting a work rate difference of at least 2 km·h-1. sEMG-TL may be an acceptable metric for the measurement of internal loads and could potentially be used as a surrogate for oxygen consumption.

Several studies have confirmed the efficacy of flywheel eccentric overload training in order to improve or increase muscle volume and strength, as well as several performance-related fitness attributes, but to date, there are no studies that have reviewed the effects of these training methods in basketball. Therefore, the present systematic review aimed to collect the updated information about the influence of flywheel training on performance of basketball players. A search in four electronic databases (PubMed, SCOPUS, Web of Science, and Google Scholar) was conducted up to November 20, 2022. Articles were selected as valid for review if: (i) they were an experimental trial published in English; (ii) participants were basketball players without illnesses or injuries, and (iii) a rotational inertial overload method was used as a performance-dependent variable. A total of 93 articles were found. After filtering procedures, only seven studies were considered in this systematic review. In the selected studies, 203 participants were included, 58 females and 145 males. Focusing on basketball related performance variables, all the studies included the countermovement jump (CMJ), while five included sprinting and a change of direction test (COD). The results showed an improvement in performance-related variables associated with basketball (sprint, CMJ, and COD) through the use of inertial methods. Improvements were reported in professional, semi-professional and amateur players as well as both male and female players. However, given the low number of studies, additional investigations on this topic are warranted before a "clear picture" can be drawn concerning the effects of flywheel eccentric overload training in basketball players.

OBJECTIVES: To explore the feasibility and effectiveness of telehealth-supervised exercise for adults with Systemic lupus erythematosus (SLE). METHODS: This was a non-randomised controlled pilot trial comparing telehealth-supervised exercise (8 weeks, 2 days/week, 45 min, moderate intensity) plus usual care with usual care alone. Mixed methods were used to assess change in fatigue (FACIT-fatigue), quality of life (SF36), resting fatigue and pain (11-point scale), lower body strength (five-time sit-to-stand) and endurance (30 s sit-to-stand), upper body endurance (30 s arm curl), aerobic capacity (2 min step test), and experience (survey and interviews). Group comparison was performed statistically using a two-sample T-test or Mann-Whitney U-test. Where known, we used MCID or MCII, or assumed a change of 10%, to determine clinically meaningful change within groups over time. Interviews were analysed using reflexive thematic analysis. RESULTS: Fifteen female adults with SLE were included (control group n = 7, exercise group n = 8). Statistically significant differences between groups, in favour of the exercise intervention, were noted for SF36 domain emotional well-being (p = 0.048) and resting fatigue (p = 0.012). There were clinically meaningful improvements over time for FACIT-fatigue (+6.3 ± 8.3, MCID >5.9), SF36 domains physical role functioning (+30%), emotional role functioning (+55%), energy/fatigue (+26%), emotional well-being (+19%), social functioning (+30%), resting pain (-32%), and upper body endurance (+23%) within the exercise group. Exercise attendance was high (98%, 110/112 sessions); participants strongly agreed (n = 5/7, 71%) or agreed (n = 2/7, 29%) they would do telehealth-supervised exercise again and were satisfied with the experience. Four themes emerged: (1) ease and efficiency of exercising from home, (2) value of live exercise instruction, (3) challenges of exercising at home, and (4) continuation of telehealth-supervised exercise sessions. CONCLUSION: Key findings from this mixed-method investigation suggest that telehealth-supervised exercise was feasible for, and well-accepted by, adults with SLE and resulted in some modest health improvements. We recommend a follow-up RCT with more SLE participants.

BACKGROUND: The results of rehabilitation trials are often not fully attained when the intervention is implemented beyond the initial trial. One of the key reasons is that a patients' ability and/or capacity to take part in their own healthcare is not considered in the trial design yet has significant impact on the outcomes during the implementation phase. BODY OF TEXT: We propose a shift from a therapist-focus to patient-focus in trial design, through addressing patient engagement as a core consideration in trials. We argue that engaging patients in any rehabilitation program is a process of behavioural change. Exercise prescription is used as an example to illustrate how the Behaviour Change Wheel can be applied to analyse barriers and facilitators associated with patients' capabilities, opportunities and motivations in integrating trial interventions into their daily life. We propose a framework to assist in this shift. CONCLUSION: A core part of implementing rehabilitation interventions at the primary care level requires patient engagement. Related aspects of interventions should be identified and assessed using the COM-B model at the outset of trial design to ensure that the results are realistic, meaningful and transferable, so as to enable real impact.


