Comparative assessment of iron metabolism in the population of multiparas and grand multiparas indicates that the latter are at risk to develop iron-deficiency anemia. Ferrokinetic parameters in both populations evidence iron deficiency, especially in grand multiparas. As shown by ferritin assays in the serum, body iron in the grand multiparas is 15.5 times less than that in healthy nonpregnant women. In grand multiparas iron deficiency presented in light and moderate degrees.

Efficacies of ferroplex, conferon, and feramide in the treatment of pregnant patients with iron-deficiency anemia were compared. Ferroplex proved to be the most effective. Therapy with this drug brought about the best correction of iron metabolism parameters. Feramide was the least effective agent. The efficacy of iron preparations depends on the ionic form of this metal and the combination of iron with the agents conducive to its better absorption in the gastrointestinal tract.

A total of 160 multiparous women with iron deficiency anemia (IDA) who have 4 and more children were under study. Hemoglobin parameters in patients with non-severe anemia (n = 64) fluctuated from 119 to 90 g/l, in patients with moderate severity (n = 60) from 89 to 70 g/l, in those with severe stage of anemia--69 g/l and lower. The latent anemia group consisted of women whose hemoglobin values were 120 g/l and higher, however, the figures characterizing serum iron, transferrin and ferritin content evidenced iron deficiency. Parameters of serum iron, transferrin and ferritin metabolism were studied in all the patients. Decreased levels of serum iron and ferritin and a compensatory increase of transferrin content were recorded. Manifest signs of acquired dyserythropoiesis and a significant increase of ineffective erythropoiesis (34.96 +/- 0.89%) were demonstrated in myelograms. It has been recommended that routine measures used for the treatment of IDA in multiparous women should be supplemented by anabolic agents and blood substitutes for protein parenteral nutrition.

Though the total iron-binding activity of the blood serum directly depends on blood serum transferrin level, these are different parameters. It is possible to measure transferrin levels with direct estimation of the share of this protein saturation with iron without preliminary measurement of the total iron-binding activity of the blood serum. The degree of transferrin saturation with iron is virtually unrelated to the results of its measurement by various immunochemical methods.

Milk levels of iron, the iron-binding protein lactoferrin, percentage of iron in this protein have been estimated in 288 milk samples of normal lactating women. Milk iron concentrations varied within the first day and declined with time of lactation. Similar patterns of change were characteristic for lactoferrin which was highest in milk during the 1st month of lactation but steadily fell by the end of lactation. Iron percentage in lactoferrin remained low during one year of lactation, suggesting a bacteriostatic function of milk lactoferrin.

The authors provide statistical data attesting to the fact that during the development of iron deficiency anemia in schoolchildren, the hematological, biochemical indicators characterizing iron metabolism in the body and phagocytic activity of peripheral blood leukocytes, one of the parameters of cellular immunity, undergo noticeable changes. It has been noted that in the regions where toxic chemicals are intensely used, both children with iron deficiency and normal ones demonstrate a significant decrease of phagocytic activity of leukocytes in spite of the fact that no quantitative alterations in iron metabolism are detectable in such a case.

Biochemical parameters of iron metabolism have been examined in 36 newborns born to mothers with normal iron metabolism and in 58 healthy children aged under one year. Normal values of serum iron, serum transferrin and ferritin, and of transferrin saturation with iron in newborns and infants aged under one are presented.

Importance of blood serum ferritin is discussed.
