RESUMO

Investigation of catamnesis of 100 patients with atherosclerotic carotid artery stenosis (50 were operated and 50--non-operated) revealed, that in some cases carotid endarterectomy prevented repeated cerebrovascular disorders and had prophylactic significance. The frequency of repeated stroke in the group of non-operated patients was 3 times higher than in the group of operated patients. In operated cases no more TIAS were observed, but they persisted in non-operated cases.

RESUMO

The efficacy of carotid endarterectomy (CE) was estimated in 50 patients with stenosis of the internal carotid artery due to atherosclerosis. In addition to neurological investigations, all the patients were subjected to USDG of the head vessels, cerebral angiography and CT of the brain. Patients with the signs of cerebral circulation disorders or those who suffered ischemic brain stroke with a reversible neurological deficit were the most prospective candidates for CE. According to the cerebral angiography data, the degree of stenosis ranged from 50 to 90%. Surgical intervention was performed within different times after derangement of brain circulation, most frequently from 3 months to 1 year. The overwhelming majority of the patients suffered operations without any complications. The lethality amounted to 4% (because of acute cardiovascular failure and myocardial infarction). The total mortality and morbidity due to brain stroke were 5.9%. Indications for carotid endarterectomy in stenosis of the ICA are under consideration.

RESUMO

The paper analyzes the study into the specific features of cerebral hemodynamics in patients with internal carotid artery occlusion. The study was performed by using Doppler ultrasound techniques. It revealed three different types of collateral blood flow of the brain according to the function of orbital anastomosis. The authors propose a new unique assessment of indications for a reconstructive operation on the external carotid artery for brain revascularization. The criteria proposed by the authors for assessing the cerebral hemodynamics enable the clear-cut indications for the above operation to be defined. The paper also summarizes the experience in examining and surgically managing 38 patients with internal carotid artery occlusion, who underwent revascularization of 39 channels of the external carotid artery.

RESUMO

The clinical experience of the Department of Vascular Surgery of the Vishnevsky Institute of Surgery, covering a period between November, 1983 and December, 1987 was analysed. Problems of the etiology and pathogenesis of vertebrobasilar insufficiency and the peculiarities of choosing the methods for surgical correction according to the extent and localization of the occlusive lesions were studied. The sequence and volume of the surgical intervention in patients with isolated and combined affections of the vessels of the vertebrobasilar and carotid channels are discussed.

RESUMO

The authors have performed a complex ultrasonic angiographic and nephrological examination of 32 patients with associated occluding injuries of branches of the arch and renal arteries. The indications and tactics of reconstructive interventions on renal arteries in patients with pathology in question have been established which were not followed by neurological complications. All the patients who survived have shown substantial improvement of neurological symptoms.

RESUMO

The article deals with characteristics of the hemodynamics of the extracranial major arteries in patients with different forms of cerebrovascular insufficiency due to occlusive lesions of the brachiocephalic arteries. Clinicoangiographic characteristics of patients with this pathology associated with atherosclerosis and nonspecific aortoarteritis are presented. Angiography showed two variants of occlusive damage to the brachiocephalic arteries: bicarotid and multiple. The authors were the first to use comprehensive examination of patients with the employment of the spectral computerized analysis (SCA) of ultrasonograms (USG) of the common carotid and suprapubic arteries and ultrasonic angiography with the calculation of the circulation rate in the common carotid arteries. Using SCA of USGs criteria of different degrees of stenosis of the common and internal carotid arteries were developed. A correlation analysis of the data obtained by SCA of USGs and of the values of the circulation rate in the common carotid arteries was made. The highest correlation of these parameters was observed in patients with stenosis of 60-90% and occlusion of the internal carotid and common carotid arteries.

RESUMO

A review of combined ultrasonic data in a group of normal subjects and in 20 patients with occlusive brachiocephalic aortic lesions has demonstrated that combined use of highly-informative ultrasonic techniques, including ultrasonic dopplerography, ultrasonic angiography and spectrum analysis of the Doppler echo expands considerably possibilities of the diagnosis of occlusive lesions and enables quantitative assessment of regional hemodynamic disorders.