Rehabilitation programs tested in clinical trials often focus on the therapists' effort and the program itself.Engaging patients in any rehabilitation program is a process of behaviour change of patients.Our proposed framework based on the COM-B model, including the Behaviour Change Wheel, enriches rehabilitation trial design through considering core aspects of patient engagement to ensure the real impact of any rehabilitation program can be achieved.

Over the past ten years there has been a dramatic rise in female sport participation and accompanying female professional national leagues across multiple sports, yet research has not followed suit. Although there are known variations between female and male physiology, training protocols in female sport are predominantly underpinned by research undertaken in male athletes. The hormonal variability experienced by women across the menstrual cycle, as well as the menstrual cycle variability between women, may contribute to the complexity of conducting rigorous physiological studies, leading to a paucity of robust sports-specific research that can be confidently applied to female athletes. Moreover, barriers exist in female sport that potentially limit the ability to conduct research, including the lack of full-time programs and limited resources. Recently, there has been increased interest in the potential effects of fluctuations in the female sex hormones, progesterone and oestrogen, on sport performance across different phases of the menstrual cycle. However, current research evaluating the menstrual cycle and physical performance (such as strength, speed, aerobic fitness, and athletes' perception of their performance) have shown inconsistent results. Additionally, methodological design across studies has shown little consistency, making it difficult to draw firm conclusions, which potentially prevents female athletes optimising their physical and sporting performance. It further impacts coaches and sports science researchers in their ability to provide appropriate training recommendations and educational opportunities. It is important to progress in female athlete research with an understanding of how the unique physiology of female athletes may influence their ability to physically perform in their respective sport, which requires representation in sports science research. This paper will provide an overview on current evidence and limitations within menstrual cycle research and provide considerations and directions for future research in this space within team sports.

BACKGROUND: Sleep is considered an essential component related to physiological and psychological recovery in athletes and particularly in basketball, given the impact of condensed travel and game schedules on player health and performance. OBJECTIVE: The aim of this systematic review is to examine studies published to date on sleep and basketball performance. METHODOLOGY: All scientific articles that reported a relationship between sleep and its possible impact on performance in basketball are included. The research processes followed the PRISMA criteria, and the relevant articles were extracted (PubMed, WOS, Scopus) as of December 31, 2021. RESULTS: Twenty-eight articles were selected for inclusion and data extraction, with 27 demonstrating that sleep is a vital component in the recovery of basketball players and their corresponding on-court performance. Three central themes that we identified: (1) the quality and extension of sleep (the better quality and more extension of sleep, better performance and lower probability of injury); (2) influence of the players circadian rhythm (travel and game scheduling do not currently facilitate or take this into account); and (3) higher training loads and/or increased stress may jeopardize a subsequent good night's sleep, which should be taken into account when scheduling practices and workouts. CONCLUSION: The current systematic review regarding sleep and basketball performance and highlights that there is a strong relationship between both variables. Collectively, the evidence supports the critical influence of sleep on player recovery and basketball performance and risk for injury.

Forefoot pain is common in clinical practice. Careful history taking and routine physical examination are initially performed for diagnosis, but imaging can confirm the clinical suspicion and play a key role in management. Ultrasound (US) can provide a visualization of the fine anatomy of the forefoot and is a useful method for evaluating various lesions causing forefoot pain. In this review, we provide the detailed anatomical structures of the forefoot and their normal appearances on US. We also focus on the most common pathologies affecting the forefoot, including plantar plate tear, sesamoiditis, bone fracture, synovitis, tenosynovitis, bursitis, Morton's neuromas, and foreign bodies.

