[Nonstrain plasty of incarcerated hernias].

The article presents an experience with treatment of 129 patients with incarcerated hernias of the abdomen. Immediate results of the operative treatment with using plasty with polypropylene transplant were analyzed. The data obtained are compared with the results of plasty with local tissues.

[Complex use of ultrasound tomography in the algorithm of diagnostics for patients with acute destructive pancreatitis].

The article describes the experience of puncture aspiration biopsy in 136 patients (110 males and 26 females at the age of 19-74) with suspected destructive pancreatitis in one-day inpatient conditions, followed by microscopic and microbiological tests. In order to increase the diagnostic accuracy when detecting necrotic zones, 49 patients were examined by ultrasonic tomography using the color Doppler mapping and the method of energetic Doppler with subsequent spot puncture. The results of microbiological and cytological tests of the smear materials taken by fine-needle aspiration under ultrasonic control enable the authors to use the most efficient antibacterial therapy as well as to support the strategy of surgical treatment of destructive pancreatitis in its various forms in the conditions when clinical differential diagnosis is complicated. These methods increase the efficacy of medical treatment for this category of patients.

[Pancreatoduodenal resection in treatment of cancer of pancreatic head and periampullar zone]. / Pankreatoduodenal'naia rezektsiia v lecheniia raka golovki podzheludochnoi zhelezy i periampuliarnoi zony.

Results of 120 pancreatoduodenal resections are analyzed. Early postoperative period complications were seen in 51 (42.5%) patients. Overall lethality during 30 days after surgery was 13.3%. Over the last decade lethality reduced to 5%. Life time was studied in 91 patients with cancer of the pancreas (PS) and periampullar zone (PAZ) with Kaplan-Meyer method. Median of life of patients with cancer of PS was 14 months, Vater's papilla (VP)--37 months, terminal part of common bile duct (TPCBD)--30 months, duodenum--30 months. One-, 3-, 5-year survival in cancer of PS was 53, 11 and 3%, in cancer of VP--80, 50 and 31%, in cancer of TPCBD--80, 20% and 0, in duodenal cancer--80, 50 and 33%, respectively. Prognosis factors correlating with the survival were following: 1) primary location of tumor; 2) size of tumor; 3) radicality of surgery; 4) regional lymphatic metastases; 5) invasion of vessels by tumor; 6) index of oncomarker CA 19-9. Early diagnosis of cancer of PS and PAZ, optimization of surgical policy and techniques, using other methods in complex improve results of pancreatoduodenal resections.

[The results of 5-year-long research of sclerotherapy of liver metastases by ultrasonography]. / Piatiletnie rezul'taty skleroterapii metastazov pecheni pod UZ-kontrolem.

64 patients with liver metastases have been studied (mammary gland--6 cases, stomach--15, colon--32, other locations--11) and US metastasis semiotics and the fine aspiration biopsy have been identified. 12 cases had underwent intraoperative 96% ethanol infusion, 52 had got transdermal infusions under US control. The survival rate of colon cases with dissemination was: 1 year S.--100%, 3 year S.--53.1%, 5 year S.--3.1%. In cases with noncolon dissemination the percentage of survival was: 1 year S.--68.8%, 2 year S.--9.4%, 3 year S.--3.0%. 96% ethanol sclerotherapy proved to be more effective in single metachronic metastases of the colon malignancies.

[Combined method of ultrasound tomography in patients with local diseases of pancreas and abdominal abscesses]. / Kompleksnoe primenenie metoda ul'trazvukovoi tomografii pri ochagovykh porazheniiakh podzheludochnoi zhelezy i abstsessakh briushnoi polosti.

We performed US control of the pancreas lesions and upper abdominal abscesses and malignant according to 94 descriptive US images signs. We also used miniinvasive diagnostic involvements and small curative techniques. Diagnostic efficacy of US technology findings includes: sensitivity--90.5%, specificity--85.0% and accuracy--89.2%.
