RESUMO

BACKGROUND: This study aimed to investigate the differentiation of ameloblastic-like cells and the nature of the secreted eosinophilic materials in adenomatoid odontogenic tumors. METHODS: We studied histological and immunohistochemical characteristics of 20 cases using: cytokeratins 14 and 19, amelogenin, collagen I, laminin, vimentin, and CD34. RESULTS: Rosette cells differentiated into ameloblastic-like cells positioned face-to-face, displaying collagen I-positive material between them. Epithelial cells of the rosettes can differentiate into ameloblastic-like cells. This phenomenon probably occurs due to an induction phenomenon between these cells. The secretion of collagen I is probably a brief event. Amelogenin-positive areas were interspersed by epithelial cells in the lace-like areas, outside the rosettes and distant from the ameloblastic-like cells. CONCLUSIONS: There are at least two types of eosinophilic material in different areas within the tumor, one in the rosette and solid areas and another in lace-like areas. The secreted eosinophilic material in the rosettes and solid areas is probably a product of well-differentiated ameloblastic-like cells. It is positive for collagen I and negative for amelogenin, whereas some eosinophilic materials in the lace-like areas are positive for amelogenin. We hypothesize that the latter eosinophilic material could be a product of odontogenic cuboidal epithelial or intermediate stratum-like epithelial cells.

RESUMO

Using computer vision through artificial intelligence (AI) is one of the main technological advances in dentistry. However, the existing literature on the practical application of AI for detecting cephalometric landmarks of orthodontic interest in digital images is heterogeneous, and there is no consensus regarding accuracy and precision. Thus, this review evaluated the use of artificial intelligence for detecting cephalometric landmarks in digital imaging examinations and compared it to manual annotation of landmarks. An electronic search was performed in nine databases to find studies that analyzed the detection of cephalometric landmarks in digital imaging examinations with AI and manual landmarking. Two reviewers selected the studies, extracted the data, and assessed the risk of bias using QUADAS-2. Random-effects meta-analyses determined the agreement and precision of AI compared to manual detection at a 95% confidence interval. The electronic search located 7410 studies, of which 40 were included. Only three studies presented a low risk of bias for all domains evaluated. The meta-analysis showed AI agreement rates of 79% (95% CI: 76-82%, I2 = 99%) and 90% (95% CI: 87-92%, I2 = 99%) for the thresholds of 2 and 3 mm, respectively, with a mean divergence of 2.05 (95% CI: 1.41-2.69, I2 = 10%) compared to manual landmarking. The menton cephalometric landmark showed the lowest divergence between both methods (SMD, 1.17; 95% CI, 0.82; 1.53; I2 = 0%). Based on very low certainty of evidence, the application of AI was promising for automatically detecting cephalometric landmarks, but further studies should focus on testing its strength and validity in different samples.

RESUMO

INTRODUCTION: The use of teaching resources and digital objects has gradually been incorporated into dental schools. This study aimed to evaluate the digital anatomy table in teaching-learning process of the temporomandibular joint (TMJ) anatomy and the student's perception regarding this resource. MATERIALS AND METHODS: A cross-sectional study was conducted on 41 undergraduate dental students. Knowledge assessment tests were applied at different time intervals: before and after the TMJ theoretical class, after the practical class on prosected specimens and after the use of digital anatomy table. The medians of the scores obtained in the three groups (theoretical class, practical class and digital table) were descriptively analysed and submitted to Wilcoxon, Kruskal-Wallis and Student-Newman-Keuls statistical tests. The perception survey was conducted at the end of the study. RESULTS: When the teaching strategies were compared amongst each other, the medians of the scores with the digital anatomy table were significantly higher than after the theoretical and practical classes. At the end of the research, there were no differences amongst the three groups (theoretical class, practical class and digital table). Regarding the perception, the majority of the students reported that the digital anatomy table helped them to understand the content of the theoretical class. CONCLUSION: It was concluded that learning with the use of the digital anatomy table did not increase the knowledge of dental students with respect to the TMJ anatomy. The students' perceptions of the digital anatomy table were positive and that it could be used as an additional resource in the teaching-learning process.

RESUMO

ABSTRACT This study is a literature review on the contributions of active methodologies in Higher Education in the health area. From the reviewed literature (texts, articles and books), an academic essay was written, addressing ideas, reflections, literature citations and authors' personal impressions on the theme. After analyzing the literature, it was realized that active methodologies despite being involved in the teaching-learning process for several years, still require that teachers and students widely appropriate their benefits. The implementation of this change will help in coping with the factors that have negatively interfered in higher health education. It is a challenge that needs to be embraced by teachers and students. Conclusions: Active teaching methodologies can be included in all teaching models and methods and aim to promote learning and expand the possibilities for students to develop their competencies.

