[Immune and fibrinolytic system functions in the cardiac manifestations of atherosclerosis]. / Sostoianie immunnoi i fibrinoliticheskoi sistem pri kardial'nykh proiavleniiakh ateroskleroza.

A total of 77 patients with various clinical forms of cardiac atherosclerosis were examined. Fibrinolytic and immune systems were studied together with blood plasma concentration of fibronectine. Disorders of the same type have been revealed of immunologic reactivity manifested by enhancement of humoral immunity and impairment of cellular immunity, increase in coagulative activity, decline in the activity of the system of fibrinolysis. Latent DBS syndrome recorded in the above patients associated with decline in the activity of monocytic-macrophagal link of immunity and low fibrinolytic potential is a pathogenetic prerequisite of progression of atherosclerosis indicating main trends in drug therapy.

A total of 168 patients were examined presenting with different clinical and roentgenological forms of acute pneumonia, such as focal one with an acute course, focal protracted, croupous with an acute course, croupous protracted, abscessing ones. Blood serum elastinase-like activity was studied. The complicated forms of pneumonia were found to be accompanied by two-phase dynamics of elastinase-like activity, such that it soars by day 10-15 of the disease process and declines by day 20-15, unlike the prognostically favourable forms of the condition where the initial reduction of this parameter turns to its gradual normalization.

[Fibronectin and the phagocytic activity of the neutrophilic granulocytes in patients with different clinical variants of bronchial asthma]. / Fibronektin i fagotsitarnaia aktivnost' neitrofil'nykh granulotsitov u bol'nykh s razlichnymi klinicheskimi variantami bronkhial'noi astmy.

A study of 125 cases of bronchial asthma revealed low contents of fibronectin. The degree of the decrease depended on the severity of the clinical course of the disease and treatment with glucocorticoid hormones. Patients with bronchial asthma showed an essential reduction of the number of neutrophilic granulocytes capable of phagocytosis. In the process of incubation of polynuclears with fibronectin occurs restoration of their phagocytic activity which was less pronounced in patients receiving glucocorticoids.

A study of the immune and fibrinolytic systems of the body in 216 patients with different clinical forms of atherosclerosis (exertion and rest stenocardia, arrhythmias, atherosclerotic hypertension), chronic obstructive bronchitis revealed that the antiatherogenous effect of chronic obstructive bronchitis is predetermined on the one side by an increased function of the monocytic-macrophagal link of immunity and on the other by activation of the fibrinolysis system.

Women suffering of infectious-dependent bronchial asthma show at the second half of pregnancy a sharp reduction of the fibrinolytic potential that is manifested in a decrease of plasminogen activator and urokinase activity of the urine, in an elevation of the antiactivator and antiplasmin activity, growth of the concentration of alpha2-macroglobulins. At the same time high levels of fibrinogen are noted and low levels of fibrin degradation.

The study of the fibrinolytic system and the activity of interleukin 1 and 2 in 75 patients with various cardial manifestations of atherosclerosis (angina of effort, angina decubitus, arrhythmia, symptomatic hypertension) revealed a decrease in the activity of plasminogen--a blood activator--in patients with angina of effort, angina decubitus and cardiac arrhythmias. In those with atherosclerotic hypertension the activatory activity was in the normal limits. A decrease in the activity of interleukin 1 and 2 was noted in all those examined.

Reduction of the content of fibronectin in blood plasma depending on severity and form of the disease was revealed in patients with infection-dependent bronchial asthma. Discreteness of influence of autologous leucocytes upon the concentration of fibronectin in plasma, caused by the factor of hormone dependence of the disease was discovered. It was determined that tactivin recovers capability of leucocytes in the patients with hormone dependent asthma directly (monocytes) or indirectly (Lymphocytes, neutrophils) to produce fibronectin.

The comparison of activator activity of blood, urine and pleural fluid in 55 patients suffering from chronic circulatory insufficiency, tuberculosis, pneumonia and lung cancer helped to determine pathognomonic signs for each of the exudates examined. Cardiac decompensation is associated with high activity of plasminogen activator in the transudate against low activator activity of the blood and urine. In tuberculous pleuritis there is low activator activity of the exudate in normal blood and urine parameters. Reduced activator activity of the blood, urine and pleural fluid is characteristic of parapneumonic pleuritis, while high activity of plasminogen activator in the blood and pleural exudate in its normal activity in the urine is seen in cancer pleuritis. The findings obtained can be used in clinical medicine for verification of pleural fluid nature.

Fibrinolytic system, immune reactivity and isoelectric focusing of serum albumin were examined in 94 patients exhibiting combination of obstructive lung disease (chronic obstructive bronchitis and bronchial asthma) with atherosclerosis. Plasminogen activator showed discrete activity, the discreteness being less in respiratory distress of the I degree but higher in the distress of the II and III degree. Relative number of E-RFC and monocytes expressing receptors to IgM and IgG Fc-fragment decreased. Percentage of EAC-RFC rose. Serum albumin fractions changed pH range due to modification of albumin molecules resultant from forming complexes with fibrinogen degradation products. Concentration of the latter under conditions of respiratory distress induced by obstructive lung diseases associated with atherosclerosis substantially exceeded the standard levels.

Urokinase receptors on lymphocytes described in the paper link immune system of the body with fibrinolytic one, play a biological role in sano-++- and pathogenesis in aggravation of chronic purulent bronchitis, of focal and croupous pneumonia. A correlation between the trend in the expression of urokinase receptors on lymphocytes and changes in activity of plasminogen activator in the blood and urine was established: the more the expression, the more the activity. The method can serve a criterion of the treatment effectiveness+ in nonspecific inflammatory bronchial and pulmonary diseases and be helpful in designing methods of their treatment improvement.

The state of cellular and humoral immunity, systemic and regional fibrinolysis was studied in 47 patients with chronic purulent bronchitis of a moderately severe and severe course. The suppression of cellular immunity, expressed in the reduction of the number and function of T-cells, the reduction of the number of theophylline-resistant cells with the unchangeable level of theophylline-sensitive lymphocytes was revealed. An additional study of O-cells made it possible to reveal an increase in the number of precursor and blocked T-cells, the prevalence of the fraction of suppressor-killer lymphocytes in T-lymphocyte subpopulation. The detected shifts lead to disturbance of T-B-cell cooperation, antibody genesis, and breakage of fibrinolysis immune regulation which determine progression of inflammatory changes in the bronchopulmonary system. Their detection and elimination form the basis of modern pathogenetic therapy of purulent forms of chronic bronchitis.
