The chemo-morphological properties of soils on ancient landforms (quarzitic pliocene alluvial fans, pleistocene terraces), namely "rañizos", on middle-high mountains (Eastern-Central System, Iberian Peninsula, Rio Negro, Cogolludo) were investigated. Several properties were analyzed by standard procedures. A detailed soil diagnostics and classification on "rañizos" were done, unlike those widely studied on "rañas" (similar landform), by identifying parallel pedogenetic processes in both formations. The genetic and geographic features of Alfisols, Ultisols and Inceptisols are closely related to the nature of their parent materials, based on quartzite and quartz conglomerates, sometimes with an arkosic matrix, red shale, polygenic gravels and pebbles. Soil features were determined by genuine soil-forming inherent lithological rock properties. Other driving factors were flat topography and enough soil formation time to allow intense pedogenesis. The main soil-forming processes were intense weathering, clay enrichment horizons with illuviation, red color caused by iron oxide dehydration and signs of pseudogleyzation processes. Such pedological formations can be considered endemisms; that is, "rare" soils and, up to a point, "relict". The soil reaction is acid/slightly acid with low base saturation. Despite lying on mountains, soils are characterized by moderate-low organic matter content. Soil conditions and climate provide good vine production requirements despite acidity levels. A common feature of all Rio Negro soils is the presence of gravel (size up to 3-5 cm), which is evenly distributed on arable layers. The results can be used to assess vineyard soil use in a potential Pago (Protected Denomination of Origin) and to extend the database of vineyard soils from poorly studied Mediterranean continental mountain regions.

PURPOSE: To evaluate Red blood cell distribution width (RDW) as a sepsis prognostic biomarker. METHODS: 203 septic patients admitted to the ICU. Analysis of RDW dynamics, hospital mortality discrimination ability and the added value when incorporated to the SOFA, LODS, SAPS-II and APACHE-II scores using the AUC-ROC. RESULTS: Non-survivors presented higher RDW values during the first week after ICU admission (p = 0.048). Only SOFA and RDW were independently associated with mortality when adjusted by Charlson, immunosuppression, nosocomial infection, NEWS2, SAPS-II, septic shock and haemoglobin (p < 0.05). After adjustment, AUC-ROC was 0.827, 0.822, 0.824, 0.834 and 0.812 for each model including admission, 24, 48 and 72-h and 7-days RDW, respectively. When added to the scores, 24-h RDW and admission RDW improved their discrimination ability (SOFA AUC-ROC = 0.772 vs 0.812 SOFA + admission RDW, p = 0.041; LODS AUC-ROC = 0.687 vs 0.710, p = 0.002; SAPS-II AUC-ROC = 0.734 vs 0.785, p = 0.021; APACHE-II AUC-ROC = 0.672 vs 0.755, p = 0.003). Admission RDW with SOFA presented the better discrimination ability for mortality. CONCLUSION: RDW is an independent prognostic marker of death in septic patients admitted in the ICU that improves SOFA, LODS, APACHE-II and SAPS-II discrimination ability. This parameter could be incorporated to the prognostic scores as a marker of systemic dysfunction and dysregulated inflammatory response.

Introduction: Cytomegalovirus (CMV) infection is a well-known factor associated with invasive aspergillosis in immunocompromised hosts. However, its association with COVID-19-associated pulmonary aspergillosis (CAPA) has not been described. We aimed to examine the possible link between CMV replication and CAPA occurrence. Methods: A single-center, retrospective case-control study was conducted. A case was defined as a patient diagnosed with CAPA according to 2020 ECMM/ISHAM consensus criteria. Two controls were selected for each case among critically ill COVID-19 patients. Results: In total, 24 CAPA cases were included, comprising 14 possible CAPA and 10 probable CAPA. Additionally, 48 matched controls were selected. CMV replication was detected more frequently in CAPA than in controls (75.0% vs. 35.4%, p = 0.002). Probable CMV end-organ disease was more prevalent in CAPA (20.8% vs. 4.2%, p = 0.037). After adjusting for possible confounding factors, CMV replication persisted strongly associated with CAPA (OR 8.28 95% CI 1.90-36.13, p = 0.005). Among 11 CAPA cases with CMV PCR available prior to CAPA, in 9 (81.8%) cases, CMV replication was observed prior to CAPA diagnosis. Conclusions: Among critically ill COVID-19 patients, CMV replication was associated with CAPA and could potentially be considered a harbinger of CAPA. Further studies are needed to confirm this association.

