RESUMO

In a double-blind study, 117 non-pregnant women with vaginal candidosis were treated for 3 days with 200 mg-tablets of ketoconazole taken once, twice or three times daily. The incidence of predisposing factors or of a recurrence history did not differ between treatment groups. Their male partners were randomly assigned to receive ketoconazole, 200 mg twice daily or placebo for 3 consecutive days. Cure and recurrence rates were not different in the three treatment groups, with or without simultaneous treatment of the male partner. Treatment of the sexual partner in the three dose-regimen-groups proved not to have influenced the therapeutic effect.

RESUMO

Fluid aspirated from the vagina of pregnant or non-pregnant women, either normally menstruating or using hormonal contraceptives, proved to be an adequate culture medium for Candida albicans. The growth-promoting activity did not differ between groups. A medical history of vaginal candidosis did not influence this growth-promoting activity. It is concluded that an enhanced susceptibility to this manifestation with C. albicans is not related to this capacity of the fluid contained in the vaginal cavity to serve as a growth-promoting medium.

The growth of Candida albicans in fluid aspirated from the vagina of 18 women was studied. The growth of C. albicans in the aspirated vaginal fluid to which antibiotics were added proved to be enhanced in 16 out of 18 samples. The growth of C. albicans in Sabouraud medium proved not to be enhanced by the addition of antibiotics. Dilution of the Sabouraud broth and the reduction of the amount of added antibiotics did not change the outcome. It is concluded that enhancement of the growth of C. albicans in the vagina is not due to a direct growth-promoting effect of the antibiotics on the yeast. It is concluded that most probably the growth-promoting effect of antibiotics is obtained by way of an eradication of the bacterial flora harboured in the vagina.

Plasma samples and biopsies of vaginal tissue were obtained from 23 healthy women undergoing operative sterilization, 1 to 6 h after a single oral dose of ketoconazole 200 mg. Drug concentrations in plasma and tissue, were measured by a specific gas chromatographic method. The vaginal tissue concentration averaged 2.4 times less than the corresponding plasma levels. Equilibrium between tissue, and plasma was established within 1 h after dosing, when vaginal tissue levels exceeded 1 microgram/g. Ketoconazole concentrations decayed monoexponentially over the time interval studied (1-6h), with the similar half-lives of 1.2 and 1.4 H in plasma and tissue, respectively. Following an oral 200 mg dose, a tissue concentration not less than 0.01 microgram/ml was maintained over a 12 h period. This concentration has been shown to prevent outgrowth of the invasive (pseudo) mycelial form of Candida albicans. Hence, a b.i.d. or t.i.d. dosage schedule of ketoconazole in vaginal candidosis would give continuously effective levels at the site of infection. Ketoconazole concentrations in vaginal fluid are thought to be much higher than in the tissue because of ion-trapping. The present data may explain the efficacy of oral ketoconazole in the treatment of vaginal candidosis.

RESUMO

The efficacy of a new, orally active antimycotic, ketoconazole, in the treatment of vaginal candidiasis was studied at different doses involving 63 patients. All patients had subjective complaints and were mycologically positive. There was no significant difference in the cure rates of the different doses. The oral treatment was successful in 55 out of 63 patients (87.3%). No correlation was found between the relapse rate of vaginal candidiasis and the dose regimen. There were no serious side-effects, and no significant changes were reported in hematological and biochemical parameters.
