RESUMO

The authors report on a 3-year experience of interventions by a psychomotrician in the waiting-room of a mother-child protection (PMI) centre in order to recognize and prevent mother-infant interaction difficulties. The main mission of the psychomotrician was to help mothers to discover the competences and the desire of communication of their infants. This experience appears to be positive, allowing the improvement of some mother-infant difficulties, and a better acceptance by the mothers of a child psychiatric consultation when it appears to be necessary.

From an epidemiological study of post-partum maternal depression, the authors have identified several risk factors which have to be taken in account in order to establish preventive measures. The early separation of the mother and her neonate is a stressful event. The "hostile" form of post-partum depression must be recognized as such; it can be misinterpreted, and may result in rejection of the mother by the hospital staff. Team-work is therefore necessary so that this particular attitude can be recognized and understood, and that at-risk-mothers may be correctly taken care of, thus preventing resulting interactional disturbances and child abuse. Some mothers refuse treatment proposals, which may have serious consequences for the child. In maternal depression, a rapid intervention by the mothering delegation may lead to a "psychological abduction" of the child and subsequent difficulties in mother-child relationship. The authors emphasize the need for early detection and treatment of puerperal maternal difficulties in order to prevent disturbances in infant-mother interactions.

RESUMO

Cyclops represents the worst form of a rare malformation of the brain and face: holoprosencephaly. Two cases are reported, including one that was diagnosed at the 23rd week of pregnancy. Now, ultrasonography makes it possible to diagnose early; as soon as the 15th week of gestation. Ultrasound diagnosis is based on hypotelorism and anomalies of the mid-line, with or without facial abnormalities. The outcome is always poor. This malformation is associated with various conditions and a complete assessment, including the karyotype, morphologic studies of the fetus and of his family, and search for related minor facial dysmorphias (median labial or cleft palate with, with a single upper incisor) is mandatory. The condition recurs in 2 to 50% of cases depending on various associated conditions and on the family histories.