BACKGROUND: COVID-19-associated pulmonary aspergillosis (CAPA) is a major complication of critically ill COVID-19 patients, with a high mortality rate and potentially preventable. Thus, identifying patients at high risk of CAPA would be of great interest. We intended to develop a clinical prediction score capable of stratifying patients according to the risk for CAPA at ICU admission. METHODS: Single centre retrospective case-control study. A case was defined as a patient diagnosed with CAPA according to 2020 ECMM/ISHAM consensus criteria. 2 controls were selected for each case among critically ill COVID-19 patients. RESULTS: 28 CAPA patients and 56-matched controls were included. Factors associated with CAPA included old age (68 years vs. 62, p = .033), active smoking (17.9% vs. 1.8%, p = .014), chronic respiratory diseases (48.1% vs. 26.3%, p = .043), chronic renal failure (25.0% vs. 3.6%, p = .005), chronic corticosteroid treatment (28.6% vs. 1.8%, p < .001), tocilizumab therapy (92.9% vs. 66.1%, p = .008) and high APACHE II at ICU admission (median 13 vs. 10 points, p = .026). A score was created including these variables, which showed an area under the receiver operator curve of 0.854 (95% CI 0.77-0.92). A punctuation below 6 had a negative predictive value of 99.6%. A punctuation of 10 or higher had a positive predictive value of 27.9%. CONCLUSION: We present a clinical prediction score that allowed to stratify critically ill COVID-19 patients according to the risk for developing CAPA. This CAPA score would allow to target preventive measures. Further evaluation of the score, as well as the utility of these targeted preventive measures, is needed.

The mineralogical and geochemical patterns of calcareous vineyard soils located in Alcubillas (La Mancha, Central Spain) have been evaluated; also their variability has been studied. The information provided by this study supports the assessment of geochemical spatial variability, the origin of these soils, their elements and the factors that control their distribution. The presence of quartz, calcite, feldspar and, in particular, illite and kaolinite is due to their inheritance from surrounding lithologies (and pedological processes), which mostly include limestones, marls and other sedimentary rocks, as well as metasedimentary rocks of Hercynian origin. Furthermore, since the presence and accumulation of certain trace elements in vineyard soils is a relevant global hazard (in particular with respect to wine production quality), the spatial distributions of Ba, Cr, Cu, Pb, Rb, Sr, V and Zr (carried out using geostatistical techniques and geometry-based interpolation methods) were investigated in order to determine the origin of these trace elements. The presence of these elements can be interpreted as being due to geogenic, pedogenic and, in certain cases, anthropic influences. The nature of certain agricultural practices, including the use of fertilizers, phytosanitary products and machinery, could explain the local increases in some trace element contents.

Resumen Introducción: Durante el alta hospitalaria es importante que el enfermero identifique los cuidados que requiere el paciente, permitiéndole guiar intervenciones mediante el plan de alta que fomenta el autocuidado, enfocado a necesidades específicas de cada persona, para prevenir complicaciones y disminuir reingresos. Objetivo: Evaluar la ejecución del plan de alta por los profesionales de enfermería en los servicios de medicina interna y quirúrgicas del Hospital San José, Popayán durante el año 2017. Materiales y métodos : Estudio de tipo cuantitativo, diseño descriptivo y transversal. La población conformada por los enfermeros de los servicios de medicina interna y quirúrgicas del Hospital San José y pacientes con alta programada de 24 horas o menos antes del egreso hospitalario. Resultados: La información se recolectó por medio de un instrumento de valoración realizado a participantes que cumplen con criterios de inclusión. El 94,7% de los pacientes tenían egreso programado, de estos 58,6% recibieron indicaciones en las últimas 48 horas sobre los cuidados en casa y el 100% manifestaron entender las recomendaciones brindadas por el enfermero. Respecto al profesional de enfermería el 100% refiere no conocer alguna metodología para elaborar el plan de alta y el 80% no conoce si en la institución existe un procedimiento de egreso y seguimiento al paciente. Conclusiones: El plan de alta es un proceso que no se encuentra estandarizado y por lo tanto no es evaluado en la institución, el personal de Enfermería se limita a brindar educación sobre las indicaciones médicas de egreso y no sobre un plan de alta propio de Enfermería.


