RESUMO

Deltas and estuaries are formed through periods of marine transgression and regression, which are the continuity of a river and provide key information about its evolution. However, many of the world's deltas are increasingly exposed to the impacts of human activities. While changes affecting the subaerial parts of deltas have been intensively studied, much less is known of their subaqueous parts, the understanding of which is important in gauging overall potential delta vulnerability. This research evaluates the bathymetric changes in the submerged delta of the Turia river (Western Mediterranean, Spain) before and after the extreme flood event of 1957, after which the riverbed was diverted. Three nautical charts were processed (1878, 1988 and 2022), including georeferencing and Digital Elevation Model (DEM) generation. In order to evaluate changes before and after the event, models for 1878-1988 and 1988-2022 were compared and differences were quantified in order to assess erosion and aggradation trends. The results indicate a more aggradated submerged delta in the surroundings of the old river mouth, favored by the high sediment availability since the end of the Little Ice Age (LIA), and the presence of a smaller delta next to its current river mouth to the south of the harbor of Valencia. Bathymetric reconstructions also made it possible to map some incisions in the inner continental shelf as river channels that migrated eastwards when the sea level was lower during MIS 2. Finally, the comparison of bathymetric models also revealed the scarcity of sediments on the current shelf since 1988, which is attributed to anthropogenic action. The successive extensions of the harbor are increasingly distorting the distribution of sediments along the coast and are thus remodeling seabed sediment distribution. Knowing the sedimentation in deltaic systems means better predicting future alterations due to increased anthropization and of the climate change.

RESUMO

INTRODUCTION: Combination therapy with an alpha blocker (AB) plus an antimuscarinic (AM) is recommended for men with moderate-to-severe mixed lower urinary tract symptoms (LUTS) when monotherapy is not effective in relieving storage symptoms. This study compared treatment persistence and adherence with an AB plus AM fixed-dose combination (FDC) vs an AB plus AM free-dose combination in men with LUTS in Spain. METHODS: Retrospective study using the Spanish IQVIA Cegedim Electronic Medical Records database. Men prescribed AB plus AM combination therapy were included in an FDC or free-dose combination cohort based on their index treatment. Treatment persistence was the time from index date to first discontinuation of ≥1 of the two index drugs over 12 months. Adherence was measured using the fixed medication possession ratio (MPR). RESULTS: Of 3114 patients identified, 999 were included (FDC, n = 790; free-dose combination, n = 209). Median (95% CI) persistence was longer in the FDC (125 [109-151] days) than in the free-dose combination (31 [31-36] days) cohort (hazard ratio [HR], 2.9; 95% CI, 2.4-3.4; P < .0001). The 12-month persistence rates were 31.1% (FDC cohort) and 8.9% (free-dose cohort). The mean (SD) fixed MPR was higher in the FDC cohort (48.8 [37.2]) compared with the free-dose cohort (23.1 [28.4]); more patients in the FDC cohort (34.2%) than in the free-dose cohort (10.0%) were adherent (MPR ≥ 80%). The probability of treatment persistence and adherence increased with age (>80 vs <65 years, persistence HR, 0.7 [95% CI, 0.5-0.9]; MPR difference, 12.5), polypharmacy (persistence HR, 0.7 [95% CI, 0.6-0.9]; MPR difference, 10.7) and previous use of AB (persistence HR, 0.8 [95% CI, 0.7-1.0]; MPR difference, 5.7) or AB/AM combinations (persistence HR, 0.7 [95% CI, 0.5-0.9]; MPR difference, 11.1). CONCLUSIONS: Treatment with an AB/AM FDC is associated with better persistence and adherence vs a free-dose combination in men with LUTS in Spain.

RESUMO

The environmental variables that determine the distributions of benthic foraminiferal assemblages on the shoreface and inner shelf of the north of Valencia province (Western Mediterranean) are identified. The possible influence of variables such as water depth, distance from shore, hydrodynamics, substrate type, carbonate content, organic matter content and human activity is evaluated. Multivariate cluster-Q-type analysis and redundancy analysis (RDA) are used to identify the environmental variables that have the greatest influence on the assemblage distribution. The spatial distribution of the assemblages is closely associated with water depth and substrate. The diversity and abundance of foraminifera shells increase with depth and their conservation improves. The most common species in the study area are Ammonia beccarii, Rosalina globularis, Buccella granulata, Planorbulina mediterranensis, and Lobatula lobatula. The presence of wastewater in the study area has not polluted the foraminiferal assemblages (absence of anomalous shells). The direction of the discharge plume is a potential source of nutrients for deep water.
