RESUMO

BACKGROUND: The efficiency of the application of normobaric interval hypoxic therapy and enteral oxygen therapy for the treatment of the patients suffering from chronic obstructive pulmonary disease remains to be confirmed. AIM: The objective of the present study was the identification of the pathophysiological mechanisms responsible for the efficiency of normobaric interval hypoxic therapy and enteral oxygen therapy in the patients presenting with a chronic obstructive pulmonary disease. MATERIAL AND METHODS: A total of 230 patients at the age from 45 to 60 years were examined. All of them presented with chronic obstructive pulmonary disease (COPD). 150 patients were given a course of normobaric interval hypoxic therapy and enteral oxygen therapy while the remaining 80 ones constituted the control group. RESULTS: It was shown that the combined application of normobaric interval hypoxic therapy and enteral oxygen therapy provided an effective tool for the correction of the health status in the patients with chronic obstructive pulmonary disease. This method allowed to improve arterial blood supply with oxygen, increase the blood hemoglobin level and its oxygen capacity, augment oxygen tension and content in the arterial blood. The combination of the intake of oxygen cocktails and hypoxic therapy resulted in the improvement of metabolic processes in the lungs manifested as the alteration in the characteristics of the condensate of the exhaled air. The decrease in the content of malondialdehyde gave evidence of the reduction of lipid peroxidation while the enhanced activities of glutathione peroxidase and superoxide dismutase in blood confirmed the activation of the antioxidant system. The improved oxygen supply at all stages of its mass transfer along with the resulting improvement of oxygen utilization by the tissues was responsible for the reduction of hypoxia in the bronchial tree and lung tissue. CONCLUSION: The present study confirmed the high effectiveness of the combined application of normobaric interval hypoxic therapy and enteral oxygen therapy for the rehabilitation of the patients presenting with chronic obstructive pulmonary disease.

UNLABELLED: At present, there is no uniform approach to the treatment of subclinical hypothyroidism (SH). The objective of the present study was to estimate the influence of normobaric hypoxytherapy on the thyroid status and immunological characteristics of the patients presenting with subclinical hypothyroidism. MATERIAL AND METHODS: The study involved 140 patients at the age varying from 18 to 60 years suffering from SH associated with autoimmune thyroiditis: 75 of them underwent a course of hypoxytherapy, the remaining 65 patients comprised the control group. RESULTS: The changes in the immune status of the patients with SH were apparent as the depression of cellular immunity and activation of humoral immunity which confirmed the autoimmune nature of the disease. Hypoxytherapy caused positive changes in the immune status of the patients in the form of increased number of CD3+ and CD(8+)-cells, reduction of the number of CD(4+)-cells, and the normalization of the immunoregulatory index. The evaluation of the parameters of humoral immunity has demonstrated normalization of the total B-lymphocyte content along with the level of serum IgA, IgM, IgG, reduction of circulating immune complexes and anti-thyroid antibodies. These changes promoted the recovery of the thyroid function that manifested itself as the normalization of the blood thyroid-stimulating hormone level. CONCLUSION: It is concluded that hypoxytherapy can be recommended as monotherapy for the treatment of subclinical hypothyroidism as a tool for the influence on the key components of pathogenesis of this disease and prevention of the manifestation of its clinically expressed form.

RESUMO

The search for non-medicamentous therapeutic modalities for the treatment of patients with chronic obstructive pulmonary disease is a challenging problem in pulmonology. Properly adjusted adaptation to hypoxia in the course of normobaric interval hypoxic training (IHT) can be successfully applied to the prevention and the treatment of chronic obstructive pulmonary disease because it produces neither side effects nor complications. Better understanding of pathogenic mechanisms of action of interval hypoxic training will allow to more extensively exploit this method for the treatment of patients with chronic obstructive pulmonary disease and prevent the most undesirable complications of hypoxic therapy by choosing its most optimal variants.

RESUMO

The combined examination and treatment in the course of normobaric interval hypoxic training of 550 patients with chronic obstructive pulmonary disease was performed. Characteristics of change of state of functional respiratory system, immunological reactivity, electroencephalography after adaptation to hypoxia in the course of interval hypoxic training were determined. Revealed improvement of oxygen supply on all stages of its transfer and increase of oxygen consumption in tissues resulted in normalization of electrobiological activity of cerebral cortex, correction of immunological reactivity. High efficiency of interval hypoxic training in the combined treatment for patients with chronic obstructive pulmonary disease was proved.