Abstract: Introduction: When the patients are going to be discharged from the hospital, the nurses must execute the discharge plan based on their specific needs. It is essential to promote self-care in patients to prevent future medical complications and readmissions. Objective: To evaluate the implementation of the discharge plan by the nursing professionals in the internal medicine and surgical services at the San José hospital in Popayán, Colombia, in 2017. Materials and methods: This quantitative study has a descriptive and transversal design. The research population is composed of nurses from the internal medicine and surgical services from the San José Hospital and patients that are going to be discharged from the clinic in 24 hours or less. Results: The information was collected by using an instrument that was designed for the participants to confirm if they fit the inclusion criteria. According to collected data, 94.7% of the patients had scheduled discharges, 58.6% of them had received indications in the last 48 hours before the discharge on how the self-care routines must be followed at home. Additionally, 100% of people said they understood the recommendations given by the nurses. Finally, 100% of the nursing professionals claimed that they did not know any methodology to prepare the discharge plan, and about 80% of them did not know about the existence of a patient's exit procedure and patient's medical follow-up in the institution. Conclusions: The patient discharge plan is not standardized, so it is not evaluated in the institution. The nursing staff is focused only on explaining the medical indications, but they are not focused on creating their discharge plans.

Castilla-La Mancha (central Spain) is a region characterized by significant agricultural production aimed at high-quality food products such as wine and olive oil. The quality of agricultural products depends directly on the soil quality. Soil geochemistry, including dispersion maps and the recognition of baselines and anomalies of various origins, is the most important tool to assess soil quality. With this objective, 200 soil samples were taken from agricultural areas distributed among the different geological domains present in the region. Analysis of these samples included evaluation of edaphological parameters (reactivity, electrical conductivity, organic matter content) and the geochemistry of major and trace elements by X-ray fluorescence. The dataset obtained was statistically analyzed for major elements and, in the case of trace elements, was normalized with respect to Al and analyzed using the relative cumulative frequency (RCF) distribution method. Furthermore, the geographic distribution of analytical data was characterized and analyzed using the kriging technique, with a correspondence found between major and trace elements in the different geologic domains of the region as well as with the most important mining areas. The results show an influence of the clay fraction present in the soil, which acts as a repository for trace elements. On the basis of the results, of the possible elements related with clay that could be used for normalization, Al was selected as the most suitable, followed by Fe, Mn, and Ti. Reference values estimated using this methodology were lower than those estimated in previous studies.

BACKGROUND: Control of the sea louse Caligus rogercresseyi in the Chilean salmonid industry is reliant on chemical treatments. Azamethiphos was introduced in 2013, although other organophosphates were previously used. In 2014, reduced sensitivity to azamethiphos was detected in the Los Lagos Region using bioassays. The main target of organophosphates is the enzyme acetylcholinesterase (AChE). Mutations in the AChE gene are the main cause of organophosphate resistance in arthropods, including other sea lice. In the present study, we aimed to characterize C. rogercresseyi AChE(s) gene(s) and to study the association between AChE variants and azamethiphos resistance in this sea louse species. METHODS: Samples of adult male and female C. rogercresseyi were collected in the Los Lagos Region in 2014. Twenty-four hour exposure bioassays with azamethiphos were performed to select sensitive and resistant lice. The full-length cDNA coding sequences encoding for two AChEs in C. rogercresseyi were molecularly characterized. One of the AChE genes was screened by direct sequencing in the azamethiphos-selected lice to search for variants. An additional louse sampling was performed before and after an azamethiphos treatment in the field in 2017 to validate the findings. RESULTS: The molecular analysis revealed two putative AChEs in C. rogercresseyi. In silico analysis and 3D modelling of the protein sequences identified both of them as invertebrate AChE type 1; they were named C. rogercresseyi AChE1a and 1b. AChE1a had the characteristics of the main synaptic AChE, while AChE1b lacked some of the important amino acids of a typical AChE. A missense change found in the main synaptic AChE (1a), F318F/V (F290 in Torpedo californica), was associated with survival of C. rogercresseyi at high azamethiphos concentrations (bioassays and field treatment). The amino acid change was located in the acyl pocket of the active-site gorge of the protein. CONCLUSIONS: The present study demonstrates the presence of two types of AChE1 genes in C. rogercresseyi. Although enzymatic assays are needed, AChE1a is most probably the main synaptic AChE. The function of AChE1b is unknown, but evidence points to a scavenger role. The AChE1a F/V318 variant is most probably involved in organophosphate resistance, and can be a good marker for resistance monitoring.

