RESUMO

PURPOSE: To determine anatomic and visual acuity outcomes of posterior segment complications after ganciclovir implant surgery. METHODS: We reviewed the medical records of 63 patients with acquired immunodeficiency syndrome who had active cytomegalovirus retinitis in 82 eyes and who underwent 110 consecutive ganciclovir implant procedures. Preoperative and postoperative visual acuity, type of postoperative complication, treatment, and lines of visual acuity change were determined. RESULTS: Thirty-eight eyes of 19 patients had bilateral ganciclovir implant procedures, and 25 eyes of 19 patients underwent two or more ganciclovir implant procedures. Thirteen (12%) of 110 ganciclovir implant procedures developed posterior segment complications: rhegmatogenous retinal detachment in six, vitreous hemorrhage in four, endophthalmitis in two, and cystoid macular edema with epiretinal membrane in one. Treatment included pars plana vitrectomy with silicone oil in two cases and without silicone oil in three cases, scleral buckling in one, intravitreal antibiotic injection in two, and laser photocoagulation in two. Overall, median visual acuity was 20/25 preoperatively. Median follow-up was 6 months for all eyes and 7 months for eyes with complications. Postoperative median visual acuity was 20/25 for eyes without complications vs 20/40 at 1 month, 20/60 at 3 and 6 months, and 20/100 at 12 months in eyes with complications (P < .001). More eyes with than without complications lost 2 or more lines of visual acuity (P < .001). CONCLUSION: Postoperative complications occurred in 12% of the ganciclovir implant procedures and were associated with decreased visual acuity despite treatment.

The Cincinnati Eye Bank had six corneoscleral rims in which Streptococcus pneumoniae was cultured after preservation in corneal storage media. To determine the survival of this organism under conditions common for corneal storage, gentamicin-supplemented McCarey-Kaufman (M-K) medium and chondroitin sulfate/Dextran medium (Dexsol, Ciron Ophthalmics, Irvine, CA, U.S.A.) were inoculated with S. pneumoniae and kept at 4 degrees C. Thioglycollate broth plus 10% rabbit serum (Thio-S) and tryptic soy broth (TSB) served as growth controls. At day 14 after inoculation of 10(5) colony-forming units (CFU)/ml, Dexsol showed a 1-log decrease in bacterial concentration, the M-K medium a 2-log decrease and Thio-S a 4-log decrease, whereas TSB showed no detectable organisms. By day 21 Dexsol had only a 2-log decrease in bacteria. These data suggest that corneal storage medium supplemented with gentamicin does not exert bactericidal activity against S. pneumoniae and may actually support its survival at 4 degrees C.

RESUMO

This report describes a case of traumatic corneal rupture 22 months after radial keratotomy in a 39-year-old woman. The patient struck her face on the side of her car resulting in a limbus rupture in her cornea through the horizontal incisions of a four-incision radial keratotomy. An orbital blowout fracture was also present. A sclerotomy was performed to drain persistent choroidal detachment. After the hemorrhage in the anterior chamber resolved, the iris and lens were found to be absent. Visual acuity of 20/40 was achieved with aphakic correction.

RESUMO

Chronic hematic cysts of the orbit are rare causes of proptosis. They progressively enlarge causing expansion or erosion of the orbital bones and may give the appearance on computed tomography (CT) scan of a malignant orbital process. Hematic cysts have a characteristic appearance on magnetic resonance imaging (MRI) which is virtually pathognomic. The authors present two cases in which MRI was used to establish the diagnosis.
