RESUMO
A 71-year-old man developed a large multi-loculated subdural empyema following the evacuation of a chronic subdural haematoma. The pockets of pus were successfully evacuated endoscopically via the burr holes resulting in good recovery and no re-accumulation. The advantages of this technique and the difficulties encountered during this procedure are discussed.

Some new instruments for anterior cervical fusion of the spine are described. They are believed to be superior to those currently available.

Intermittent catheterization has been well described and found to be very successful in the management of what is possibly the major cause of severe illness, even death, in paraplegia - namely bladder infection. It is an extremely simple technique to introduce and contrary to widespread opinion, does not entail a great deal of time and effort. The time and effort spent treating urinary tract infections, not to mention the drug costs, can be astronomical. Many simplifications have been described, and the patient usually needs one or two consultations before he becomes an expert in the technique of self-catheterization. Unfortunately some spinal units and doctors are slow to introduce this technique. This is a very grave mistake as the well-being of the paralysed patient can be greatly facilitated by its diligent use. Early stabilization of the spine is also discussed.

A black male laborer was admitted with a classic carpal tunnel syndrome. He was examined, and an electromyelogram revealed compression of the median nerve. This was released surgically. The nerve was found to be compressed within the tunnel by an anomalous median artery. This would appear to be the first recorded case of a carpal tunnel syndrome caused by an unthrombosed anomalous persistent large median artery.
