BACKGROUND: Staphylococcus aureus has become the leading cause of bacteremia among previously healthy children older than 5 years. METHODS: We aimed to describe the infants presenting to pediatric emergency departments and diagnosed with S. aureus bacteremia (SAB) and identify predictors (clinical and laboratory variables) of poor outcome (complications, sequelae or death). We performed an observational study and subanalysis of a multicenter prospective registry, including every patient under the age of 18 years with a positive blood culture obtained at any of the 22 participating Spanish pediatric emergency departments between 2011 and 2016. We considered chronic diseases, immunosuppressive treatment and the presence of mechanical devices as risk factors for SAB. RESULTS: Of the 229 patients with SAB, 176 (76.9%) were previously healthy, 192 (83.8%) had a normal pediatric assessment triangle and 158 (69.0%) had an associated focal infection (mainly osteoarticular infection, skin and soft tissue infection and pneumonia). Fifty-three patients (23.1%) had 1 or more risk factors for SAB. Thirty-one (13.5%) presented a poor outcome, including 4 deaths (1.7%). A poor outcome was more common in patients with SAB risk factors [odds ratio (OR): 7.0; 95% CI: 3.2-15.4], abnormal PAT (OR: 5.9; 95% CI: 2.6-13.3), elevated procalcitonin (OR: 1.2; 95% CI: 1.05-1.3) and C-reactive protein, the latter being the only independent predictor of poor outcome (OR: 1.01; 95% CI: 1.01-1.02). CONCLUSIONS: Overall, children with SAB are previously healthy, appear well, and have an associated focal infection. One of 7 had a poor outcome, with C-reactive protein being the only predictor identified.

BackgroundTo investigate the impact of the COVID-19 pandemic and infection prevention measures on children visiting emergency departments across Europe. MethodsRoutine health data were extracted retrospectively from electronic patient records of children aged <16 years, presenting to 38 emergency departments (ED) in 16 European countries for the period January 2018 - May 2020, using predefined and standardized data domains. Observed and predicted numbers of ED attendances were calculated for the period February 2020 to May 2020. Poisson models and incidence rate ratios (IRR) were used to compare age groups, diagnoses and outcomes. FindingsReductions in pediatric ED attendances, hospital admissions and high triage urgencies were seen in all participating sites. ED attendances were relatively higher in countries with lower SARS-CoV-2 prevalence (incidence rate ratio (IRR) 2{middle dot}62, 95% CI 2{middle dot}19 to 3{middle dot}13) and in children aged >12 months (12-<24 months IRR 0{middle dot}89, 95% CI 0{middle dot}86 to 0{middle dot}92; 2-<5years IRR 0{middle dot}84, 95% CI 0{middle dot}82 to 0{middle dot}87; 5-<12 years IRR 0{middle dot}74, 95% CI 0{middle dot}72 to 0{middle dot}76; 12-<16 years IRR 0{middle dot}74, 95% CI 0{middle dot}71 to 0{middle dot}77; vs. age <12 months as reference group). The impact on pediatric intensive care admissions (IRR 1{middle dot}30, 95% CI 1{middle dot}16 to 1{middle dot}45) was not as great as the impact on general admissions. Lower triage urgencies were reduced more than higher triage urgencies (urgent triage IRR 1{middle dot}10, 95% CI 1{middle dot}08 to 1{middle dot}12; emergent and very urgent triage IRR 1{middle dot}53, 95% CI 1{middle dot}49 to 1{middle dot}57; vs. non-urgent triage category). Reductions were highest and sustained throughout the study period for children with communicable infectious diseases. InterpretationReductions in ED attendances were seen across Europe during the first COVID-19 lockdown period. More severely ill children continued to attend hospital more frequently compared to those with minor injuries and illnesses, although absolute numbers fell. ABSTRACT: Cutaneous clear cell proliferations encompass a heterogenous group of several primary cutaneous neoplasms and metastatic tumors with different histogenesis. Many of these clear cell proliferations may seem strikingly similar under the microscope resulting in challenging diagnosis. In many of these clear cell lesions, the reason for the clear or pale appearance of proliferating cells is unknown, whereas in other ones, this clear cell appearance is due to intracytoplasmic accumulation of glycogen, mucin, or lipid. Artifacts of tissue processing and degenerative phenomenon may also be responsible for the clear cell appearance of proliferating cells. Awareness of the histopathologic findings as well as histochemical and immunohistochemical techniques are crucial to the accurate diagnosis. This review details the histopathologic features of clear cell cutaneous proliferations, classifying them according their type of differentiation and paying special attention to the histopathologic differential diagnosis among them.

