AIM: The aim of the study was to evaluate the efficacy of the professional bleaching treatment with Pola Office (SDI): the shade, chroma and value parameters of the anterior upper elements were analysed. METHODS: At the Prosthetic Unit of the Modena and Reggio Emilia University, 10 patients were selected. All patients underwent oral hygiene treatment and spectrophotometric analysis (Spectro-Shade) of the 6 front elements: the variables L (value), c (chroma), and h (shade) were examined in the cervical, mesial and incisal sections. Spectrophotometric analysis was repeated 6 months after initial bleaching. The patients were asked to report the degree of sensitivity after 3, 6, 12 and 24 h and after 3, 6, 7, and 10 days. RESULTS: The value (L) of the treated elements increases after bleaching, the chroma parameter (c) showed lower values than those prior to treatment (except for the first patient); this means a shift towards a lower degree of saturation. The course of the shade parameter is uniform and constant for all patients, with a shift towards yellow chroma values. Postoperative sensitivity was confined to the first 3 h after treatment, after which it showed a steady decrease over 24 h. CONCLUSIONS: Esthetic results obtained using the Pola Office bleaching procedure were visibly significant; confirmed by colorimeter analysis. The harmony between chroma, shade and value remained 6 months after the bleaching treatment; the sensitivity is particularly high in the first hour, however after 24 h sensitivity is greatly reduced.

OBJECTIVE: Various dental procedures can generate thermal increase in the dental pulp, in particular if they are incorrectly conducted. In literature the pulp cells are considered very heat sensitive. In this study the symptomatological response of patients during and after thermal administration to the dental crown has been recorded. The analysis of the symptomatology was used as an indication for the definition of the threshold of thermal damage. EXPERIMENTAL DESIGN: Twelve healthy teeth of four patients programmed for extraction were subdivided into six couples of homologous teeth. One tooth out of each couple was used for the immediate in vivo recording of the symptoms; the other, once extracted, was used to determine the thermal increase applied through the insertion of a thermocouple sensor. In each couple of teeth the thermal stimulus was equal. RESULTS: The average thermal increase was 11.2 degrees C. Pain starts at temperatures ranging from 39.5 to 50.4 degrees C with an average of 44.6 degrees C. This agrees with classical physiological data which reports the threshold of pain at 45 degrees C. CONCLUSIONS: The threshold of pain registered suggests that at temperatures below 44.6 degrees C damage to the dental pulp is improbable, at least in healthy teeth. The limit of 45 degrees C appears, therefore, to be a probable safe threshold, contrary to what is reported in literature.

Surgical treatment in patients with heavy coagulation disorders needs measures that minimize immediate and post-surgical hemorrhagic risk. Besides the subjects afflicted with constitutional coagulation pathologies such as haemophilia A, B, C, AB, Von Willebrand disease, platelet disorders and others, too many are the patients that need pharmacological therapy to impede the creation of intra-vascular coagula. Today, surgical treatment of these patients is possible with the use of human fibrin glue (Tissucol) that permits excellent surgical and post-surgical hemostasis without therapy suspension, independently of coagulation disorder. The purpose of this study is a clinical evaluation of a surgical procedure based on the use of human fibrin glue in association or not with suture, hemostatic sponge and lyophilized dura mater in patients with coagulation disorders. In a group of 309 patients in oral anticoagulant therapy have been done a total of 822 operations such as root fragments extraction with mucoperiosteal flap elevation, teeth or roots simple extractions, third molar extractions, cyst removal, apicoectomies. Only patients with PA over 20% have been treated. All the operations have been leaded with minimal trauma; after the extractions, alveolar margins have been regularized to improve flaps adaptation. Hemostatic sponge or dura mater have been used like basis for the flaps and the fibrin glue, that was injected in the residual cavity after surgical suture. In a few cases, for example with little extraction wounds, only fibrin flue without suture or sponge has been used. In all cases and after all operations, the presence or absence of bleeding has been evaluated after 2 hours, 24 hours, 7 days after surgery.(ABSTRACT TRUNCATED AT 250 WORDS)
