[Reflexotherapy and evidence-based medicine].

Assessment of the role and place of nontraditional methods of treatment and reflexotherapy, widely applied in hospitals is presented in the article. Besides, we become alerted regarding not serious approach of some scientists and health service managers to reflexotherapy as a whole and to one of its methods acupuncture.

[Structural and functional state of the left and right ventricle and the factors that affect the parameters of intracardiac hemodynamics in patients with hypertension and chronic alcohol intoxication].

The parameters of endocardiac hemodynamics, heart rate variability, endothelial function and function of the external breathing (FEB) were studied. 67 patients with arterial hypertension (AH) at the stage II and with chronic alcohol intoxication (CAI) were observed. Method of correlation analysis allowed to determine the heart rate variability effect on the hemodynamics, endothelial function and respiratory systems in these patients. Patients with AH and CAI in comparison with those without the syndrome of CAI had dilatation of right ventricle (RV) and left auricle and aorta on a background of low (aged 24-44 years) or unchanged (aged 45-59 years) total peripheral vascular resistance. It was accompanied by ventricular diastolic impairment on a relaxation type, more shown in RV at patients of middle age. There are signs of sympathicotonia, obstructive disorders of the FEB in patients with hypertension and CAI.

[Place of reflexotherapy and some other methods of alternative medicine in modern medical practice].

Assessment of the role and place of nontraditional methods of treatment and reflexotherapy, widely applied in hospitals is presented in the article. An analysis of the situation developed in the legislation concerning training of reflexotherapy specialists for last 15-20 years not only in Ukraine, but also abroad was done. The article presents a historical parallel between the use of medicamentous and nonmedicamentous methods of treatment.

[Changes of the endothelial function and cardiac valves in patients with reactive arthritis].

Our investigations showed endothelium's malfunction and changes of the cardiac valves in patients with reactive arthritis, which could be precondition of the cardiac failure development in these patients.

[Role and place of military medical service in Ukraine in the state service of medicine of catastrophes].

The authors discusses in the article important questions related to providing medical care during emergency situations. Promising directions on creation of unique state medicine service of catastrophes and generalized plan of interaction of medical components refering to different ministries and institutions of the country were proposed by the authors. The role and place of medical care of military army of ukraine in state service of medicine of catastrophes were shown in the article.

[Principle military medical institution in Ukraine: history pages (to 250 anniversary of the foundation of Kiev Military Hospital)].

The article is dedicated to the history of the foundation, development of the principle military medical institution of Ukraine, which celebrates its 250 anniversary in June 2005. The authors proposed the periodization of the origin, main achievements and tendency of the development of the management of this military medical institution.

[The evolution of the clinical course of infectious endocarditis]. / Evoliutsiia klinicheskogo techeniia infektsionnogo éndokardita.

As many as 188 records of IE patients were studied as were 67 autopsies of deceased persons. Significant changes in the clinical course of IE were disclosed over the past 10-15 years versus previous 15: more prominent role of staphylococcal and gram-negative flora in the etiology of the illness, with microbial associations-caused IE cases having become more common. Diagnostic value of hemoculture did not appear to show much promise. The following items were found to be important risk factors for IE: prior viral and bacterial infection, invasive interventions, and chronic somatic diseases. There prevailed those IE forms having appeared on the intact heart valves in elderly persons and those at a mature age. In the course of the illness there tended to be more toxic and allergic manifestations, thromboembolic complications, and extensive vascular pathology.

[Immune disorders and their correction in patients with primary and recurrent pneumonias]. / Immunnye narusheniia i ikh korrektsiia u bol'nykh pervichnoi i povtornoi pnevmoniei.

Data are submitted of the analysis of indices for humoral and cell-mediated immunity in those patients presenting with recurrent pneumonias versus the above indices in patients with sporadic forms of the illness. Shown in patients with recurrent pneumonias is formation and progression of T-lymphocyte derangement associated with decline in their functional activity, which event is characteristic of different clinical types of acquired immune deficient states.

[The combined treatment of peptic ulcer with the differentiated use of nontraditional physiotherapeutic methods]. / Kompleksnoe lechenie iazvennoi bolezni s differentsirovannym primeneniem netraditsionnykh fizioterapevticheskikh metodov.

Efficiency was studied of multimodality treatment of 586 patients with ulcer disease. Of these, 110 had gastric ulcer, 460 patients had duodenal ulcer, sixteen were suffering from both gastric and duodenal ulcer. 275 patients (46.8%) had various concurrent chronic disorders involving cardiovascular system and alimentary canal. Three groups of patients were identified: those receiving magnetoresonance therapy apart from conventional antiulcer therapy (n = 89), those undergoing laser therapy (n = 170), those exposed to hyperbaric oxygenation (HBO), variable magnetic field (VMF) and acupuncture (AP) according to generally accepted techniques. In the control group, drug therapy promoted ulcer healing in 73.3% of patients, whereas multimodality treatment involving MRT induced healing in 80.6% that of incorporating computer-aided laser therapy led to healing in 85.7 to 89.4%. HBO, VMF and AP group demonstrated healing in 92.4 to 93.7%.

[Secondary pneumonias and their place in clinical cardiology]. / Vtorichnye pnevmonii i ikh mesto v klinicheskoi kardiologii.