RESUMO

A new paediatric multisystem inflammatory syndrome, linked to SARS-CoV-2 (MIS-Paed), has been described. The clinical picture is variable and is associated with an active or recent infection due to SARS-CoV-2. A review of the existing literature by a multidisciplinary group of paediatric specialists is presented in this document. Later, they make recommendations on the stabilisation, diagnosis, and treatment of this syndrome.


Se ha descrito un nuevo síndrome inflamatorio multisistémico pediátrico vinculado a SARS-CoV-2 (SIM-PedS). Este cuadro presenta una expresividad clínica variable y se asocia a infección activa o reciente por SARS-CoV-2. En este documento se revisa la literatura existente por parte de un grupo multidisciplinar de especialistas pediátricos. Posteriormente se realizan recomendaciones sobre estabilización, diagnóstico y tratamiento de este síndrome.

Introducción: La clavícula es uno de los huesos más propensos a lesiones, debido a su localización subcutánea y relativamente anterior; su manejo, ortopédico o quirúrgico, sigue siendo controvertido. Por este motivo, el objetivo que guió la revisión fue justamente identificar las tendencias actuales en cuanto al diagnóstico y al manejo de la fractura de clavícula. Materiales y Métodos: Se realizó una revisión de artículos, usando las bases de datos PubMed/Medline, ScienceDirect, Embase, Ovid, SpringerLink. Se seleccionaron 57 referencias, con base en la calidad de la evidencia presentada por las mismas. Resultados y Discusión: La elección del tratamiento más adecuado para la fractura de clavícula sigue constituyéndose en un reto para el ortopedista. El análisis se debe basar en las características individuales de cada paciente, de una cuidadosa consideración de los beneficios y en los daños relativos de cada intervención, como también en las preferencias del paciente. Para avanzar en la definición de estándares terapéuticos se requiere la realización de estudios que tengan en cuenta un mayor número de individuos especialmente en los extremos de la vida y que permitan generar un mejor nivel de evidencia.


Introduction: The clavicle is one of the bones more prone to injury because due to their subcutaneous and relative anterior position. Its management, either orthopedic or surgical, remains controversial. Therefore, the objective that guided this review was to identify current trends in the diagnosis and management of broken clavicle. Materials and Methods: a review of articles was conducted using PubMed / Medline, ScienceDirect, Embase, Ovid, and SpringerLink databases. Fifty-seven (57) references were selected based on the quality of the evidence they presented. Results and Discussion: Choosing the most appropriate treatment for broken clavicle continues to be a challenge for the orthopedist. The analysis should be based on the individual characteristics of each patient, a careful consideration of the benefits and the relative harms of each intervention, as well as patient preferences. To advance in the definition of therapeutic standards, are required the performance of studies that take into account a greater number of individuals particularly in the extremes of life, which allow generating a higher level of evidence.

El presente artículo tiene por objetivo analizar la información actualizada, sobre el sistema de la coagulación sanguínea y reflexionar sobre el mismo. Se analizó la literatura disponible de los últimos 50 años en las bases de datos BBCS-LILACS, Fuente Académica, IB-PsycINFO, IB-SSCI, IB-SciELO, Scopus y Scirus, al igual que artículos históricos, textos y referencias citadas en trabajos públicos. Se obtuvo información pertinente relacionada con los objetivos propuestos en el presente artículo, por lo cual puede clasificarse en 2 secciones a saber: recorrido histórico de la coagulación, y lo actual: modelo celular de la hemostasia. Se concluye que en la actualidad, para una mejor comprensión del funcionamiento de la cascada de la coagulación, se han propuesto nuevos postulados que permiten conocer y diferenciar mejor dicho proceso. El modelo clásico de la coagulación sanguínea, en la actualidad presenta varios reparos y por tanto es poco probable que éste funcione en condiciones fisiológicas.


The objective of the present review is to analyze updated concepts related to blood coagulation, and to generate a reflection about it. Information from the last 50 years including the BBCS-LILACS, "Fuente Académica", IBPsycINFO, IB-SSCI, IB-SciELO, Scopus and Scirus databases, as well as historical articles, texts and references cited in public published papers to date were analyzed. Pertinent information related to the objectives proposed in the present article was found and analyzed. It was then divided into two sections as follows: history of coagulation and the cellular model of hemostasis. It can be concluded that new postulates have been presented today for a better understanding of the function of coagulation cascade, which allow knowing and differentiating in an improved way such process. Nowadays the classic model of blood coagulation shows several observations and then it is unlikely that in physiological conditions this can useful.