OBJECTIVE: To assess the effectiveness of different anesthetic solutions for pain control immediately after the extraction of lower third molars. METHODS: Nine databases were used to identify randomized clinical trials, without restriction of language or year of publication. The "JBI Critical Appraisal Tools for Systematic Reviews" was used to assess the risk of bias in the studies. The network meta-analysis was performed to compare the effectiveness of different anesthetics to control the pain immediately after the surgery of lower third molars, using the standardized mean difference (SMD) as the effect estimate. The GRADE approach was used to assess the certainty of evidence. RESULTS: The search presented 13,739 initial results, from which 45 met the eligibility criteria and presented low to moderate risk of bias. Thirteen studies were included in the meta-analysis. The 2% lidocaine + clonidine presented the lowest pain scores (SMD = - 1.44; - 2.72 to - 0.16) compared to 4% articaine + adrenaline, followed by 0.5% bupivacaine + adrenaline (SMD = - 1.36; - 2.13 to - 0.59). The certainty of evidence varied between very low to moderate. CONCLUSION: 2% lidocaine + clonidine and 0.5% bupivacaine + adrenaline were the anesthetics with the highest probability for pain control immediately after the surgical procedure of removing impacted lower third molars. CLINICAL SIGNIFICANCE: The use of an adequate anesthetic with effective pain control can contribute to a more comfortable postoperative period.

RESUMO

BACKGROUND: The study aimed to assess the influence of different implant insertion angles and depths on the stresses produced on the surface of peri-implant bone tissue under axial and oblique loading. MATERIAL AND METHODS: The entire study followed the recommendations of the Checklist for Reporting In-vitro Studies (CRIS). The implant was placed in the region of element 36, according to the following models: M1 (0 mm / 0°); M2 (0 mm / 17°); M3 (0 mm / 30°); M4 (2 mm / 0°); M5 (2 mm / 17°); M6 (2 mm / 30°). The models were subjected to loading, with intensity of 100 N. The stress assessment followed the Mohr-Coulomb criterion and qualitative and quantitative analyses were performed. RESULTS: Angled implants and installed below the bone crest produced the highest stresses on the cortical bone, and the axial load presented the highest stress peaks on the buccal side of implants perpendicular to the bone crest. Regardless of the type of load (axial or oblique), inclined implants presented the highest stress peaks on the lingual side of the cortical bone. CONCLUSIONS: Implants installed perpendicular to and with a prosthetic platform at bone crest height provided the lowest stresses to peri-implant bone tissue under both axial and oblique loading. Key words:Finite element analysis, dental implants, axial loading, biomechanical phenomena.

RESUMO

ABSTRACT Objetive To evaluate whether the area of study of undergraduate students influences oral health knowledge, attitudes and practices, in addition to the use of dental services, self-reported oral morbidity, self-perception and impacts of oral health. Methods A cross-sectional study was performed, with a probabilistic sample of 681 students from the second semester of a Higher Education Institution, who were allocated into Health (H; n = 347) and Non-Health (NH; n = 334) groups, and answered a questionnaire encompassing their sociodemographic profile and aspects of oral health. Data were analyzed by Fisher's Chi-Square/Exact Test (α = 5%). Results The majority of students were female (H = 70.0%, NH = 56.9%, p = 0.0004), single (H = 82.7%, NH = 76.0%, p = 0.0429) and did not work (H = 30.5%, NH = 37.7%, p = 0.0482). The groups differed in terms of receiving oral health guidelines from the media (H = 19.9%, NH = 13.8%, p = 0.0333) or from other health professionals (H = 25.6%, NH = 13.2%, p <0.0001). Regarding the reasons for their most recent dental appointment, in the Health group "treatment" (42.9%) and "pain" (6.9%) prevailed, while for the Non-Health group "review, prevention or check-up" (41, 9%) and "extraction" (8.1%) (p = 0.0169) were the most prevalent. The Health group was more satisfied with their oral health, with less frequent reports of a self-perception of bad breath (H = 30.3%, NH = 38.3%, p = 0.0483). There was no association between self-perception and impacts on oral health (p> 0.05). Conclusions The groups differed regarding the source of information on oral health, the reason for seeking and type of treatment, and the self-perception of bad breath, with the Health group reporting greater satisfaction with their oral health.

