[FEDERAL CLINICAL RECOMMENDATIONS IN DIAGNOSIS, TREATMENT AND PREVENTION OF HEARING LOSS DUE TO NOISE].

Noise induced hearing loss is a slowly developing hearing impairment, caused by occupational exposure to excessive noise levels, constitutes a lesion of the auditory analyzer and clinically manifested as chronic bilateral sensorineural hearing loss. Currently, there is not a treatment that provide a cure of sensorineural hearing loss. Regular, individually tailored treatment should be directed to the pathogenic mechanisms and specific clinical symptoms of hearing loss, as well as the prevention of complications. We recommend using non-drug therapies that can improve blood flow in labyrinth, tissue and cellular metabolism.

[On the discussion of the new aspects of the occupational hearing loss classification].

The objective of the present work was to analyse the materials of verification of the newly developed classification of occupational hearing loss harmonized with the international criteria an medico-social expertise of hearing impairment. The classification is designed for the practical application in the clinical work of otorhinolaryngologists, occupational pathologists, and medico-social experts who must practice the identical approaches at all stages of the preliminary and final diagnostics of occupational ear diseases, competence assurance, and social maintenance for the subjects presenting with hearing disorders. The authors emphasize the importance of taking account of presbyacusis characteristics at the stage of pre-nosological changes of the hearing function in the subjects exposed to strong occupational noise, i.e. prior to the appearance of the «signs of noise influence on the organs of hearing". Such an approach makes it possible to eliminate the discrepancies in drawing conclusion about the presence of presbyacusis or "signs of noise influence on the organs of hearing".

[The problems of hearing impairment in the flying staff of commercial aviation in Russia].

The authors discuss the problems pertaining to the growing incidence of hearing impairment in the members of the flying staff employed in commercial aviation of Russia and the main criteria used to elucidate the causes behind occupational diseases of the organs of hearing. Special attention is given to the principal normative documents regulating the methodological basis on which the acoustic factor in the aircraft cockpit is evaluated, peculiarities of occupational sensorineural hearing impairment and the methods for its detection. The main errors in the determination of the relationship between the working conditions and the diseases of the organs of hearing are discussed.

[Early medical consequences of the radiation accidents in the former USSR Territory].

The paper sums up the information on radiation incidents at the former USSR territory, which incidents are related to the human exposure of clinically significant effects. Within more than 60 years (since 1949), at least 356 radiation incidents accompanied by the clinically significant human exposure have happened at the former USSR territory, which has resulted to clinically significant health effects in 765 victims. This exposed cohort includes 348 acute radiation sickness (ARS) patients including cases aggravated by local radiation injuries (LRI). 407 victims were affected by local radiation injuries only. Totally, 71 radiation induced fatalities were observed within first 3-4 months after the irradiation. [Experience of contemporary treatment of radiation burns in individuals subjected to ionizing radiation].

The article presents new approach to treatment of radiation disorders presenting as indolent ulcers. The approach is based on autologous multipotent mesenchimal stromal cells applied locally and injected around the injuried zone. The ulcers heal due to the fact that the transplanted cells stimulate local regeneration processes, angiogenesis, ceased inflammation and apoptosis. The method is a new medical technology based on vast preclinical studies and could be put into medical practice.

[Mesenchymal stem cells transplantation in the treatment of radiation skin lesions].

Transplantation of mesenchymal stem cells, both at early and later stage after local exposure of rats source beta radiation dose, 90Sr/90Y 140 GR, stimulates recovery of damaged skin. Diminution area local radiation injuries and accelerate healing radiation ulcers. Clinically shows the high efficiency of the transplantations autologous mesenchymal stem cells in treatment of deep beam ulcers, intractable standard conservative treatment. Found promising application of mesenchymal stem cells for treatment of severe local radiation injuries and the need to develop the best possible conditions for their use.

[Hematopoiesis during remote period after acute radiation syndrome].

Based on the long (19.7 +/- 1.8 year) hemopoiesis follow-up study in 152 patients after acute radiation syndrome (ARS) as a result of exposure to gamma-, gamma-beta and gamma-eta radiation in a wide dose range (1.2-9.8 Gy) it was detected that cytopenia appears in the late consequences period: thrombocytopenia was found in 26.9% cases, leukocytopenia, neutropenia and lymphocytopenia--in 13.1% patients. A higher ARS degree causes the increase of various disorders (cytopenia and cytosis) in the late period. It reflects a tight interrelation between blood cell contents and radiation dose. Frequency of cytopenias increases if such somatic disorders: persistent hepatitis, hepatic cirrhosis and late radiation ulcers as appear.

[Occupational diseases in nuclear industry workers]. / Professional'nye zabolevaniia rabotnikov atomnoi otrasli.

The authors analysed contemporary work conditions, occupational morbidity and traumatism in atomic industry. Major types of the occupational diseases are listed. The article contains measures on lower morbidity and prophylaxis of occupational diseases and traumatism among atomic industry workers.
