[Color-coded duplex ultrasound and contrast medium enhanced duplex ultrasound in deep venous thrombosis]. / Farbkodierte Duplexsonographie und kontrastmittelverstärkte Duplexsonographie bei tiefer Beinvenenthrombose.

Even with highly developed ultrasound systems, diagnosis of deep leg vein thrombosis is difficult in lower leg veins and in the area of the adductor channel. In order to evaluate color duplex-sonography, contrast media duplex-sonography and phlebography in comparison, 79 patients (38 men and 40 women aged 54 to 89) with deep leg vein thrombosis were examined. As a result, patients suffering from thrombosis were characterized by a vein-artery quotient of > 2.0 (p < 0.01). In color Doppler duplex-sonography false negative findings occurred in 12 cases of lower leg vein thrombosis and in 3 cases of thrombosis within the adductor channel. By contrast media duplex-sonography the diagnostic reliability could be increased to a sensitivity of over 95 per cent.

[Value of color-coded duplex sonography in diagnosis of extracranial vascular changes]. / Wertigkeit der farbkodierten Duplex-Sonographie bei der Diagnostik extrakranieller Gefässveränderungen.

72 patients at the age of 44 to 76 years with transitory ischaemic attacks which happened and for a short time reversible deficits, respectively, were diagnosed both conventionally duplex-sonographically and with the colour-coded duplex sonography under the question of vascular stenotic process in the region of the cervical vessels. After the ultrasound examination within two weeks an angiography was performed. As a result was shown that in 10 patients with an angiogram without pathological findings by means of the colour-coded duplex sonography in 3 cases plaque formations could be proved which could be clearly diagnosed only by colour marking.

[Experiences with the Günther filter]. / Erfahrungen mit dem Güntherfilter.

In the investigation 17 patients with lung embolism being cared for with a Guenther-filter were post-observed during a period up to two years. Early complications after the implantation of the Guenther-filter did not occur in any case. Late complications after the implantation of the Guenther-filter were dislocations, deformations of the filter and breaks of the spokes. In the result of the post-observation the Guenther-filter application despite its easy handling should be regarded very critically.

72 Patients with occurred transitory ischemie attacks and passed reversible neurologie deficits were duplexsonographically diagnosed. After wards a renewed duplexsonographic investigation with color coding followed in all patients. The patients were angiographically diagnosed within two weeks. As a result in three of ten patients with an unobtrusive angiogramm plaques formations were proved by means of the color codes duplexsonography.

[Doppler sonography in the diagnosis of extracranial vascular changes]. / Doppler-Sonographie zur Diagnostik extrakranieller Gefässveränderungen.

On account of cerebrovascular insufficiencies or asymptomatic vascular noises which had taken place 45 patients were examined by means of Doppler sonography and after this by means of angiography. The judgement of the common carotid artery, the internal and external carotid arteries, the vertebral artery, the subclavian artery and the supratrochlear artery were involved into the ultrasound investigation. The sensitivity of the supratrochlear compression test was 50% in the diagnosis of high-degree stenoses and occlusions of the internal carotid artery, whereas the sensitivity of the complete ultrasound investigation way about 80%. The advantages and disadvantages of the ultrasound Doppler sonography investigation are discussed.

[Therapeutic transfemoral testicular vein occlusion of varicocele]. / Zur therapeutischen transfemoralen Testikularisokklusion der Varikozele.

It is reported on the percutaneous transfemoral obliteration of the testicular vein with ethoxysclerol. This method was used in 75 patients for the treatment of the varicocele. 81.3% could successfully be treated by this means. The applicability of the method is restricted by anatomical particularities in the course of the testicular vein. Advantages and disadvantages of the method are discussed.

[Varicocele and fertility--spermiographic and histologic findings]. / Varikozele und Fertilität--spermiografische und histologische Befunde.

The results of the treatment of 251 patients with varicocele are demonstrated. By means of spermiographic and histologic investigations the fertility-disturbing influence of this vascular disease of the testis shall be proved and the effect of the operative sanation shall be estimated. For many years the high ligature of the testicular vein has been favoured method of operation; since 1984 exclusively the percutaneous transfemoral occlusion of the testicular vein has been performed, when a left-sided varicocele is present. In 61% of the patients examined an improvement of the spermiogram is to be proved, in which case the most conspicuous rate of increase lies at the motility of spermatozoa with 58%. After removal if the varicocele the investigations of the testicular tissue reveal in 42% of the cases an improvement of the findings by decrease of the tubular atrophy and increase of the tubular cell number. Among the number of our patients the rate of conception is 49%. The treatment of varicocele is a fertility-protective measure and therefore should be performed in all adolescents and males at fertile age.
