RESUMO

The aim of this work is to review current trends in the minimally invasive treatment of adenomyosis, in particular uterine artery embolization (UAE). Adenomyosis is an unsolved problem in modern gynecology, its clinical manifestations and associated infertility lead to a decrease in the quality of life of women. Advances in imaging are making it possible to diagnose adenomyosis in young women. Today, conservative methods of treating adenomyosis and maintaining fertility are an urgent problem. Uterine artery embolization is a common treatment for uterine fibroids and is gaining popularity as a treatment for adenomyosis. The data reviewed report minimal side effects of UAE and indicate the effectiveness of this method for the treatment of adenomyosis in the short and long term. Despite the promising results, the issue of using UAE for the treatment of adenomyosis requires more detailed study.

RESUMO

The article deals with analysis of technical and anatomical factors influencing the efficacy of uterine arteries embolization (UAE) in treatment of patients with uterine myoma. Based on experience gained in 420 interventions, the authors revealed that inefficiency of UAE may be attributed to the following causes: 1) a technical failure associated with anatomical peculiarities of the uterine artery or a spasm thereof; 2) incomplete embolization apparently attributable to both technical and anatomical factors; 3) an adventitious restoration of blood supply to the uterine myoma. We also determined the significance of utero-ovarian arterial anastomoses in blood supply of the myomatous nodes, suggesting technical manipulations making it possible to attain adequate embolization thereof, if such. Technical and anatomical factors play a decisive role in successful outcomes of UAE, which, if failed due to restoration or preservation of blood supply of the myomatous nodes, should be followed by a repeat intervention.

RESUMO

Endovascular rotary fragmentation using a modified pigtail catheter was provided to 20 patients with massive pulmonary thromboembolism. Fragmentation wes supplemented by the course of standard thrombolytic therapy (13 patients) or by the shortened treatment course (3 patients). Just after intervention the Miller index dropped on the average from 28.2+/-2,56 marks to 25.5+/-2.1 marks. Systolic pressure in the pulmonary artery increased on the average from 50.9+/-13.08 up to 51.9+/-13.06 mm Hg, One female patient died 1.5 h after intervention because of cardiopulmonary insufficiency progression, 3-11 days after intervention (mean 6.3 days) the patients underwent repeated cardiopulmonography. Within this period the Miller index constituted 16.7+/-4.3 marks on the average, systolic pressure in the pulmonary artery was equal to 33.8+/-9.45 mm Hg. Endovascular rotary fragmentation allows to reach swift recanalization of embolic occlusion and thus to gain a "gold hour" for thrombolysis. It improves the results of thrombolytic therapy and can become an original modality for the treatment of patients contraindicated thrombolysis.

RESUMO

The authors analyze the possibility of predicting the results of aphakia correction by interlamellar refraction keratoplasty from one of the eye parameters measured before surgery, the anterior corneal surface radius. The prognosis efficacy has improved by 10% vs. the prediction from the mean value; this is insufficient for practical application of the method. The prediction may be made more accurate if the number of the eye parameters measured before surgery is increased and the measurements are more precise.

RESUMO

Problems arising in devising the assisted circulation apparatus and artificial heart can be solved only with investigations conducted on hydrodynamic models of the circulation system--the stands. An analysis of literature sources dealing with test stands justifies their classification according to the purpose, the types of tests, the specific structure and basic characteristics. It is necessary to proceed with further improvement of the stand equipment in the direction of creating a sufficiently accurate imitation of the anatomic structure and resilient properties of major arteries and veins, of constructing a geometrical and elastic model of the aorta, of pressure and flow control instruments at different points of the vessels, of the ones regulating the energy parameters, as well as of working out criteria for effectiveness of the devices to be used.

RESUMO

A systematization of methods for assisted circulation, based on the formulation of a concrete procedure, the nature of the deranged or lost function substitution and conditions for effective realization of such a substitution, is offered. This made it possible to classify the equipment for assisted circulation and to analyze the current trends of its further development. The most characteristic automation levels of the equipment under consideration and of generations of some apparatus, differing in the degree of automation, control and monitoring the process of assisted circulation, are presented in the form of tables and pictures.
