RESUMO

@#Objective To compare the safety and efficacy of the da Vinci robot and thoracoscopic subxiphoid approach for the treatment of anterior mediastinal tumors. Methods The clinical data of patients who underwent anterior mediastinal tumor resection through the subxiphoid approach admitted to the same medical group in the Department of Thoracic Surgery of the First Hospital of Lanzhou University between June 2020 and April 2022 were retrospectively analyzed. According to the surgery approach, the patients were divided into a robot-assisted thoracoscopic surgery (RATS) group and a video-assisted thoracoscopic surgery (VATS) group. The perioperative data and the incidence of postoperative complications were compared between the two groups. Results A total of 79 patients were enrolled. There were 41 patients in the RATS group, including 13 males and 28 females, with an average age of 45.61±14.99 years. There were 38 patients in the VATS group, including 14 males and 24 females, with an average age of 47.84±15.05 years. All patients completed the surgery successfully. Hospitalization cost and operative time were higher or longer in the RATS group than those in the VATS group, and the difference was statistically significant (P<0.05). Intraoperative bleeding, postoperative hospital stay, postoperative water and food intake time, postoperative off-bed activity time, white blood cell count, neutrophil percentage and visual analogue scale (VAS) score on the first postoperative day, white blood cell count and neutrophil percentage on the third postoperative day, duration of analgesic pump use, the number of voluntary compressions of the analgesic pump, and mediastinal drainage volume were all superior to those in the VATS group (P<0.05). The differences in VAS scores on the third postoperative day, duration of drainage tube retention and postoperative complication rates were not statistically different between the two groups (P>0.05). Conclusion RATS subxiphoid anterior mediastinum tumor resection is a safe and feasible surgical method with less injury and higher safety, which is conducive to rapid postoperative recovery and has wide clinical application prospects.

RESUMO

Objective To understand the etiological characteristics of children’s respiratory viruses in Hai’an City. Methods The clinical data of 6 810 children with acute respiratory infection in our hospital from January 2018 to May 2020 was retrospectively analyzed. The antigens of seven common respiratory viruses were detected by direct immunofluorescence assay. Results (1) Of the 6 810 specimens tested, 957 were positive, with a total positive rate of 14.05%. Among them, the number of RSV (respiratory syncytial virus) positive cases was the largest, with a total of 524 cases, followed by ADV (adenovirus), PIVⅢ (parainfluenza virus Ⅲ), PIVⅠ (parainfluenza virus Ⅰ), IFA (influenza A), IFB (influenza B), and PIVⅡ (parainfluenza virus Ⅱ), accounting for 154 cases (16.09%), 80 cases (8.36%), 65 cases (6.69%), 58 cases (6.60%), 55 cases (5.75%), and 21 cases (2.19%), respectively. (2) There was no statistically significant difference in respiratory virus infection between different genders (χ2=3.218, P>0.05). (3) The positive rate of antigen detection in children aged 11 days to 1 year old (19.24%) was the highest, with the viral infection rate decreasing with age. (4) The positive rates of viruses were different in different diseases, of which the highest was upper respiratory tract infection (27.53%), followed by pneumonia (16.90%), and bronchiolitis (7.19%), and the lowest was asthmatic bronchitis (6.18%). RSV was the main virus in each disease. Conclusion RSV is the main virus of respiratory tract infection in children in Hai’an City. The positive rate of viral infection in children aged 11 days to 1 year old is the highest, mainly with upper respiratory tract infection.

Aquatic and terrestrial animals have different swimming performances and mechanical efficiencies based on their different swimming methods. To explore propulsion in swimming frogs, this study calculated mechanical efficiencies based on data describing aquatic and terrestrial webbed-foot shapes and swimming patterns. First, a simplified frog model and dynamic equation were established, and hydrodynamic forces on the foot were computed according to computational fluid dynamic calculations. Then, a two-link mechanism was used to stand in for the diverse and complicated hind legs found in different frog species, in order to simplify the input work calculation. Joint torques were derived based on the virtual work principle to compute the efficiency of foot propulsion. Finally, two feet and swimming patterns were combined to compute propulsive efficiency. The aquatic frog demonstrated a propulsive efficiency (43.11%) between those of drag-based and lift-based propulsions, while the terrestrial frog efficiency (29.58%) fell within the range of drag-based propulsion. The results illustrate the main factor of swimming patterns for swimming performance and efficiency.
