In diabetics, vascular examination may consider great and medium arteries and microcirculation. The aim of this study is correlation between stenosis in carotid and femoral bifurcation, ultrasound biopsy, and laser-Doppler measurements of distal microcirculation in diabetics. During a six month period, we have examined 90 diabetics in the diabetological service of the Civita Castellana Hospital (Viterbo, Italy), asymptomatic for any vascular distal, cerebral or coronary disease. Subjects were 49 +/- 20 years old with 15 +/- 5 years of diabetes; glycosilate haemoglobin was 7% (mean). 70% of diabetics (62% type I, 38% type II) had an altered veno-arteriolar reflex and 30% of these an ultrasound biopsy value of more than 25. These data demonstrate that most diabetic subjects develop an early, preclinical neuropathy that is directly proportional to the arterial wall condition.

The discovery of America still goes on through the analysis of its medical matter. From the exploitation of the resources made by Columbus, to the comprehension of local civilizations and to the reception of their therapeutic means, is a path that winds, supported by different reasons: the economic return, the therapeutic efficacy, the cultural exchanges between peoples. Unfortunately, with the passing of time, many traditions, fruit of the experience of mysterious centuries, are lost. It is through the examination of American medical matter that we try - with the advantage of acquiring new curative means - to find a new key for the comprehension of its peoples.

Hemodynamic variables were used to evaluate the effectiveness of lumbar chemical sympathectomy in 20 patients with ischemic foot lesions. Early clinical improvement was obtained in 68% of the cases. The two-year cumulative limb salvage rate was 52%. Results indicate that lumbar chemical sympathectomy offers the same benefits as surgical sympathectomy and represents a useful alternative in patients with advanced ischemic disease of the lower limb, decreasing the rate and the level of amputations.

Hemorrhagic shock was experimentally induced in 40 mongrel dogs, and the resulting pulmonary vascular resistances studied. A mechanical test with closure of the left branch of the pulmonary artery and two pharmacological tests with isoproterenol (isuprel) and cyclic adenosine-monophosphate (cAMP) were performed. The results suggest that there is a block in the beta-control during prolonged shock, and that beta-control is restored by cAMP.

Mebendazole--an anthelmintic drug which is known to have a lethal effect on hydatid cysts--was administered preoperatively to patients affected by Echinococciasis, in order to sterilize cysts and prevent surgical contamination. Our results suggest that mebendazole treatment (1500 mg/die for 25 days) was effective in ten out of eleven cases; no major toxic effect was observed.