RESUMO

BACKGROUND: The present study aimed to assess the stresses produced on the surface of the bone tissue around dental implants with three different insertion angulations subjected to axial and oblique loading. MATERIAL AND METHODS: The study was created according to the recommendations of the Checklist for Reporting In-vitro Studies (CRIS). The Straumann™ bone level RC (4.1 x 10 mm) implant, Cone Morse connection (CM), RC Straumann Variobase™ with abutment (3.5 mm) was placed in the region of element 16, with the platform positioned at the height of the bone crest. Three assessment models were produced: model M1 or control - implant perpendicular to the bone crest; model M2 - implant angulated at 17° relative to the bone crest; and model M3 - implant angulated at 30° relative to the bone crest. The masticatory loads were simulated with 100 N of intensity and two loading patterns (axial and oblique) were applied to each model. Then, the models were exported to the finite elements simulation software Ansys Workbench V19.2 (Ansys Inc., Canonsburg, PA, USA). To assess the finite elements, qualitative and quantitative analyses were performed. RESULTS: It was observed that, under axial loading, qualitatively, the peaks occurred in the cavosurface region, palatal aspect in M1 and M2, and buccal aspect in M3. Quantitatively, the greatest angulation resulted in a low stress peak. Under oblique loading, qualitatively, the peaks occurred in the cavosurface region, buccal aspect in the three groups. Quantitatively, the greatest angulation of the implant resulted in an increase in stress peaks on the buccal aspect. CONCLUSIONS: Under axial loading, the three insertion angulations of the implant - M1, M2, and M3 - were clinically viable. When subjected to oblique loading, the 30° angulation (M3) suggested a significant risk of bone loss and it was contraindicated. Key words:Finite element analysis, dental implants, load support.

RESUMO

PURPOSE: Skin ageing is marked by structural and functional changes in epidermis and dermis, which result clinically in wrinkles, loss of elasticity, and rough-textured appearance. In this context, different dermal fillers have been used to overcome these negative effects associated with skin ageing, such as hyaluronic acid (HA) and poly-L-lactic acid (PLLA). Despite their low immunogenicity, these materials can cause an inflammatory reaction after application. MATERIALS AND METHODS: Considering high demand of HA and PLLA as filler material, this study aimed to evaluate their in vitro and in vivo effects. For the in vitro study, human dermal fibroblast cell cultures were supplemented with HA or PLLA for 24, 48, and 72 h. The following parameters were assayed: 1) cell proliferation, 2) cell viability, and 3) quantification of type I collagen by ELISA. For the in vivo study, HA or PLLA was injected in the dermis of Wistar rats and the tissues were collected after 15, 30, and 60 days for histologic evaluation and for quantification of type I collagen by Western blotting. The quantitative data were statistically analyzed using an ANOVA two-way. The significance level was set at 5%. RESULTS: At 72 h, high cell proliferation was observed for HA compared to control (p<0.05). Cultures exposed to PLLA exhibited a reduction in both cell proliferation and viability compared to control in all time points (p<0.05). Type I collagen expression was greater in cultures exposed to HA or PLLA compared to control (p<0.05). Histologic analysis showed the presence of multinucleated cells only in the PLLA group in all experimental time points. Western blotting analysis revealed high content of type I collagen in HA compared to PLLA (p<0.05). CONCLUSION: The present study addresses a potentially unfavorable effect of dermal PLLA filler on the fibroblast phenotype, with possible clinical complications, unlike HA.

RESUMO

BACKGROUND: Periodontitis and coronary artery disease (CAD) share an inflammatory etiology; there is a recent concern regarding the investigation of an association between these two conditions. Current theories indicate that cytokines and proteins have an important role in this process. C-reactive protein and interleukin-6 are inflammatory derivatives produced in the presence of periodontitis and in the pathophysiology of coronary disease. The polymorphisms of CRP + 1444 C > T and IL6-174 G > C are recognized in the literature as being related to CAD. OBJECTIVE: This study investigates the association between periodontitis and coronary artery disease, through the presence of PCR and IL-6 polymorphisms. METHODS: We selected 80 patients who underwent diagnostic catheterization in the HU of UFSM. The presence of periodontitis was determined by the Community Periodontal Index, whereas the CAD was established by the medical report. DNA was collected from a saliva sample and the presence of polymorphism was determined by PCR and restriction enzymes. A significance level of 5% was adopted. RESULTS: The mean age of all participants (p = 0.035, OR 2.65; 95%CI: (1.02-6.87) male gender (p = 0.012, OR 3.37; 95% CI: (1.28- (p = 0.013, OR 3.66; 95% CI: (1.27-10.5)), PCR polymorphism + 1444C > T (p = 0.001, OR 6.37; 95% CI:, (2.25-17.9)) and IL6 -174 G > C polymorphism (p = 0.025, OR 2.87, 95% CI: (1.09-7.55)) were statistically associated with the presence of CAD. Age > 60 years and presence of the PCR +1444 C > T polymorphism remained independently associated with CAD after adjustment by logistic regression. CONCLUSIONS: The presence of the PCR + 1444 C > T polymorphism in this study was independently associated with the presence of coronary artery disease.

