RESUMO

An analysis of short- and long-term results of the combined treatment of tumors of the trachea and adjacent organs with the involvement of the trachea in 100 patients suggests that this method holds promise. In 41 patients the irradiation phase preceded operation, in the rest of 59 patients it followed operation. Radiotherapy was performed daily, 5 fractions every week at a single focal dose of 2-3 Gy. The summary focal dose was 36-48 Gy, for some of the patients 70-80 Gy. Adjuvant radiotherapy did not prevent the performance of complex reconstructive plastic operations with circular, distal or window resection of the trachea. Postoperative complications occurred in 16%, lethal outcome in 4%. Five-year survival following the combined treatment of tracheal tumors was observed in 57%.

The authors provide their data on the combined treatment of 50 lung cancer patients aged 40 to 70 using irregular postoperative irradiation. There were 47 men and 3 women; 5 patients had Stage I, 33 Stage II, and 12 Stage III. Histologically, 80% had squamous cell carcinoma of various degree of differentiation, 20% had differentiated bronchogenic adenocarcinoma. Central cancer was found in 26, peripheral in 24 patients. Postoperative irradiation of the mediastinal lymph nodes and primary tumor bed was given in case of an uneventful postoperative course 3-4 weeks after surgical intervention. gamma-Beam therapy was performed under the conditions of a static regimen using a lead perforated filter C-2; the average single focal dose was 3 Gy, with the daily irradiation rhythm, the summary focal dose was 36 Gy. Radiation therapy was adequately tolerated by the patients, no radiation complications were observed. The duration of life was: 1 year 90%, over 1 year 88.9%, over 2 years 64.9% and over 3 years 50%. In the course of 2-3 years recurrences were detected in 3 patients.

RESUMO

A detailed analysis of immediate and late results of reconstructive-plastic operations performed on 387 patients with carcinoma of the lung has been made. Lobectomy with a resection and plasty of bronchi and trachea were fulfilled in 325 patients (84,0 %) and pneumonectomy with a resection of the trachea bifurcation--in 62 patients (16,0 %) with carcinoma of the lung. The authors have studied the question of performing reconstructive-plastic operations for the combined treatment of carcinoma of the lung on 281 patients (72,6 %). The postoperative lethality following pneumonectomy (9,7 %) was not higher than after routine resections of the lung. A 5-year, 10-year and 15-year survival after lobectomy with a resection and plasty of the bronchi and trachea was 60,7. 41,8 and 30,3 % respectively. After pneumonectomy with a resection of the trachea bifurcation the 5-year survival was 20,4 %, 10-year survival was 4 out of 25 patient. Three of 7 patients survived for 15 years. The combined treatment of the lung carcinoma was found to give better results.
