Introduction: In swimming, performance gains after tapering could be influenced by the pre-taper level of fatigue. Moreover, this level of fatigue could be associated with sleep. This study aimed to assess (1) the effect of tapering on performance according to the pre-taper level of fatigue in swimmers and (2) the association between sleep and pre-taper level of fatigue. Methods: Physiological, psychological and biomechanical profiles were evaluated in 26 elite swimmers on 2 occasions to estimate the pre-taper level of fatigue: at T0 and T1, scheduled respectively 10 and 3 weeks before the main competition. Sleep quantity and quality were also evaluated at T0 and T1. Race time was officially assessed at T0, T1 and during the main competition. The level of significance was set at p ≤ .05. Results: Fourteen swimmers (17 ± 2 years) were allocated to acute fatigue group (AF) and 12 swimmers (18 ± 2 years) to functional overreaching group (F-OR). From T1 to the main competition, performance was improved in AF (+1.80 ± 1.36%), while it was impaired in F-OR (-0.49 ± 1.58%, p < 0.05 vs. AF). Before taper period, total sleep time was lower in F-OR, as compared to AF. Conversely, the fragmentation index was higher in F-OR (p = .06). From wakefulness to sleep, body core temperature decreased in AF but not in F-OR. Discussion: Performance gain after tapering was higher in AF swimmers than in overreached. Moreover, pre-taper sleep was poorer in overreached swimmers, which could contribute to their different response to the same training load. This poorer sleep could be linked to a lower regulation of internal temperature.

PURPOSE: The accuracy of heart rate (HR) measured with a wrist-worn photoplethysmography (PPG) monitor is altered during rest-exercise and exercise-rest transitions, which questions the validity of postexercise HR-recovery (HRR) parameters estimated from this device. METHODS: Thirty participants (50% female) randomly performed two 13-minute sequences (3' rest, 5' submaximal-intensity exercise, and 5' passive recovery) on treadmill and bicycle ergometers. HR was measured concomitantly with a 10-lead electrocardiogram (ECG) and a wrist-worn PPG monitor (Polar Unite). HRR was assessed by calculating Δ60 (the difference between HR during exercise and HR 60 s after exercise cessation) and by fitting HRR data into a monoexponential model. RESULTS: By focusing on Δ60 and τ (the time constant of the monoexponential curve), levels of association (r) of the Unite versus the 10-lead ECG were high to very high (.73 < r < .93), and coefficients of variation were >20% (in absolute value), except for Δ60 in the bicycle ergometer condition (11.7%). In 97% of cases, the decrease in HR after exercise appeared later with the Unite. By adjusting the time window used for the analysis according to this time lag, coefficients of variation of Δ60 decreased below 10% in the bicycle ergometer condition. CONCLUSIONS: If a wrist-worn PPG monitor is used to assess HRR, we recommend performing the submaximal-intensity exercise on a bicycle ergometer and focusing on Δ60. Furthermore, to obtain a more accurate Δ60, the time lag between the end of the exercise and the effective decrease in HR should also be considered before the calculation.

PURPOSE: The accuracy of heart rate measured with a wrist photoplethysmography monitor can be influenced by the tightening of the wristband, movement of arms, or kinetics of the signal (eg, steady-state exercise vs on- and off-transients). To test these hypotheses, photoplethysmographic and electrocardiographic (ECG) signals were compared. METHODS: Thirty participants (50% female) randomly performed two 13' sequences (3' rest, 5' submaximal-intensity exercise, and 5' passive recovery) on a motorized treadmill and a bicycle ergometer. Heart rate was measured concomitantly with a 10-lead ECG, a chest-strap monitor, and 2 wrist photoplethysmography monitors (Polar Unite) with different tightening (free vs imposed at the maximum tolerable). RESULTS: The level of association (r) and coefficient of variation (CV; ie, the error of measurement) of the Polar Unite versus the 10-lead ECG is affected by the tightness of the wristband (normal vs high; r = .83 and .96, CV = 16.1 and 8.1% for the treadmill, respectively; r = .71 and .97, CV = 20.3% and 6.2% for the bicycle, respectively) by the phase of the signal (transition vs steady state; r = .90 and .97, CV = 9.0% and 7.6% for the treadmill, respectively; r = .93 and .99, CV = 7.5% and 3.1% for the bicycle, respectively) and movement of arms (treadmill vs bicycle; r = .90 and .93, CV = 9.0% and 7.5% during the transition phase, respectively; r = .97 and .99, CV = 7.6% and 3.1% during the steady-state phase, respectively). CONCLUSION: The accuracy of heart rate measured with a wrist photoplethysmography monitor is affected by the tightness of the wristband and the phase of the signal. A high tightening is required when high accuracy is expected.

