RESUMO

Popliteal and femoral aneurysms account for about 70% of the incidence of peripheral aneurysms. Only 4% of aneurysms run their course with the clinical evidence of rupture. The ruptured aneurysm is dangerous because of a potential limb loss as well as due to the threat of lethal outcome in patients at a high surgical risk. Presented herein is a successful experience (first in this country) gained with the treatment of a ruptured femoral aneurysm by means of endoprosthetics using a new stent graft with PTFB coating in a patient at a high risk for traditional surgical treatment.

RESUMO

The new method dynamic proprioceptive correction using a medical loading costume was included into complex therapy in patients with late-stage residual stroke. Application of the costume promoted normalization of complex locomotor acts of walking by correcting proprioceptive pulsation resulted from the system of elastic draughts. Thus, a new motor stereotype was forced upon the patients. Clinical observation, computer analysis of the motor potential, spectrum of heart rhythm variabilities before and after therapy, and psychological testing were performed in 120 patients with motor disorders (pareses, paralyses) resulted from acute cerebral circulatory disorders. A steady-state clinical effect (p < 0.05) was found in 71.9% of the patients after 15 sessions of therapy. The patients became to walk independently, a possibility to self-attendance appeared, their speech became better. Lateralization of a damage and preservation of both the most significant homeostatic vasomotor mechanisms and the cerebral mechanisms of preparation of a movement with the minimal manifestations of emotional-affective disorders were shown to have impact on the patients' rehabilitation and on the early clinical features of pyramidal defect.

RESUMO

In 48 patients with hemorrhagic strokes accompanied by disorders of vital functions the authors conducted respiratory resuscitation. Artificial lung ventilation was made from 1 day to 6 months. Among the studied patients 10 of them with a lateral localization of the hematoma and blood penetration into the subarachnoidal spaces and perhaps into the brain ventricules survived, 27 died during the first 2 weeks from the beginning of resuscitation due to destruction of wide brain areas with penetration of blood into the brain ventricules. In more remote periods (from 2 weeks to 8 months following a stroke)--11 patients due to a pathology of the internal organs died. The achieved data indicate that respiratory resuscitation in a very severe development of a hemorrhagic stroke may be used in a limited group of patients with lateral localization of the hematoma.

RESUMO

Post-mortem morphological studies of the kidneys from 2 patients, who due to complications occurring in the post-operative period (operation cryothalamectomy) developed the myorenal syndrome and acute renal insufficiency, were carried out. Changes were noted in all parts of the nephron, stroma and vessels of the kidneys. The most characteristic of the myorenal syndrome was accumulation of a great number of pigmental cylinders in the lower parts of the nephrom with focal delatation of the departments located above. In pathogenesis of acute renal insufficiency in the myorenal syndrome along with impairment of the renal hemodynamics a mechanical block of the tubules with decomposition products of myoglobin is also of importance.
