RESUMO

Problems of pathogenesis, diagnosis and therapy of gout and chondrocalcinosis in which deposits of uric acid salts and calcium are found in the articular tissue are of primary importance in modern rheumatology. The authors give observations of 10 out of 218 patients with primary gout in whom roentgenology had revealed signs of calcium salt deposits in the joints and periarticular tissue.

RESUMO

One hundred and sixteen males with mixed Ureaplasma ++, chlamydial and gonorrheal urethritis were treated. 22 of them suffered from recent torpid urethritis and 94 were chronic patients including those with complications. The diagnosis was confirmed by the laboratory tests. The patients were subjected to complex treatment with immunostimulation (by the gonorrhea vaccine, methyluracil and pyrogenal), local therapy and the use of antibiotic combinations on day 8: gentamicin (0.84 g per treatment course) and doxycycline (2.1 g per treatment course). The treatment course was equal to 14 days. Favourable etiological and clinical results were observed in 114 patients. The recovery was confirmed by 3-fold verification. A high efficacy of the developed method studied clinically was stated.

Sixteen specimens of cadaveric intertoe folds of patients who had suffered from intertrichinous mycoses of the soles (miners who died in accidents or because of traumas) were examined by histologic and histochemical methods. For control intertoe fold specimens from 10 suddenly dead workers without mycoses were examined. Examinations of skin specimens from mycosis foci have revealed fungus mycelium in the epidermis and superficial layers of the derma, focal perivascular infiltrates consisting mainly of T-lymphocytes (a positive reaction to alpha-naphthylacetate esterase), macrophages, sensitized lymphocytes (high acid phosphatase activity); this necessitates administration, along with mycocidal agents, of angioprotectors, proteolysis inhibitors, immunostimulants. Histoenzymologic studies have revealed impairments of the principal components of metabolism and activation of glycolysis processes. Less manifest changes were revealed in apparently intact skin, this being an evidence of a systemic involvement of the skin in a prolonged mycotic process.

The authors emphasize the significance of independent work of physicians, trained at the Chair, for the improvement of their continuous education. The role of the teacher in this process is discussed, the methods of training are presented, and various forms of independent work of the physicians in hospitals and outside the classrooms are described.

RESUMO

Communal and hygienic conditions have been examined in families of 50 miners with a history of mycoses of the feet. Mycotic infection has been detected in 44 of 121 members of their families. Infection in the families is explained by neglect of preventive measures, inadequate hygienic conditions, and nonobservance of individual prophylaxis rules.

The authors give their comments on the paper by A. A. Kalamkarian and V. A. Samsonov on the taxonomy of diffuse neurodermatitis and atopic dermatitis. They consider that the term "atopic disease" is more validated; discussing the wording of a detailed diagnosis, they come to the conclusion that the term "diffuse" should be used only as a characteristic of the process dissemination in neurodermatitis.

RESUMO

The authors analyze the contribution of the Scientific Society of Dermatologists and Venereologists to continuous education of physicians: discussion of pressing problems in clinical dermatology and venereology, organization of infectious diseases control, analysis of the research carried out by the Society's members. Special attention is paid to philosophic problems in biology and medicine. A positive effect of field sessions of the Society in the major towns of the region is emphasized; such sessions promote a wider acquaintance of the local physicians with the newest trends in this field.

RESUMO

Comprehensive examinations of dermal morphology and function, carried out in 50 patients suffering from psoriasis, have revealed manifest hypoxia, depression of the intramitochondrial energy production, reduction of the cellular synthetic potency, increased protein catabolism, impaired transcapillary transport and vascular and tissue permeability, immediate and delayed type hypersensitivity local immune reactions. Basing on these findings, the authors have developed a new effective method for multiple modality treatment of psoriasis patients. The new scheme includes, besides the routine modalities (sedative and lipotropic drug, vitamins, UV irradiation, diets, ointments), subcutaneous injections of oxygen into the large foci (200 ml daily, 12-18 injections per course, alternating the injection sites) and oral intake of copper sulfate and reduced iron (3 times daily after meals for 20-30 days). The study has involved 112 patients with psoriasis (61 patients in the test group and 51 in the reference one). Clinical and morphologic examinations have demonstrated the advantages of the new method: clinical remission has occurred 1.8 times more often, the length of treatment has been cut by 7 days, an earlier and more complete recovery of the morphofunctional characteristics of the dermis has been achieved.

RESUMO

Examinations of 18 patients with mycoses of the foot complicated by allergic processes, have revealed elevated blood serum concentrations of histamine, serotonin, cathepsin D, and acid phosphatase, and a lowered activity of monoamine oxidase, as compared to 22 patients with mycoses not complicated with allergic manifestations. This fact reflects one of the aspects of a pathogenetic difference between these patients and necessitates combined therapy to correct the detected abnormalities.