ResumenProsopis ruscifolia es una especie arbórea pionera en áreas inundadas o salinas. El objetivo de este trabajo fue determinar cambios anatómicos en raíces e hipocótilos de plántulas de P. ruscifolia sometidas a estrés salino, bajo condiciones controladas. Las semillas se recolectaron en bosques nativos de la Región Chaqueña Occidental de Argentina. Las semillas se sembraron sobre toallas de papel humedecidas con soluciones salinas de 100, 200 y 300 mM de NaCl y un control humedecido con agua destilada. Se sembraron cuatro repeticiones de 50 semillas cada una, correspondientes a cada tratamiento, se ubicaron en cajas plásticas herméticas dentro de cámara de siembra a 27 ºC y con fotoperíodo de 12 horas. Doce días después de la siembra, se extrajeron plántulas para estudios anatómicos. Se estudiaron 35 plántulas correspondientes a cada tratamiento. Se midieron en raíces e hipocótilos las siguientes variables anatómicas: diámetro de la raíz principal e hipocótilo (µm), espesor de la corteza (µm), número de estratos celulares en la corteza, diámetro del cilindro central (µm), diámetro de la médula (µm), número de estratos celulares en el periciclo y diámetro tangencial de los vasos (µm). Se realizó ANOVA con diámetro de la raíz o hipocótilo como variable dependiente y espesor de la corteza, número de estratos celulares en la corteza, diámetro del cilindro central, diámetro de la médula, número de estratos celulares en el periciclo, diámetro tangencial de los vasos y concentración salina como variables independientes. El diámetro de la raíz disminuyó significativamente con el aumento de la concentración salina (P < 0.0001). El espesor de la corteza redujo su espesor a 100 mM (P < 0.0001) e incrementó el número de estratos celulares que la componen (P < 0.0002). El diámetro del cilindro central se redujo a la concentración salina de 100 mM (P < 0.0001) y el diámetro de la médula y el número de estratos celulares del periciclo (P < 0.0003) disminuyó progresivamente hasta 300 mM. El diámetro tangencial de los vasos (P < 0.0001) se redujo recién a 300 mM de NaCl. Estos cambios anatómicos podrían estar relacionados con la alteración de la expansión y división celular causada por la salinidad y comprometer la formación de raíces laterales y el almacenamiento de reservas. Los hipocótilos no mostraron cambios anatómicos significativos en respuesta al incremento en la salinidad, con excepción de la variación en la posición de estomas y un incremento en el espesor de la hipodermis. Estos cambios parecen indicar el estrés hídrico impuesto por el bajo potencial osmótico causado por las sales. Las plántulas de P. ruscifolia experimentaron cambios anatómicos en respuesta a las concentraciones salinas analizadas, en rasgos vinculados al almacenamiento de reservas, a la absorción y la conducción de agua y la formación de raíces laterales.


