This is believed to be the first reported case of carotid artery thrombosis after elective maxillary or mandibular osteotomy. The most likely cause was a sudden, sharp blow to the right carotid artery region either when the mandibular split was completed or when the maxillary tuberosity was separated. The blow was associated with neck hyperextension and some lateral flexion. In a patient with tight, underdeveloped soft tissue, the resultant intimal tear caused thrombosis of the artery. Although this event is associated with a 40% mortality and further 52% serious morbidity, our patient has regained nearly full neurological function--probably thanks to a competent circle of Willis that allowed adequate collateral flow to the brain.

A comparison study of donor site care using 5 common modalities was undertaken. A total of 210 treated donor areas were examined for healing differences, comfort, complications, and cost. At the conclusion of the study we were able to recommend certain treatment methods depending on the nature of the donor wound.

Our preliminary experience with tattoo removal utilizing the carbon dioxide laser at the Vancouver General Hospital is reviewed. We developed the method by trial and error until we produced a system yielding an acceptable procedure time with good results.

The authors conducted a 20-year review of the records of patients with torsion of greater omentum or appendices epiploicae admitted to the Vancouver General Hospital. Presenting signs and symptoms, operative findings and some recent reports are examined and conclusions are given. Diagnosis of the condition is rarely made preoperatively owing to the lack of specific symptoms. When a straightforward diagnosis of appendicitis has been made, yet a normal appendix is found, torsion of omentum should be considered and excluded.
