BACKGROUND: Pediatric transphyseal anterior cruciate ligament reconstruction (ACLR) bears several advantages and is widely used. The main concern is the risk of growth disturbance. Our purpose was to investigate the incidence and risk factors of growth disturbance in skeletally immature patients who underwent transphyseal ACLR. We hypothesized that this procedure would generate neither clinically relevant limb length discrepancy (LLD) nor axis deviation. METHODS: This prospective, consecutive, single-center series included skeletally immature patients who underwent primary transphyseal ACLR using semitendinosus tendon autograft, with a 2-year follow-up bone length standing radiograph of both lower limbs from pelvis to ankle in anterior posterior view. Lower limb length, mechanical axis deviation (MAD), lateral distal femoral angle (LDFA), and medial proximal tibial angle (MPTA) were measured. The definition of postoperative growth disturbance was defined as ≥10 mm for LLD or ≥3 degrees for axis deviation in comparison to the contralateral lower limb. Predictive variables included age at surgery, gender, side, and diameter of bone tunnels. Student or Mann-Whitney test was used for numerical variables, and Chi-square test or Fisher exact test was used for categorical variables. P values <0.05 were considered statistically significant. RESULTS: Fifty consecutively treated patients were included. Forty-seven patients (31 boys, 16 girls) with a mean age of 13.2 years (range, 9 to 16) at the time of surgery were available for analysis. Six patients had an LLD of at least 10 mm. Twenty-five patients had a difference in MPTA of a least 3 degrees (range, 5 to 8). Sixteen patients had a difference in LDFA of a least 3 degrees (range, 4 to 9). No patients presented with a clinical deformity or related symptoms. Regarding coronal alignment, there was no statistical difference in mechanical axis deviation, LDFA, or MPTA. Gender, side, age, and bone tunnel diameter did not influence growth disturbance. CONCLUSIONS: Transphyseal pediatric ACLR generated a high rate of growth disturbances (leg length discrepancy and axis deviation) although none clinically relevant. Mild proximal tibial axis deviation in patients operated on near skeletal maturity should be further investigated. LEVEL OF EVIDENCE: Level III. STUDY DESIGN: Case-control study.

INTRODUCTION: The 8-week lockdown for the Covid-19 epidemic in France restricted travel, and interrupted schooling and sports. The study hypothesis was that this exceptional situation temporarily altered childhood trauma epidemiology. MATERIAL AND METHOD: A prospective study was performed during the 8 weeks of lockdown. Pediatric traumatology emergency activity was compared to that during the same period in the previous 3 years. RESULTS: During lockdown, emergency consultations decreased by 50%. The number of patients operated on was 86% of that in the previous 3 years. Patients operated on during lockdown had a mean age of 7.6 years (median, 7.5 years) compared to 9.3 years (9.4 years). The rate of domestic accidents (59% versus 23%) and trampoline accidents (16% versus 5%) increased, while those of sport and locomotion-related accidents decreased. Wounds were more frequent, at 35% of procedures, versus 13% previously. The rate of surgery for upper-limb fracture deceased, while that of lower-limb fracture was unchanged. Distal forearm fracture was less frequent, as was distal tibial fracture. DISCUSSION: The present study found a 50% decrease in pediatric traumatology emergency activity during lockdown, without decrease in surgery. In case of renewed lockdown, we recommend reorganizing emergency admission to free teams for management of Covid-19 patients, while maintaining operative rooms for emergency surgery. A general public information campaign could help prevent domestic accidents and risk related to use of trampolines. LEVEL OF EVIDENCE: III.

INTRODUCTION: The transition from medical student to surgical resident is not a simple one. The aim of this study was to report the experience of a university hospital in the organization of the induction course for future surgical residents and the contribution of a video support in the learning of the suture. MATERIAL AND METHOD: We were able to study two consecutive years of students (October 2020 and 2021). Concerning the practical and technical workshops (learning suture) we carried out a comparative study between two groups of students. A group that had video support for learning suture (video group) and a group without video (control group). The evaluation of the suture was performed in a blinded manner by two supervising surgeons. The other practical workshop was drain fixation; the students did not have a video for this workshop. A comparative study was also performed for the drain fixation workshop between the two groups (video group and control group). A program of theoretical courses was also set up. This program is established according to the different future functions of the residents by integrating medico-legal notions and teamwork. Satisfaction questionnaires were given to the students and the answers were given two months after taking up their duties in the hospital (6 questions with Likert scale and 4 free questions). RESULTS: The cohort consisted of 58 students (29 each in 2020 and 29 in 2021). Comparative analyses of the evaluation of the suture workshops showed better performance in the video group compared with the group without video. The comparison of these two groups did not show significant differences in the drain fixation workshop. The theoretical teaching was broken down according to the students' future tasks and each speaker was a specialist in his or her field of expertise. The results of the questionnaires showed a desire on the part of the students to increase the time spent on practical workshops and theoretical forensic teaching. CONCLUSION: We were able to show through these two years of a program that we were able to offer a surgical resident preparation course. In addition, we have highlighted the contribution of a video support in the learning curve of the suture.