Background: Movement competency screens (MCSs) are commonly used by coaches and clinicians to assess injury risk. However, there is conflicting evidence regarding MCS reliability. Purpose: This study aimed to: (i) determine the inter- and intra-rater reliability of a sport specific field-based MCS in novice and expert raters using different viewing methods (single and multiple views); and (ii) ascertain whether there were familiarization effects from repeated exposure for either raters or participants. Study Design: Descriptive laboratory study. Methods: Pre-elite youth athletes (n=51) were recruited and videotaped while performing a MCS comprising nine dynamic movements in three separate trials. Performances were rated three times with a minimal four-week wash out between testing sessions, each in randomized order by 12 raters (3 expert, 9 novice), using a three-point scale. Kappa score, percentage agreement and intra-class correlation were calculated for each movement individually and for the composite score. Results: Fifty-one pre-elite youth athletes (15.0±1.6 years; n=33 athletics, n=10 BMX and n=8 surfing) were included in the study. Based on kappa score and percentage agreement, both inter- and intra-rater reliability were highly variable for individual movements but consistently high (>0.70) for the MCS composite score. The composite score did not increase with task familiarization by the athletes. Experts detected more movement errors than novices and both rating groups improved their detection of errors with repeated viewings of the same movement. Conclusions: Irrespective of experience, raters demonstrated high variability in rating single movements, yet preliminary evidence suggests the MCS composite score could reliably assess movement competency. While athletes did not display a familiarization effect after performing the novel tasks within the MCS for the first time, raters showed improved error detection on repeated viewing of the same movement. The long head of the biceps tendon (LHBT) has been recognized as an important generator of anterior shoulder pain, causing a significant reduction in the shoulder flexion range. Various tendinous and ligamentous structures form the anchoring apparatus of the LHBT along its course to maintain its appropriate location during shoulder movements, including the coracohumeral ligament (CHL), superior glenohumeral ligament (SGHL), subscapularis (SSC) tendon and supraspinatus (SSP) tendon as well as the less recognized tendons of pectoralis major (PM), latissimus dorsi (LD) and teres major (TM). Lesions of this stabilizing apparatus may lead to an instability of the LHBT, resulting in pain at the anterior shoulder. Ultrasonography (US) has been increasingly used in the assessment of shoulder injuries, including the anchoring apparatus of the LHBT. An accurate diagnosis of these injuries is often challenging, given the complex anatomy and wide spectrum of pathologies. In this review article, US anatomy and common pathologic conditions that affect the anchoring apparatus of the LHBT are discussed, including biceps pulley lesions, adhesive capsulitis, chronic pathology of SSC and SSP tendons, tears in the PM tendon and injuries to the LD and TM. Knowledge of a normal anatomy, an appropriate scanning technique and US findings of common pathologic conditions are the keys to accurate diagnoses.

BACKGROUND: Despite the proven benefits of exercise rehabilitation for numerous health conditions, musculoskeletal injuries and recovery from surgery, patient adherence to such programs is reported to often be less than 35%. Increasing patient engagement therefore has the potential to improve patient health outcomes, benefiting the patient, their carers and the services that support them. The aims of this review were to identify the factors that contribute to 'patient' engagement in prescribed exercise rehabilitation using the COM-B (capability, opportunity, motivation-behaviour) framework of behavioural analysis. METHODS: Five electronic databases (PubMed, Embase, Cochrane, Web of Science, and ClinicalTrials.gov) were searched. 'COM-B' was the key word searched for specifically within titles and abstracts, combined with either 'physical activity' OR 'exercise' included using the 'AND' operation. Records were then filtered and excluded following full-text screening based on the predetermined eligibility criteria. RESULTS: Twenty studies were included in the review. The main COM-B themes highlighted for improving patient engagement were: capability-improving patient knowledge and cognitive skills for behavioural regulation, such as 'action planning' and 'action control', which could also benefit time-management; opportunity-a balanced life situation that enabled time to be devoted to the exercise program, social support, easily accessible and affordable resources and services; and motivation-increasing patient levels of self-efficacy and autonomous motivation, which were noted to be influenced by levels of perceived 'capability', additionally 'motivation' was noted to be influenced by patients perceiving the benefits of the exercise, and adherence to the program was promoted by 'goal-setting'. Other issues in the 'capability' domain included a fear and/or dislike of exercise. CONCLUSION: Patient engagement behavior has been shown to be influenced by both external (opportunity) and intrapersonal variables (capability and motivation). Those prescribing exercises within a rehabilitation program need to discuss these factors with their patients and co-design the exercise rehabilitation program in partnership with the patient, since this is likely to improve patient engagement, and thereby result in superior health outcomes. Furthermore, these factors need to be a consideration in clinical trials, if the findings from such trials are to translate into mainstream healthcare settings.

