RESUMO

We report on a patient with HIV infection and persistent generalized lymphadenopathy (stage III of HIV infection), who developed transitory acantholytic dermatosis (Grover's disease). Both the clinical picture and the histological features were typical for Grover's disease; the skin eruptions subsided within 6 months.

28 HIV-infected persons (19 stadium II, 5 stadium III, 4 stadium IV) were investigated for CD4+ lymphocytes, HIV p24 core antigen and urinary neopterin. Monitoring the course of infection repeated estimations were done. Urinary neopterin was found elevated in 26 out of 28 infected people (p less than 0.001). There is an increase in urinary neopterin levels associated with clinical progression in the course of HIV-infection. An inverse correlation to CD4+ lymphocyte count exists. High levels of neopterin early indicate a clinical progression to disease, as well as a decrease of CD4+ lymphocytes below 0.4 or 0.2 Gpt/l, respectively, and a reappearance of HIV p24 core antigen. The course of 5 cases is presented. In HIV-infection urinary neopterin is a suitable marker of activity and progression.

Chlamydia trachomatis represents today's most important agent of genital contact infections. It occurs more frequently than Neisseria gonorrhoeae. A prevalence-study of Chlamydia trachomatis (C.t.) and Neisseria gonorrhoeae (N.g.) among venerological out-patients is presented. C.t. was grown in cycloheximide treated McCoy cell culture, N.g. on human blood agar. Our of 551 patients (321 males, 230 females) C.t. was isolated in 17.8%, whereas N.g. was isolated in 8.5%. The isolation rate for males was estimated as 23% for C.t., 12% for N.g., compared to 5% for C.t. and 2% for N.g. in women. Nearly half of gonococcal infections was accompanied by C.t. (men 42.1%, women 55.6%). Age related, both C.t. and N.g. infections occurred mainly between 21 and 26 years (about 50%). Infection with C.t. occurred earlier than infections with N.g., 16 years and 18 years respectively. The high prevalence of C. trachomatis infection demand some consequences: 1. Extensing the number of diagnostic facilities. 2. Inauguration of new methods (monoclonal antibody-test, enzymimmunoassays). 3. Diagnostic and treatment of sexual partner. 4. Treatment with tetracyclines. 5. Therapy regiment for gonococcal infection: single high dose penicillin combined with probenicide followed by tetracycline therapy for 7-10 days. 6. Establishing of a chlamydial diagnostic on a broad base.

RESUMO

Urogenital chlamydial infection should act as a trigger of peripheral and axial form of arthritis like sexually acquired reactive arthritis and Reiter's syndrome assumedly. HLA B-27 positive patients with rheumatic disorders, mainly M. Bechterew (55 out of 62 men and 18 out of 25 women) were investigated for serum-IgG-antibodies to Chlamydia trachomatis (ELISA). Among HLA B-27 positive men and women urogenital disorders in their history were more common as compared with controls. There were found no differences concerning the rate of antichlamydial antibodies between patients and controls. It is supposed, that urogenital bacterial infections rather others than chlamydial infection may be connected with M. Bechterew.

RESUMO

In HIV-infected people the course of syphilis is altered and aggressive. Besides seronegativity Neurosyphilis can manifest premature and as a outcome of failed penicillin-therapy in secondary syphilis. Two cases of neurosyphilis are reported and discussed. The usefulness of spinal fluid analysis is pointed out. Recommendations for diagnostics and therapy are given.