BACKGROUND: Hallux valgus (HV) in adults is an acquired pathology related to 1st metatarsal (M1) abduction. In children, it is related to abnormal M1 joint surface orientation. HYPOTHESIS: HV deformities in children and adults differ. OBJECTIVE: Descriptive study of radiologic abnormalities in a pediatric population of symptomatic HV, with comparison to a population without symptomatic HV. MATERIALS AND METHODS: Weight-bearing X-rays were studied in pediatric patients undergoing surgery for HV and in a control population. Measurements comprised M1P1, M1M2, DMAA, DM2AA, MPAA, and sesamoid subluxation index. RESULTS: Twenty-five patients (42 feet) were included in the HV group, and 16 patients (29 feet) in the control group. Mean age was 13 years in both. Interobserver reproducibility was excellent for M1P1, and good for M1M2, DMAA and DM2AA. In HV, 71% of feet showed M1M2 angle>12° and 98% DMAA>10°; DM2AA was>0°, except in 1 foot. DISCUSSION: Normal values are the same in children and adults: M1P1<15°, DMAA<10° and M1M2<12°. In the HV group, DMAA was systematically pathological, while M1M2 was pathological in only 71% of cases. Childhood HV is related to abnormal DMAA, sometimes associated with increased M1M2 angle, especially in severe forms. DM2AA assesses distal M1 joint surface orientation with respect to the M2 axis; in the HV group, it was systematically>0° (except in 1 foot). Thus, in case of concomitant DMAA and M1M2 abnormalities, DMAA is more severely abnormal than M1M2 (DM2AA=DMAA-M1M2). CONCLUSION: Childhood HV is mainly due to abnormal M1 joint surface orientation, sometimes associated with increased 1st metatarsal abduction. DM2AA reflects the balance between 1st metatarsal abduction and M1 joint surface orientation abnormality. LEVEL OF EVIDENCE: III.

STUDY DESIGN: This is retrospective comparative study. SUMMARY OR BACKGROUND DATA: Numerous studies have focused on the efficacy, safety, and restoration of thoracic kyphosis in adolescent idiopathic scoliosis surgery using posteromedial translation method with sublaminar bands and hook claws at the top. The relevance of exchanging the penultimate anchor, that is, the pedicle hook with a pedicle screw in a hybrid construction has not yet been assessed. OBJECTIVE: Our objective was to assess, in adolescent scoliosis Lenke 1 and 3 operated by posteromedial translation using sublaminar bands, the proximal fixation claw influence (transverse-pedicular hook vs. transverse hook-pedicular screw) for postoperative correction and the proximal junctional kyphosis (PJK) at 2 years follow-up. MATERIALS AND METHODS: A comparative monocentric retrospective study included adolescent idiopathic scoliosis thoracic requiring surgery, between 2015 and 2017, with 2 years follow-up. Clinical (complications, revision surgery, and scoliosis research society-30) and radiographic (coronal and sagittal parameters) assessment were reported. Radiologic PJK was defined by a proximal junctional angle increase of 10 degrees or more between postoperative period and the last follow-up. RESULTS: Sixty patients (age at surgery=16±2 y; mean Cobb angle=58.2±12 degrees) were divided into 2 similar groups according to upper fixation: transverse-pedicular hook (H-H) and transverse hook-pedicle screw (S-H). No statistical differences were found for correction parameters (P>0.05). We reported 2 cases of PJK in H-H group (N=2/30=6.6%) and none in S-H group (N=0/30) (P=0.1) none requiring revision. No infectious or neurological events were reported. Mean scoliosis research society-30 was 126±12.7 in H-H group whereas 129.3±10 in S-H group (P=0.4). CONCLUSIONS: No differences in postoperative correction and clinical results at 2 years follow-up were found. A claw with transverse-pedicular hook increase proximal junctional angle without significant increase on radiographic PJK incidence compared with a claw with transverse hook and pedicle screws.