BACKGROUND: Success at the elite level in sport is often attributed to physical prowess, technical skill, and mental attitude. However, underpinning these factors are various lifestyle, support and social factors that may contribute to successful performance, but which may be absent from athlete development programs. METHODS: An online survey was used to investigate athlete perceptions of lifestyle practices and support services amongst 135 Australian Olympic, Paralympic, National, and state-level athletes across 25 Olympic sports. RESULTS: International athletes perceived psychological skills and attributes, along with strong interpersonal relationships as vital to their success, and they also rated 'Recovery practices' as very important and made extensive use of available support services. These athletes also indicated that they would have liked access to these services earlier in their careers, a wish that was reiterated by the sub-elite athletes. Furthermore, athletes wanted greater knowledge, mentoring, and autonomy earlier in their careers, and the importance of 'athlete wellbeing' as well as 'athletic performance' was evident in a number of contexts. CONCLUSIONS: An athlete development system into which these are included may assist in generating an environment that facilitates athlete success, repeated podium performances, retain athletes in high-performance sport for longer, encourage human-flourishing, wellbeing and smooth transitions for retiring athletes.

OBJECTIVE: Recognizing the high stakes associated with winning and losing in the National Basketball Association (NBA), a deep understanding of the underlying mechanisms of NBA game-play performance would provide substantial benefit to all stakeholders involved with preparing NBA players and teams for competitive success. To the best of the authors' knowledge, this systematic review presents the first attempt to systematically amalgamate and appraise the scientific literature published in the XXI Century, following a constraints-led approach (CLA). In particular, two underpinning factors of NBA game-play performance were investigated: (1) NBA player constraints (internal variables) and (2) NBA contextual constraints (external variables). METHODS: Databases included PubMed (MEDLINE), Web of Science (WOS), ResearchGate, SPORTDiscus, SCOPUS, Google Scholar, and the World Association of Basketball Coaches' database (WABC). This review followed the Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) model and the Population, Intervention, Comparison and Outcomes (PICOS) guidelines. RESULTS: Ultimately, 43 articles met the inclusion criteria (n = 43). Promisingly, the vast majority of studies were published in recent years (>2016; n = 28; 65.1%). Topics related to 'contextual constraints' (n = 25; 58.1%) received more attention than topics related to 'player constraints' (n = 18; 41.9%). Even though the importance of longitudinal-interventional approaches to applied sports science is well-documented, descriptive-observational research emerged as the most popular method of choice (n = 27; 62.8%); interventional studies were absent; and near all researchers merely utilized secondary data sources (n = 37; 86.0%). CONCLUSIONS: Taking into account the total body of evidence (2001-2020), NBA practitioners may use this systematic review as a baseline reference to enrich their current knowledge about the nature, demands, and dynamics of the modern-day NBA ecosystem. Finally, adoption of an 'Applied Science Research Framework' is encouraged, fostering clearly outlined project incentives; standardizing taxonomies; sequencing follow-up studies; embracing holistic and cross-disciplinary viewpoints; and integrating longitudinal-interventional projects to increase the reproducibility of their findings.

In the field of sports science, the recovery umbrella is a trending topic, and even more so in the world of elite sports. This is evidenced by the significant increase in scientific publications during the last 10 years as teams look to find a competitive edge. Recovery is recognized to be an integral component to assist athlete preparation in the restoration of physical and psychological function, and subsequently, performance in elite team sports athletes. However, the importance of recovery in team staff members (sports coaches and performance staff) in elite sports appears to be a forgotten element. Given the unrelenting intense nature of daily tasks and responsibilities of team staff members, the elite sports environment can predispose coaches to increased susceptibility to psycho-socio physiological fatigue burden, and negatively affect health, wellbeing, and performance. Therefore, the aim of this opinion was to (1) develop an educational recovery resource for team staff members, (2) identify organizational task-specific fatigue indicators and barriers to recovery and self-care in team staff members, and (3) present recovery implementation strategies to assist team staff members in meeting their organizational functions. It is essential that we do not forget the coaching and performance staff in the recovery process.