In patients with airway obstruction, an increase in breathing frequency at rest is commonly associated with a dynamic hyperinflation (DH). In such a situation, intercostal muscle oxygenation may be disturbed. This hypothesis was examined in a context of simulated airway obstruction in healthy subjects. After a control period of 5 min, twelve participants (20 ± 2 years) breathed at rest through a 20-cmH2O expiratory threshold load, either by increasing or reducing their respiratory rate (ETLF+ or ETLF). Tissue saturation index (TSI) and concentration changes in oxyhaemoglobin (oxy[Hb+Mb]) were measured as well as cardiorespiratory variables. Inspiratory capacity was decreased in ETLF+ (p < 0.001) and correlated with dyspnea. An increase in oxy[Hb+Mb] occurred in ETLF+ that was higher than in ETLF (p < 0.01). TSI was not different between conditions. In healthy subjects at rest, an increase in respiratory rate during a simulated obstruction with an expiratory threshold load resulted in paradoxical response with DH emergence while intercostal muscle oxygenation was preserved.

BACKGROUND: Muscle wasting is frequent in chronic obstructive lung disease (COPD) and associated with low branched-chain amino acids (BCAA). We hypothesized that BCAA supplementation could potentiate the effect of a pulmonary rehabilitation program (PRP) by inducing muscular change. MATERIALS AND METHODS: Sixty COPD patients (GOLD 2-3) were involved in an ambulatory 4-week PRP either with BCAA oral daily supplementation or placebo daily supplementation in a randomized double-blind design. Maximal exercise test including quadriceps oxygenation measurements, functional exercise test, muscle strength, lung function tests, body composition, dyspnea and quality of life were assessed before and after PRP. RESULTS: Fifty-four patients (64.9 ± 8.3 years) completed the protocol. In both groups, maximal exercise capacity, functional and muscle performances, quality of life and dyspnea were improved after 4-week PRP (p ≤ 0.01). Changes in muscle oxygenation during the maximal exercise and recovery period were not modified after 4-week PRP in BCAA group. Contrarily, in the placebo group the muscle oxygenation kinetic of recovery was slowed down after PRP. CONCLUSION: This study demonstrated that a 4-week PRP with BCAA supplementation is not more beneficial than PRP alone for patients. A longer duration of supplementation or a more precise targeting of patients would need to be investigated to validate an effect on muscle recovery and to demonstrate other beneficial effects.

BACKGROUND: In patients with obstructive lung disease, maintaining adequate ventilation during exercise may require greater contraction of the respiratory muscles, which may lead to a compression of muscle capillaries. Furthermore, dynamic hyperinflation (DH) is frequent during exercise in these patients, as it allows to reach higher expiratory flows and to satisfy respiratory demand. However, in such situation, intercostal muscles are likely to be stretched, which could affect the diameter of their capillaries. Thus, in a context of high level of expiratory resistance, intercostal muscle oxygenation may be disturbed during exercise, especially if DH occurs. METHODS: Twelve participants (22±2 years) performed two sessions of moderate exercise (20 min) by breathing freely with and without a 20-cmH2O expiratory threshold load (ETL). Tissue saturation index (TSI) and concentration changes from rest (Δ) in oxygenated ([O2Hb]) and total haemoglobin ([tHb]) were measured in the seventh intercostal space using near-infrared spectroscopy. Respiratory, metabolic and cardiac variables were likewise recorded. RESULTS: Throughout exercise, dyspnea was higher and TSI was lower in ETL condition than in control (p<0.01). After a few minutes of exercise, Δ [O2Hb] was also lower in ETL condition, as well as Δ [tHb], when inspiratory capacity started to be reduced (p<0.05). Changes in [O2Hb] and dyspnea were correlated with changes in expiratory flow rate (Vt/Te) (r = -0.66 and 0.66, respectively; p<0.05). CONCLUSION: During exercise with ETL, impaired muscle oxygenation could be due to a limited increase in blood volume resulting from strong muscle contraction and/or occurrence of DH.

BACKGROUND: During acute bronchial obstruction, despite a higher work of breathing, blood supply and oxygen availability may be reduced in intercostal muscles because of mechanical constraints. This hypothesis was assessed in healthy subjects breathing with and without expiratory load (ETL). METHODS: Eleven men (24 ± 2 years) breathed at rest for 5 min in unloaded condition and for 20 min through a 20-cmH2O ETL. Tissue saturation index (TSI) and changes (Δ) in concentration of total and oxy-haemoglobin ([tHb] and [O2Hb]) were measured in the seventh intercostal space by near-infrared spectroscopy. RESULTS: [tHb] and [O2Hb] decreased with ETL (-5.16 µM and -3.54 µM; p < 0.05). TSI did not vary. Negative correlations were observed between Δ[O2Hb] and changes in expiratory flow rate (ΔVt/Te) and between ΔTSI and Δ V˙E (r = -0.78 and -0.74; p ≤ 0.01). CONCLUSION: Despite decreases in Hb concentrations, saturation in oxygen was not reduced with ETL in intercostal muscles, suggesting a satisfactory ventilatory and/or hemodynamic arrangement.