Due to the danger of repeated exposure to X-rays for patients with Adolescent Idiopathic Scoliosis (AIS), reducing the number of radiographs is necessary. By using Surface Topography (ST), trunk asymmetry evaluation could be used. The number of radiographs required can be reduced by 30% when the radio is used only for patients with worsening ST. HYPOTHESIS: ST is a reliable technique for deformity monitoring in AIS. DESIGN: Observational prospective study. INTRODUCTION: The risk of curve progression in AIS is high during a growth spurt and necessitates regular radiographic follow-up, despite the adverse effects of repeated exposure to X-rays. AIM: The aim of this study was to determine a simple and reproducible parameter for deformity monitoring using ST in AIS patients. MATERIAL AND METHODS: Consecutive AIS patients with a Cobb angle between 10° and 40° were included. Every 6 months, X-ray and ST acquisitions were performed. Radiographic parameters and the ribs prominence curve calculated from ST were collected. This curve was deduced from the axial rotation of the 100 axial sections of the trunk. We analyzed correlations between the evolution of Cobb angle and the curve. RESULTS: 123 patients were included, 111 girls, 12 boys, mean age 12 years. The measurement of the curve had a good reproducibility (ICC: 0.816). ST differentiated patients with or without increased Cobb. (p = 0.0294). CONCLUSION: ST is useful for monitoring AIS. This device could reduce the number of radiographs by 30%. LEVEL OF EVIDENCE: II.

INTRODUCTION: The intervertebral disc (IVD) is made up of the annulus fibrosus (AF) and the nucleus pulposus (NP) - an inert hydrated complex. The ability of the IVD to deform is correlated to that of the NP and depends on its hydration. As the IVD ages, its hydration decreases along with its ability to deform. In adolescent idiopathic scoliosis, one of the etiological hypotheses pertains to the IVD, thus making its condition relevant for the diagnosis and monitoring of this pathology. HYPOTHESIS: IVD hydration depends on sex, age and spine level in an asymptomatic pediatric population. The corollary is data on a control group of healthy subjects. MATERIAL AND METHODS: A cohort of 98 subjects with normal spine MRI was enrolled; their mean age was 13.3 years. The disc volume and hydration of each IVD was evaluated on T2-weighted MRI sequences, using previously validated image processing software. This evaluation focused on the lumbar spine, from the thoracolumbar junction to the lumbosacral junction. It was assumed that IVD hydration was related to the ratio of NP and AF volumes. A mixed multivariate linear analysis was used to explore the impact of age, sex and spinal level on disc hydration. RESULTS: Disc hydration was higher overall in boys than in girls, but this difference was not significant. Hydration increased with age by +0.005 for each additional year (p=0.0213). Disc hydration appears to be higher at the thoracolumbar junction than the lumbar spine, although this difference was not significant. CONCLUSION: Through this MRI study, we established a database of non-pathological lumbar disc hydration as a function of age, sex and spinal segment along with 95% confidence intervals. AIMS: To compare the rates of sagittal and coronal correction for all-pedicle screw instrumentation and hybrid instrumentation using sublaminar bands in the treatment of thoracic adolescent idiopathic scoliosis (AIS). METHODS: We retrospectively reviewed the medical records of 124 patients who had undergone surgery in two centres for the correction of Lenke 1 or 2 AIS. Radiological evaluation was carried out preoperatively, in the early postoperative phase, and at two-year follow-up. Parameters measured included coronal Cobb angles and thoracic kyphosis. Postoperative alignment was compared after matching the cohorts by preoperative coronal Cobb angle, thoracic kyphosis, lumbar lordosis, and pelvic incidence. RESULTS: A total of 179 patients were available for analysis. After matching, 124 patients remained (62 in each cohort). Restoration of thoracic kyphosis was significantly better in the sublaminar band group than in the pedicle screw group (from 23.7° to 27.5° to 34.0° versus 23.9° to 18.7° to 21.5°; all p < 0.001). When the preoperative thoracic kyphosis was less than 20°, sublaminar bands achieved a normal postoperative thoracic kyphosis, whereas pedicle screws did not. In the coronal plane, pedicle screws resulted in a significantly better correction than sublaminar bands at final follow-up (73.0% versus 59.7%; p < 0.001). CONCLUSION: This is the first study to compare sublaminar bands and pedicle screws for the correction of a thoracic AIS. We have shown that pedicle screws give a good coronal correction which is maintained at two-year follow-up. Conversely, sublaminar bands restore the thoracic kyphosis better while pedicle screws are associated with a flattening of the thoracic spine. In patients with preoperative hypokyphosis, sublaminar bands should be used to restore a proper sagittal profile. Cite this article: Bone Joint J 2020;102-B(3):376-382.