As many as 1208 cards of myocardial infarction (MI) cases and 537 of those suffering from various heart diseases running a grave course and presenting with stage IIB-III chronic circulatory insufficiency were reviewed together with 289 records of anatomicopathological studies involving those having died from the above causes. Secondary pneumonias were found to occur in 7.8% of MI patients and 16 to 30.5% of those with grave cardiac abnormalities presenting with stage IIB-III chronic circulatory insufficiency. Secondary pneumonias in myocardial infarction were more commonly seen in elderly and old subject in the presence of cardiogenic shock, pulmonary edema, cardiac asthma, congestive heart failure, and other chronic diseases of the lungs. Hypodignosis of pneumonia was found to be the case in 25.2% of the deceased.

[The use of interstitial electrophoresis in the combined treatment of acute pneumonia with a severe course in young patients]. / Primenenie vnutritkanevogo élektroforeza v kompleksnom lechenii ostroi pnevmonii tiazhelogo techeniia u bol'nykh molodogo vozrasta.

The results of employing intratissue (intraorganic) electrophoresis in a complex of therapeutic measures applied in patients with acute pneumonia with a grave course in young individuals (n = 195) versus homogeneous control group of patients (n = 50) in whom intratissue electrophoresis was not done, were as follows: the percentage of those discharged from hospital in the former group exceeded those among control by 15.9%, the average bed occupancy was reduced by 5 days, the number of persons whom sick leaves had been granted dropped by 9.9% and there were no dismissals from the Army whereas in the control group these amounted to 6%. The present method of treatment used in combination with other therapeutic modalities promotes undelayed resolution of an acute pneumonia and prevention of certain complications in this category of patients.

[The mononuclear phagocytes and antiviral resistance in pneumonia]. / Sostoianie mononuklearnykh fagotsitov i protivovirusnoi rezistentnosti pri pnevmonii.

As many as 159 patients with pneumonia (all men aged 18-39) were examined. Functional activity of monocytes was studied in an integral test with nitroblue tetrazolium as was nonspecific cellular antiviral resistance by the level of viral damage to the monocytes and lymphocytes. The progression of pneumonia was found to be accompanied by a decrease in the number of phagocytizing mononuclears and the activity of phagocytosis. Nonspecific cellular antiviral resistance is significantly impaired. The proportion of monocytes and lymphocytes containing viral inclusions increases several-fold. The treatment of patients is accompanied by positive dynamics of indicators of functional status of monocytes and nonspecific cellular antiviral resistance. By the time of discharge from hospital the impaired functions of immunocompetent cells fail to completely restore.

[Monocyte functional activity and nonspecific antiviral cellular resistance in adaptation to the conditions of eastern Siberia]. / Funktsional'naia aktivnost' monotsitov i nespetsificheskaia protivovirusnaia kletochnaia rezistentnost' pri adaptatsii k usloviiam Vostochnoi Sibiri.

Monocyte functional activity and antiviral cellular resistance were studied in the newcomers to the Baikal region from the European Russia. A total of 105 patients with acute respiratory diseases and 30 normal subjects were examined. 46 healthy residents of St. Petersburg made up the reference group. The process of adaptation was found to be associated with changes in the functional activity of the immunocompetent cells. The authors came to a conclusion on the principal role of viral injury of the monocytes in the development of acute bronchitis or pneumonia.

Functional activity of mononuclear phagocytes and antiviral cellular resistance have been examined in 75 patients with acute bronchitis, 159 pneumonia patients and 46 healthy males as shown by intracellular viremia in monocytes and lymphocytes. Progression of acute bronchitis and pneumonia conformed to inhibition of phagocytosis in mononuclear phagocytes, growing percentage of virus-affected monocytes and lymphocytes.

[The clinical course of acute glomerulonephritis in Transbaikalia]. / Klinicheskoe techenie ostrykh glomerulonefritov v usloviiakh Zabaikal'ia.

452 case histories were analysed of patients with acute glomerulonephritis who were treated at the specialized unit of military hospital. 96.1% of these patients were enlisted men at the age of 18-20. Acute respiratory diseases had a great significance for etiological structure of acute glomerulonephritis. About 1/4 of all cases took place after the purulent affections of skin or subcutaneous fatty tissue. In the conditions of the Trans-Baikal region the clinical course of acute glomerulonephritis was basically represented by manifestative form of illness with pronounced swelling syndrome, arterial hypertension and typical changes in urine.

[Monocyte functional activity and nonspecific antiviral cellular resistance in a group at high risk for developing acute pneumonia]. / Funktsional'naia aktivnost' monotsitov i nespetsificheskaia protivovirusnaia kletochnaia rezistentnost' v gruppe povyshennogo riska razvitiia ostroi pnevmonii.

Overall 132 patients at greater risk for pneumonia were examined for functional activity of monocytes and antiviral cellular resistance according to the status of intracellular viremia in monocytes and lymphocytes. The control group included 46 healthy men. Proneness to pneumonia was discovered to be related to abnormal function of mononuclear phagocytes and a decrease of antiviral cellular resistance. The conclusion is made about the necessity of the control over functional activity of monocytes, viremia in immunocompetent cells and advisability of carrying out immunomodulatory and antiviral therapy.

[Pharmacological action on antiviral resistance in patients with acute pneumonia]. / Farmakologicheskoe vozdeistvie na protivovirusnuiu rezistentnost' bol'nykh ostroi pnevmoniei.

A study of 30 patients suffering of acute pneumonia indicates that the development of acute pneumonia occurs against the background of disorders of the functional activity of monocytes with prevalent reduction of the functional activity of phagocytosis and inhibition of the unspecific cellular antiviral resistance. Leucocytic interferon sharply activates phagocytosis. The number of phagocytic cells and activity of phagocytosis reduced and the number of virus-involved immunocompetent cells increased under the effect of antibiotics.
