[Diagnosis and combined treatment of esophageal chemical burns in children]. / Diagnostika i kompleksnoe lechenie khimicheskikh ozhogov pishchevoda u detei.

The experience of management of 426 children with chemical burns of the esophagus and the stomach has been analyzed. The necessity of earlier (up to 3 hours) lavage not only of the stomach, but also of the esophagus, is substantiated. It is considered advisable to carry out diagnostic fibrogastroesophagoscopy not later than 1 day after the trauma and to give up early prophylactic bouginage of the esophagus. The latfer rather stimulates sear formation than prevents it. The developed scheme of treatment was used in 256 patients with resultant decrease of postburn stenoses of the esophagus 3 times is the results of conventional methods of treatment.

[The topographic anatomical validation of variants in the surgical treatment of acute mediastinitis in children from the age aspect]. / Topografoanatomicheskoe obosnovanie variantov khirurgicheskogo lecheniia ostrogo mediastinita u detei v vozrastnom aspekte.

The article is based on 28 experimental studies and 74 clinical observations. The authors have distinguished fascial-cellular spaces of the mediastinum, noted their developmental changes and topographo-anatomical relationships, determined possible ways of spreading the pyo-inflammatory process in the mediastinal fat in children of different age groups. Substantiation of methods of drainage in acute suppurative mediastinitis is given in the age aspect, as well as the method of local detoxication. Differential approach to children with this critical pathology resulted in recovery of 64 of 74 sick children.

The authors discuss experience in 168 operations for extirpation and segmental resection of affected bronchi in children with bronchiectases. To prevent recanalization of the bronchus after its segmental resection and to prevent the formation of an abscess in the isolated part of the lung they suggest a method for intraoperative packing of the remaining bronchi with biopolymer material.

The authors discuss their experience in the use of a biopolymeric composition of a biological glue KL-3 and an antiseptic for preoperative filling of bronchiectatic cavities in 28 patients with bronchiectases. The filling is performed before the operation during bronchoscopy conducted under anaesthesia. As a result endobronchitis is relieved, the time needed for preoperative management is shortened, and the number of possible postoperative complications is reduced to minimum.

[Choice of a method of draining the mediastinum in children with anterior purulent mediastinitis]. / Vybor metoda drenirovaniia sredosteniia u detei s perednim gnoinym mediastinitom.

The authors make an analysis of results of the treatment of 10 children of early age with anterior purulent mediastinitis. Indications for different methods of drainage of mediastinum are discussed in the restricted and extended processes. The method of retrosternal drainage of mediastinum in its total length is described for extended purulent mediastinitis. The proposed technique in combination with medication and infusion therapy allowed to avoid lethal outcomes.
