RESUMO

PURPOSE: To evaluate the cause and management of paraurethral abscess developing after injection of a mixture of hyaluronic acid and dextranomer (Zuidex) for treating stress urinary incontinence. PATIENTS AND METHOD: A total of 127 women having undergone midurethral Zuidex injection and 34 women after endoscopically guided Deflux injection into the tissue around the bladder neck were followed up 1 to 24 months after the intervention. At follow-up, the paraurethral tissue was evaluated clinically by gynecologic examination and by introital ultrasound. RESULTS: Thirteen of 127 women (10 %) having undergone midurethral Zuidex injection had a sterile paraurethral abscess that was treated by transvaginal puncture (1 to 3 punctures with removal of 10-60 ml of fluid per patient). No case of postoperative paraurethral abscess formation was demonstrated in the control group treated by Deflux injection into the area around the bladder neck. CONCLUSIONS: Paraurethral abscess must be excluded in the postinterventional follow-up of patients after Zuidex injection. Patients in whom an abscess is demonstrated can be treated by transvaginal puncture. Abscess formation can be avoided by injecting dextranomer/hyaluronic acid into the area around the bladder neck.

In a retrospective analysis postoperative courses of 879 patients (1987-1989) have been analysed with respect to micturition behaviour and incidence of postoperative urinary tract infections. 58.5 per cent urinary tracts infections could be registered following uro-gynecology operations and 48.9 per cent after radical hysterectomies according to Wertheim-Held. The lowest rate of 15.9 per cent we found after simple interventions at the adnexas. Within the uro-gynecological operations vaginal plastic procedures had a higher rate (66.4 per cent) than colposuspensions with 41.5 per cent. Because of the different extent of the bladder preparation there were distinct differences with regard to first spontaneous micturition and lack of residual urine.

In 37 women being operated on gynecologically small portions of mesenchymatic tissue have been sampled and analysed with respect to the presence and concentration of estrogen and progesterone receptors within the cytosol fraction. The proofs were taken from: ligamentum cardinale, ligamentum sacrouterinum, pubo-vesico-cervical fascia, paracolpium, ligamentum teres uteri, levator muscle and serving as a comparison tissue from fornix vaginae posterior. In the proofs of pubo-vesico-cervical fascia and paracolpium there were no progesterone receptors. In all the other tissues there were as a matter of principle estrogen and progesterone receptors. However from sample to sample there were wide differences with reference to the existence and the concentration of hormone receptors. There was no correlation to the age of life. The results are discussed and compared with those to be found in literature. RESUMO

Using a self-made double-balloon-catheter the instillation of 2.5 mg Prostaglandin F2 alpha into the extraovular chorio-decidual space was down 46 times in 42 late pregnant women. The indication was the unripe cervix uteri in case of urgent parturition. This method is more effective than the pericervical and endocervical application, respectively. Under continuous supervision using external CTG there were neither complications in fetus nor in the mother.

RESUMO

In 21 women suffering from a severe motoric urge syndrome have been operated on with partial denervation of the urinary bladder according to Ingelman-Sundberg. The procedure was performed in 18 women unilaterally and in 3 bilaterally. Because of combined urge-stress-incontinence in 3 cases the plastic repair of pelvic floor was performed additionally with the unilateral nerve resection. With 5 recurrencies there has been a success rate of 75%. Out of 5 recurrencies 3 patients have been operated on twice, in 2 of them successively. The method founded by Ingelman-Sundberg is recommended as a ultima ratio for patients suffering from severe motoric urge syndrome which does not respond to conservative treatment.

RESUMO

First report about therapeutic experiences with Mictonorm (Propiverinhydrochlorid) in the field of urogynaecology. Mictonorm is a new preparation and acts as a parasympathicolytic (anticholinergic) agent and also as a direct musculotropic relaxans. In 25 patients suffering from urge- or combined urge-stress incontinence a significant increase in the capacity of the urinary bladder, a significant decrease of the intravesical pressure and a distinct reduction of the complaints has been registered. Mictonorm is recommended for the treatment of urge-incontinence and other diseases with urge-symptoms (urgency, pollacisuria, enuresis etc.) and also as a adjuvant in the treatment of acute or chronic cystitis.
