[The experience of diagnostic and surgical treatment of Barrett's oesophagus]. / Opyt diagnostiki i khirurgicheskogo lecheniia pishchevoda Barreta.

There were analyzed five-year late results of prospective investigation on surgical treatment of 23 patients, suffering long-segment Barrett's esophagus (BE). Esophagoplasty and esophagocardioplasty was performed in 7 (30.4%) patients and total fundoplication in combination with endoscopic ablation of metaplastic epithelium--in 16 (69.6%). Excellent and good results marks in 19 (827%) of patients, including those with antireflux operation, and satisfactory--in 4 (17.3%), witnessing high efficacy of surgical treatment in the BE correction.

[Specifics of diagnosis and treatment of gastroduodenal diseases associated with Helicobacter pylori infection in surgical clinic]. / Osobennosti diagnostiki i lecheniia zabolevanii zheludka i dvenadtsatiperstnoi kishki, assotsiirovannykh s gelikobakteriozom, v khirurgicheskoi klinike.

Complex of diagnostic measures in patients with gastroduodenal ulcer disease includes conduction of investigations for revealing of helicobacteriosis, dysbacteriosis, helminthosis; esophagogastroduodenoscopy with optical biopsy and biopsy for histological and cytological investigation; computeric analysis of the blood neutrophils and buccal epithelium. Conservative treatment is conducted under endoscopic control, curative endoscopy and investigation for Helicobacter pylori are done additionally if necessary. In the absence of the conservative treatment efficacy various kinds of organpreserving surgical interventions are applied.

[Application of the compression biliary-digestive anastomosis in surgical treatment of the biliary ducts obstruction]. / Primenenie kompressionnogo biliodigestivnogo anastomoza v khirurgicheskom lechenii neprokhodimosti zhelchnykh protokov.

The methods of formation and the surgical instruments for the biliodigestive compression anastomoses (BDA) doing were elaborated in the clinic. For the 1997-2000 years period 137 patients were operated on for the biliary ducts impassibility, in 36 of whom the compression BDA was formated. In 69% patients the rejection was noted on 7-10th day after the operation. The result of treatment was studied in 6 month-4 years term and estimated as good in 32 (94.12%) patients.

[Intragastric blood flow in the etiology of duodenal ulcer--new aspects of surgical treatment]. / Vnutrizheludochnyi krovotok v étiologii iazvennoi bolezni dvenadtsatiperstnoi kishki--novye aspekty khirurgicheskogo lecheniia.

In the experiment on dogs and in patients with duodenal ulcer disease, it was established that the regulation of acid production besides the vagal nerves was mediated by the intragastric gastrin transport with the blood flow. A method of preoperative diagnosis of the intensity of intragastric blood flow permitting to define more accurately the indications for the choice of a method for surgical treatment has been developed. Supplementation of the selective proximal vagotomy with circular mucosectomy at the boundary between the antrum and body of a stomach enhances the effectiveness of operation due to reduction of the antral gastrin influence on the acid-producing zones of the stomach.

[Potentialities of ultrasonic diagnosis of tumors of the stomach]. / Vozmozhnosti ul'trazvukovoi diagnostiki opukholei zheludka.

On the basis of examination of 15 patients, the possibilities of ultrasound diagnosis of gastric cancer are shown.

[Prevention of acute erosions and ulcers of the esophagus, stomach and duodenum after surgery]. / Profilaktika ostrykh érozii i iazv pishchevoda, zheludka i dvenadtsatiperstonoi kishki posle operativnykh vmeshatel'stv.

The authors have established that after different operations acute erosions and ulcers of digestive organs which are often complicated by bleedings and perforations arise in 67% of the patients. The method of complex prophylactics developed by the authors includes general measures (a rational preparation to the operation, adequate anesthesia, sparing method of operating, active management in the postoperative period) and special ones (the administration of cerucal, anabolic steroids, methyluracil and almagel). This method makes the amount of erosive-ulcerous affections 12-15 times less and practically prevents complications.

[Endoscopic diagnosis of acute erosions and ulcers of the stomach and duodenum]. / Endoskopicheskaia diagnostika ostrykh érozii i iazv zheludka i dvenadtsatiperstnoi kishki.

Under analysis was an experience with using fibroesophagogastroduodenoscopy in 114 patients with acute erosions and ulcers of digestive organs, including 33 patients in dynamics from 2 to 4 times. In 13 patients erosions and ulcers were not complicated, 95 patients had bleeding ulcers, 6 patients had perforated ulcers. The authors have shown the diagnostic informative value to be as high as 96.5%. It was stressed that fibroendoscopy allowed to determine the character and localization of the process, complications (bleedings in particular), to follow the development of the process in dynamics, to control the efficacy of the curative and prophylactic measures.