Resultados: A idade mediana de todos os participantes (p = 0,035; OR 2,65; IC 95% [1,02-6,87]), gênero masculino (p = 0,012; OR 3,37; IC 95% [1,28-8,9]), periodontite (p = 0,013; OR 3,66; IC 95% [1,27-10,5]), polimorfismo da PCR +1444 C > T (p = 0,001; OR 6,37; IC 95% [2,25-17,9]) e polimorfismo da IL-6 -174G > C (p = 0,025; OR 2,87; IC 95% [1,09-7,55]) foram estatisticamente relacionados à DAC.

Objective: Facial appearance has become an important factor of social interaction. The perception of patients regarding the changes promoted by treatment has become a focus of study today. Thus, the proposal of the present study was to evaluate the esthetic perception of lay people regarding esthetic periodontal changes. Method: The total sample comprised 180 individuals divided into three different age groups (15-19, 35-44, and 65-74 years). Nine images with digital alterations in the gingival contour were used. One of the images served as an esthetic standard for the research (ideal image). The images were presented to the evaluators using slide show software (Microsoft PowerPoint 2013, Microsoft, California). Was used the Wilcoxon, Kruskal-Wallis and Mann-Whitney test. The level of significance was 5% (α = 0.05). Results: Statistical differences were found between the esthetic perception of different age groups. The sample was composed only of lay people of three different age groups, and it was evident that the younger age group has a more accurate perception regarding changes in the gingival contour. Conclusion: The changes in the gingival contour perceived as less attractive were the changes between the central incisors. The younger age group had a more accurate esthetic perception in this study. (AU)

The Scientific Mentorship Program (SMP) seeks to foster vocational aims and encourage the development of scientific thought in undergraduate students, and participation in the program has been found to have a positive impact on the performance coefficient (PC) of Dentistry students, but not in students from the Medicine course. The present observational study was therefore conducted to identify the personal, contextual and vocational aspects of the academic experiences of Medicine and Dentistry students, which may have influenced this finding. A total of 123 students participated in the study, of whom 74 (60.2%) were from the Dentistry (D) course and 49 (39.9%) from the Medicine (M) course. The short version of the Student Academic Experience Questionnaire (or QVA-r) was applied (54 questions/Likert Scale responses). The data were tabulated and submitted to statistical analysis using the Student's t-test (α=5%). The main results revealed that the career, study and institutional dimensions differed between the groups, with higher mean scores obtained by the students of the Medicine course in the career dimension (M=4.1±0.6a; D=3.8±0.4b). Higher scores were obtained for students from the Dentistry course in the study (M=3.6±0.5 b; D=3.9±0.7a) and institutional (M=3.7±0.2 b; D=4.1±0.2a) dimensions. There was no difference between the courses in the personal and interpersonal dimensions, or in the overall QVAr score. It was concluded that different academic experiences may have influenced the contrasting impact of the SMP on the PC of the Dentistry and Medicine students (AU).

RESUMO

ABSTRACT Objective: To describe the development of a dentistry school strategy in order to reduce the occupational risk related to hepatitis B. Methods: The academic registration documents of 242 students entering the institution between the years 2006 and 2013, were evaluated, among which were of copies of the updated vaccination cards and anti-HBs serologic testing. Demographic variables and others related to the vaccination status of hepatitis B and seroconversion were considered. Results: One hundred percent of the students were found to be vaccinated, and 87.2% had vaccination records of three doses. The results of anti-HBs tests proved seroconversion in 91.3% of the students. From 2011, the dental school was able to institutionalize the follow-up behavior of the students who had anti-HBs non-reactive. Of the twenty individuals whose serology was negative, nine students (45% of the total and all of the 2011-2013 class) were followed-up and repeated the basic vaccination and anti-HBs test; eight seroconverted and one was considered a non-responder, increasing the percentage of immune students to 95%. Eleven (55%) had other unregistered behavior or the documents analyzed showed no data on them. Conclusion: The procedure of following-up the registration in vaccination records required by the Biosafety Committee of the institution was shown to be effective in reducing the occupational risk of hepatitis B among the students.