Abstract:Prosopis ruscifolia is a pioneer tree species in flooding or saline areas. The aim of this work was to assess anatomical changes in roots and hypocotyls of P. ruscifolia seedlings induced to saline stress under controlled conditions. Seeds, collected in natural forests of Western Chaco region in Argentina, were sown on paper towels moisturized with saline solutions of 100, 200 and 300 mM of NaCl, and a control group with distilled water. Four repetitions of 50 seeds per treatment were sown, located in hermetic polystyrene boxes, and included in a seeding chamber, at 27 ºC and 12 hours photoperiod. Were studied 35 seedlings from each saline concentration; these seedlings were processed 12 days after sown to obtain microscopic samples. The anatomical variables measured in roots and hypocotyls were the following: main root diameter (µm), bark thickness (µm), number of cell strata in bark, central cylinder diameter (µm), pith diameter (µm), number of cell strata in the pericycle and the tangential diameter of vessels (µm). ANOVA analysis were performed with hypocotyl and root diameters as the dependent variable, and bark thickness (µm), number of cell strata in the bark, the central cylinder diameter (µm), the pith diameter (µm), number of cell strata in the pericycle, the tangential diameter of vessels and the saline concentration as independent variables. Results showed that the root diameter decreased with increasing saline concentrations (P < 0.0001). The bark thickness decreased at 100 mM (P < 0.0001) and the number of cell strata of bark increased to 300 mM (P < 0.0002). The central cylinder diameter decreased at 100 mM saline concentration (P < 0.0001) and the number of cell strata of the pericycle and the pith diameter reduced progressively until 300 mM. The tangential diameter of vessels decreased at 300 mM. These anatomical changes suggested alterations in the expansion and cell division caused by the salinity, and could limit lateral roots formation and reserves storage. Hypocotyls did not show significant anatomical changes in response to increasing salinity, with exception of stomata position and an increase of the hypodermis thickness. These changes indicated that the water stress imposed by low osmotic potential is caused by increasing saline concentration. The seedlings of P. ruscifolia experienced anatomical changes in response to tested saline concentrations in traits related to reserve storage, the absorption and conduction of water, and lateral roots formation. Objetivo: analizar los factores relacionados con la atención intercultural en salud de las gestantes del municipio de Toribio 2008-2009. Materiales y Métodos: tipo de estudio cualitativo, etnográfico. Participaron 19 agentes de salud de la Empresa Social del Estado (E.S.E) Cxayutc'e Jxut*, 24 promotores de salud, 10 gestantes Nasa y 6 Parteras y Thewala de la asociación de cabildos indígenas del norte del Cauca. Para la recolección de datos se utilizó una entrevista semiestructurada, encuesta etnográfica y grupo focal. Resultados: las diferencias culturales entre los sistemas médicos tradicionales y facultativos crean barreras alrededor del cuidado de la gestante, determinan la búsqueda de servicios de salud facultativos. Destaca la preservación de los cuidados culturales de la familia Nasa y el reconocimiento de la medicina facultativa. El estudio permitió dilucidar al personal de salud como actor pasivo y poco comprometido. Los agentes aspiran al reconocimiento de las prácticas de salud locales y su incorporación a los sistemas de salud, para fortalecer la atención intercultural e institucional como complemento. Conclusiones: 1. las barreras para la atención intercultural fluctúan entre la divergencia de conceptos, la poca credibilidad, el desconocimiento de las capacidades y limitaciones, experiencias negativas al intentar integrar los dos sistemas de salud. 2. La búsqueda del equilibrio y armonización del cuerpo acompañado del control prenatal, son actividades de salud de la nativa que gesta y sus prácticas de cuidado, se rigen por los arraigos culturales y la hegemonía familiar como un derecho. Introduction: interculturality is a proposal for dialogue, exchange and complementarity. Objective: Analyse the factors related with attention to intercultural health of the pregnant women of the municipality of Toribio 2008- 2009. Materials and methods: qualitative and ethnographic study. Involving 19 health agents E.S.E Cxayutc'e Jxut*, 24 health promoters, 10 pregnant women Nasa and 6 midwives and Thewala of the Association of indigenous cabildos of north of Cauca. For the collection of data using a semi-structured interview, ethnographic survey, and focus group was used for data collection. Results: cultural differences between the traditional and optional medical systems create barriers around the care of the pregnant woman; it determine the search for optional health services. Highlight the preservation of the care of cultural for the Nasa family and the recognition of optional medicine. The study allowed to elucidate the committed staff of health as a passive actor uncommitted. The agents aspire to recognize local health practices and its incorporation into the health systems, to strengthen attention to intercultural and institutional as complement. Conclusions: 1.Barriers to intercultural care range from the divergence of concepts, the little credibility, lack of knowledge of the capabilities and limitations, negative experiences when trying to integrate the two systems of health. 2.The search for balance and harmonization of the accompanied by prenatal body, are health activities of the native that gestated and care practices, are governed by the cultural locally and family hegemony as a right. RESUMO

