[Susac syndrome is an important differential diagnosis in monosymptomatic encephalopathy in young adults].

This is a case report of a 29-year-old man, who presented with progressive confusion, memory problems and personality changes during 1.5 months. Later, he developed bilateral hearing impairment but had no visual symptoms. A brain MRI showed numerous small hyperintensities in both hemispheres, a retinal fluorescence angiography revealed multiple hyperfluorescent arterial occlusions, and an audiogram showed bilateral hearing impairment. The patient was treated for Susac syndrome with high-dose corticosteroids initially, followed by intravenous immunoglobulin and cyclophosphamide.

[Intramedullary spinal cord tumour as differential diagnosis to bilateral muscle weakness].

Intramedullary spinal cord tumours (IMSCT) are rare neoplasms, which can potentially lead to severe neurologic deficits. In this case report, an 83-year-old man presented with rapidly progressive bilateral muscle weakness developed within few weeks. MRI revealed a spinal cord tumour at the C7/Th1 vertebral level. Surgical resection was performed, and the histological diagnosis was subependymoma. Focus on IMSCT as a differential diagnosis is important, since early diagnosis and treatment will pave the way for a better prognosis.