(AU)

The need for orthodontic treatment is increasing in adults looking for prosthetic rehabilitation because many need molar uprighting. The aim of this study was evaluate the maximum tension caused in the periodontal ligament by different sizes of roots during mandibular second molars uprighting by the finite element method in three dimensions. Three virtual models were used. A tooth model was made with the total size of 19.64 mm, being 11.41 mm root (medium root). This root was plus 2 mm in a model (long root) and removed 2 mm of other (short root). Among the criteria of interpretation, it was used the proposed by von Mises (maximum distortion energy). Evaluating the tension in furcation region, the highest tension value was in the tooth with medium root (0.026 MPa) as well as occurred in the mesial cervical region (0.036 MPa). At the distal cervical region, the short root had an opposite behavior to its length, being the highest tension value (0.023 MPa). In mesial apical region, the tooth with short root had the highest value (0.012 MPa) as well as occurred in the distal apical region (0.016 MPa). It was concluded that the mesial cervical region obtained the highest tension and in the mesial apical region the lowest tension of the 3 sizes of root comparing to the other areas of the periodontal ligament. Most maximum tension variation occurred in the apical region, especially at the mesial region. The tooth with short root size has suffered the greatest tension in both apical regions (AU)

RESUMO

The aim of this study was to evaluate the in vitro cytotoxicity and the in vivo analgesic effect and local toxicity of the local anesthetic butamben (BTB) encapsulated in conventional or elastic liposomes incorporated in gel formulations. The results showed that both gel formulations of liposomal BTB reduced the cytotoxicity (p < 0.001; one-way ANOVA/Tukey's test) and increased the topical analgesic effect (p < 0.05; one-way ANOVA/Tukey's test) of butamben, compared to plain BTB gel. The gel formulations presented good rheological properties, and stability assays detected no differences in physicochemical stability up to 30 d after preparation. Moreover, histological assessment revealed no morphological changes in rat skin after application of any of the gel formulations tested.

RESUMO

PURPOSE: To evaluate in vitro, the efficacy of gutta-percha (GP) and polytetrafluoroethylene (PTFE) tape to act as a seal against the penetration of Escherichia coli through prosthetic implant abutments (PIA) with external hexagon (EH) and morse taper (MT) connections. MATERIALS AND METHODS: 120 sets of implant analogs with their respective PIA were used. 60 sets from each system were divided into four groups (n = 15): I - GP sealing; II - PTFE tape sealing; III - no sealing (NS) and IV - negative control. Microbiological challenge was performed by placing bacterial colonies inside the titanium PIA before screwing them into their respective analogs. Each set was immersed in 5 mL of BHI and incubated for 14 days with daily checks for contamination, according to the nutrient medium turbidity. RESULTS: The type of seal used influenced microbial penetration at all evaluation times (p < 0.05). Compared to GP, PTFE led to an increase in turbidity and its effectiveness against microbial penetration was equivalent to group NS. When the sealing efficacy of each group was compared in terms of time, Cochran's Q test revealed that in the two groups sealed with PTFE, there was a progressive increase in the number of turbid nutrient media (p < 0.0001), while in the group sealed with GP, this was only observed in the MT system (p = 0.0310). CONCLUSIONS: Sealing the PIA channel with GP was a significantly superior approach to sealing it with PTFE tape. PTFE tape or NS led to a progressive increase in microbiological leakage over time in PIA channels, however, this only occurred with GP in the MT system.

RESUMO

This study evaluated the bone regeneration process in rabbit calvaria induced by three types of biomaterials: two xenogenous, consisting of deproteinized bovine bone, while the other was alloplastic, based on biphasic calcium phosphate. Five New Zealand white rabbits weighing between 2,900 and 3,500 g were submitted to four standard 8 mm-diameter perforations at the parietal bone. Three perforations were filled with three grafts and biomaterials, two of them received bovine Bio-Oss® and Endobon® Xenograft Granules, and the other consisted of fully alloplastic Straumann® Bone Ceramic. The fourth remaining cavity was used as control with coagulum. After eight weeks, the animals were sacrificed, and the samples were prepared for morphometric and qualitative analysis. The cavities filled with alloplastic biomaterials showed higher percentages of newly formed bone (p<0.05), while the cavities with xenogenous biomaterials showed higher amount of residual graft (p<0.05). Although the results showed greater bone formation with Straumann® Bone Ceramic, further studies are required to prove which is the more effective biomaterial for bone induction process.