Gravid females of Caligus rogercresseyi were collected from Atlantic salmon (Salmo salar) from a farm located at Chiloe Island (42 degrees 40'S73 degrees 15'W), Chile, to obtain information about the reproductive output of this parasite in vitro. The egg strings removed from the females were incubated under controlled conditions to obtain virgin adult females. One female which had mated only once produced eleven generations of eggs strings in a period of 74 days. The first egg strings of the females obtained in vitro were produced at 389 degree days ( degrees D) after egg incubation, while the next generations of eggs strings were produced with a periodicity between 4 and 6 days dependent on the water temperature. The average length of the egg string was 3.1mm and the mean number of eggs per string was 31. The values recorded in captivity for the egg string length and the number of eggs per string, were lower than the values recorded in gravid females from the field. One female survived for 79 days and males, maintained separately from the females, survived for 60 days.

Introducción: el cáncer de mama afecta los componentes físico, emocional, conductual, espiritual, social y familiar de las mujeres durante las fases de diagnóstico y control. Objetivos: descripción de las vivencias y prácticas de las mujeres con cáncer de mama en cada una de las fases descritas, identificando los factores de condicionamiento básico (edad, nivel educativo, etc.), describiendo requisitos de autocuidado universal (relaciones familiares y con el personal de salud), requisitos de desviación de la salud (conocimientos generales sobre el cáncer) e identificando prácticas y necesidades de autocuidado. Metodología: estudio cualitativo etnográfico desarrollado con diez mujeres con cáncer de mama, cinco en tratamiento y cinco en control ambulatorio. La información se obtuvo a partir de entrevistas, semiestructuradas, entrevista abierta, historias clínicas y diarios de campo y fueron ajustadas de acuerdo a la teoría general del déficit de autocuidado de Dorotea Orem. Resultados: sugiere que los conocimientos sobre aspectos generales del cáncer de mama son deficientes, el cáncer sigue siendo una enfermedad tabú, sinónimo de sufrimiento, dolor, desfiguración y muerte; genera cambios en el estilo y calidad de vida haciendo que el proceso de adaptación sea lento y difícil. Los trámites que deben realizar las pacientes, constituyen un obstáculo dentro de este proceso. Conclusiones: no existe un modelo de atención integral para la mujer diagnosticada con cáncer de mama, el tratamiento es exclusivamente clínico y deja de lado los aspectos psicosociales inherentes a la enfermedad...


Background: Breast cancer affects physical, emotional, spiritual, social- familiar and behavior components of the women with this pathology. These changes occurred during phases: diagnosis and aftertreatment. Objective: The objectives of this study refers to the description of women’s breast cancer livings and practice in every stage mentioned, identifying conditional basic factors (age, educative level, etc), describing universal self-care requirements (familiar and social relationship), deviation health requirements (general knowledge of cancer) and identifying practices and self-care necessities.Design: It is an ethnographic qualitative study carried out with ten women, fi ve on them in treatment and fi ve on ambulatory control. The information was recollected from semi-structured and open interviews, clinical histories and camp diary; adjusted to self-care defi cit Dorotea Orem´s theory. Results: suggest that acquaintance breast cáncer are defi cient, it continue being a taboo disease, synonymous of suffering, pain, disfiguration and death; it generates changes on way and quality of life making process slowly and difficult. Procedures that patients must carry out constitute an obstacle in the process. Conclusions: Doesn’t exist a standard integral attention pattern for women with breast cancer, the treatment is exclusively clinical and doesn´t include psycho-social aspects own of breast cancer...
