The efficacy and the safety of bromocriptine 7.5 mg daily were investigated in a double-blind versus placebo trial in 32 patients with troublesome symptoms from benign prostatic hypertrophy. Differences between the treatments in respect of uroflowmetry, residual urine and safety parameters were not statistically significant. Differences in respect of nocturia, daytime frequency and overall efficacy were statistically significant in favour of bromocriptine. There could be a place for bromocriptine in the palliative treatment of the functional disability caused by benign prostatic hypertrophy when surgery is refused or contraindicated.

Evolution of fertility parameters in 374 patients who underwent surgical intervention (Palomo technique) for varicocele was studied. Preoperative motility, morphology, and concentration of spermatocytes did not correlate with degree of varicocele. Statistically postoperative parameters all were significantly improved and the left testicle volume increased. However, improvement did not correlate with degree of varicocele, age at intervention, or pregnancy rate. Extrapolation of results to subclinical varicoceles and treatment of varicoceles in adolescents still remains questionable.

Neurogenic dyssynergia was treated with alpha-adrenergic blockers. Only 21 of 43 highly selected patients with residual urines and low flow rates continued therapy with phenoxybenzamine for more than 6 months, despite initial benefits. The causes of therapy discontinuation are discussed. A combination of 5 mg dibenzylin, 3 times daily, with parasympathomimetic drugs is often necessary. The mechanisms of action of alpha-adrenergic-blocking agents are discussed.

Between 1963 and 1980, there were 151 resections by patients with rectal cancer. A good postoperative reanimation has decreased the mortality enormously. The corrected five years survival-rate comes to 58.5%. The corrected ten years survival-rate comes to 49.7%; 58.3% of the patients has been treated with sphincter-saving procedures. Though a bigger margin is desirable, a resection of three centimeters under the macroscopic most distal part of the tumor would be sufficient to remove eventual intramural spread. For a sufficient continence one needs : the internal and the external sphincter, the musculus levator ani, and the last centimeters of rectal mucosa at the same height of the anal canal till the linea pectinea. This is pursued with sphincter-saving operations. The younger the patient is the better the function. Psycho-social considerations play a prominent part in the treating of the patient and the evaluation of the results.
