BACKGROUND: Axillary lymph node involvement is one important prognostic factor in breast cancer, but the way to access this information has been modified over the years. This study evaluated if axillary ultrasound (US) coupled with fine-needle aspiration cytology (FNAC) can accurately predict clinically relevant node metastasis in patients with breast cancer, and thus assist clinical decisions METHODS: This is a cross-sectional study with retrospective data collection of 241 individuals (239 women and 2 men) with unilateral operable breast cancer who were submitted to preoperative axillary assessment by physical exam, US and FNAC if suspicious nodes by imaging. We calculated sensitivity, specificity, and accuracy of the methods. We compared the patient's characteristics using chi-square test, parametrics and non-parametrics statistics according to the variable. RESULTS: The most sensible method was US (0.59; 95% CI, 0.50-0.69), and the most specific was US coupled with FNAC (0.97; 95% CI, 0.92-0.99). Only 2.7% of the patients with normal axillary US had more than 2 metastatic nodes in the axillary lymph node dissection, against 50% of the patients with suspicious lymph nodes in the US and positive FNAC. CONCLUSIONS: Axillary US coupled with FNAC can sort patients who have a few metastatic nodes at most from those with heavy axillary burden and could be one more tool to initially evaluate patients and define treatment strategies.

A biópsia do linfonodo sentinela (LS) é o procedimento padrão para as pacientes com axila clinicamente negativa. O seu exame intraoperatório ainda gera dificuldades na sua abordagem. Assim, o objetivo deste estudo foi avaliar a eficácia desse exame no câncer de mama. Foram avaliadas 342 pacientes que foram operadas na Unidade de Mama do Hospital Nossa Senhora das Graças em Curitiba (PR), no período de 2000 a 2012. No exame intraoperatrório eram rea lizados cortes longitudinais, ao longo do maior eixo, a cada 2 ou 3mm. Em seguida eram feitos imprints em cada face de cada fatia e, então, realizavam se cortes histológicos em criostato em três níveis. Tanto os imprints quanto os cortes eram corados com azul de toluidina. Em sua maio ria eram tumores T1c (n=151), e 60 (17,5%) delas apresentaram axila comprometida no exame definitivo. A acurácia foi de 92%, o valor preditivo negativo, de 91% e a taxa de falso negativo, de 8%. Não foram encontrados fatores de risco significativos para falência da técnica dentro dos parâmetros estudados.


Sentinel node (SN) biopsy is the standard of care for patients with clinically negative axilla. However, in traoperatory pathological exam remains as a controversial issue. So, the aim of this study was to evaluate its efficacy in 342 breast cancer patients operated at the Hospital Nossa Senhora das Graças Breast Unit in Curitiba (PR), in the period between 2000 2012. In the intraoperatory evaluation, all SNs were cutted in the major axis, in three levels, combining frozen sections with imprints, using toluidin blue. The majority of patients were T1c (n=151), and 60 (17.5%) had positive axila in the definitive pathology evaluation. Accuracy was 92%, predictive negative value was 91%, and false negative rate was 8%. We did not find any significant risk factor for false negative SN in this series.

INTRODUCTION: Chronic kidney disease (CKD) compromises the health and routine of the patient. On the fifth stage of CKD, the patient becomes eligible to start renal replacement therapy: hemodialysis (HD), peritoneal dialysis (PD) or kidney transplantation. The type of CKD treatment is essential to improving quality of life of the patient. OBJECTIVE: To compare the quality of life of CKD stage 5 patients who perform HD and home PD. METHODS: Cross-sectional study with data collection, by convenience, through the application of socioeconomic and KDQOL SF-36 questionnaires in HD and PD patients of the Pro-Renal Foundation and satellite clinics in Curitiba-PR. RESULTS: The sample was 338 patients, 222 HD and 116 PD. Average age: 54.4 years for HD group (± 15.28) and 58.00 for the DP group (± 13.99). The variables: work status (p < 0.05), encouragement by dialysis staff (p < 0.01) and patient satisfaction (p < 0.001) were in favor of DP; while physical functioning (p < 0.05) and emotional function (p < 0.01) were to HD. CONCLUSION: Objectively, PD was more favorable regarding quality of life, for the large number of items with significant results when compared to HD. However, the two variables of greatest significance found in HD (physical functioning and emotional functioning) ended up having a much greater impact on well-being and daily-life of the patient in the environment external to the clinic than those who were higher in DP, making HD the most favorable for patient quality of life.

Resumo Introdução: A doença renal crônica (DRC) compromete saúde e rotina de seu portador. No estágio V da DRC, o paciente torna-se elegível para iniciar a terapia substitutiva renal por hemodiálise (HD), diálise peritoneal (DP) ou transplante renal. O tipo de tratamento pode ser importante para melhorar a qualidade de vida do paciente. Objetivo: Comparar a qualidade de vida de pacientes renais crônicos em estágio V-D (em diálise) que realizam DP domiciliar ou HD. Métodos: Estudo transversal com coleta prospectiva, por conveniência, por meio da aplicação de questionários socioeconômicos e KDQOL-SF 36 em pacientes do ambulatório de DP e pacientes em HD da Fundação Pró-Renal e clínicas-satélite de Curitiba-PR. Resultados: Amostra de 338 pacientes, sendo 222 em HD, e 116 em DP. Idade média de 54,4 ± 15,28 anos para HD e 58,0 ± 13,99 para DP. Variáveis: situação do trabalho (p < 0,05), estímulo por parte da equipe de diálise (p < 0,01) e satisfação do paciente (p < 0,001) foram favoráveis à DP, enquanto que funcionamento físico (p < 0,05) e função emocional (p < 0,01) foram favoráveis à HD. Conclusão: Objetivamente, a DP mostrou-se melhor em relação à qualidade de vida por apresentar um maior número de itens com resultados significativos quando comparada à HD. Porém, as duas variáveis de maior significância encontradas na HD (funcionamento físico e funcionamento emocional) têm um impacto maior no bem-estar e no cotidiano no ambiente externo à clínica do que aquelas superiores na DP, tornando a HD mais favorável à qualidade de vida do paciente.
